Flour makes lump when it is not cooked . Cook the flour in butter for almost 30 to 60 secs while mixing with spoon back . Then add small amount of milk and mix it then add whole milk

Yes it's poison when you take milk nd salt together "directly" But not poisnes when you take both with other ingredients or when you put other ingredients
Tip: don't add milk right away let the maida cook for sometime in the butter only 2-3 minutes extra than normal Then add milk Trust me no lump will be formed
What I do is first add butter .... Let it melt and then add milk..... Then in some other small bowl i add maida and water to make a semi thick slurry and then add it to the boiling milk with constant stiring... If the slurry is not smooth then strain it and then add it.... Keep the milk on boil with constant stiring till it reaches your desired consistency....

Fun fact: oldies say that milk should not be mixed with salt but people now get all high priced dishes with that combo
You can mix the maidaa with chilled milk in a bowl and then can add it into pan . This way it will not form any lumps. Hope it helps.
