    It doesn’t hurt when I pinch it it’s when I walk or run and the heel is lifted off the ground I get the pain

      That is why it is useful to not rely on just one test - none of the tests (even scans) are 100% accurate - you can find more detail on how we diagnose Achilles injuries here: czcams.com/video/cNOxd3w37K0/video.html but here are some thoughts:
      1. It may still be the tendon but the part that is injured may be located in an area that you can't pinch or that the tendon only hurts when loaded heavy as in running.
      2. It may also mean that you feel the pain in the tendon but actually it is referred from a structure that is located behind the tendon e.g. the posterior ankle joint or bursa

    Mine hurts on the back of the heel and over to the lateral side towards the ankle. Ugh. So painful.

    The crazy thing with my tendons are that pains me when i woke up from sleep and only for a while..all day long to problem!2 months now and still have this pain!

      Calcaneal apophysitis is also known as Sever’s disease and is when the growth plate where the Achilles tendon attaches into the heel bone of a growing child becomes inflamed and painful.

    Yesterday I had small motorcycle tip over. The rear soft pannier hit the back of my achilles tendon and calf, the pain is mostly in mid and upper section. Also there was small indentation in my achilles in mid section. I have 700km to travel to my home place. Today I was resting all day, also did 3x10min frozen peas 😂 on it.

    I have stiffness in the "upper" part of the tendon. Actually where the gastrocnemius muscle arch begins. Is it still achilles tendinopathy or something else?

      Yes, that can still be Achilles tendinopathy - it is just not as common to get it in the high part of the tendon.

    Hey, I was labeled today with tendinitis, but the sharp burning pain is more extreme than a knee tear I’ve had , I wanted an mri , can tendinitis be that way? It’s constant pain and to the touch it’s very sharp , I’d love your advice you’d know much better than most doctors available with your experience I see your very great help

      1. Yes, sometimes tendinitis can be super painful but other things I would want to rule out include
      2. Gout
      3. Bursitis

    Mine started after I wore uncomfortable shoes for several days. In fact, pain isn't what bothers me the most, as it is moderate and pretty bearable, but I feel something like twitching when I try to walk or move my feet, as if there was sort of friction in that place (feels like creaking but without a sound)

      That can happen when the sheath that is around the tendon gets a bit of swelling etc. and creates friction so that the tendon doesn't glide smoothly in it.

    So I supposedly have Achilles tendinitis, it's what a podiatrist said. But what I have most of my pain is around the bottom of heel and radiates around the left side. I have it on my left food. I feel like it's more plantar than tendinitis. I feel like I'm getting no relief. It wasn't constant pain before and now it is. I've reduced my exercise to only yoga right now. I ice, roll ball, use stretch contraption. And meds don't touch the pain. Any other suggestions. I do the strengthing exercises too.

      It is very unlikely to be plantar fasciitis because the pain from that does not go in the areas you describe while the pain from insertional Achilles tendonitis can do that if it is aggravated enough.
      However, I have obviously not assessed you so my advice may not be correct.     It hurts. My girlfriend is in physical therapy and we did some light stretches, and she did something called scraping, omg that was painfully 😓 my whole calf feels loose now and not tight but it hurt and surprisingly very loud

    I have achilles pain at the (back of my foot) can i know what i have ? It occurred after an ankle sprain. Anytime i aply pressure on the ball of my foot I feel a sharp pain behind my achilles tendon, anytime i try to do calves raises it hurts and i iced it everyday but still no changes? Im from 🇯🇲 I need help please

    These videos have outstanding informational content, and your thanked for having them available for us ailing with our issues. I returned yesterday from a beach vacation where I was walking in sand and in flip-flops for many miles each day. It was bothering me a little bit while on vacation, but I returned yesterday and this morning could barely walk with my left insertional Achilles being very sore and tender. Just flaring up today should I start exercises immediately or wait a period of days? Because this has happened, will it increase my likelihood for a full tear or snapping of my Achilles if I enjoy playing tennis squash, and Pickleball?

      Repeating this in case you don't see my other comment - no please don't start rehab yet - it needs to settle down first.
      Tendinopathy goes through stages - it sounds like you're in the reactive phase which (if you've never had tendon pain there before) means that your tendon is acutely irritated. If you allow it to properly settle down, it will usually recover relatively quickly and you have no real risk of rupture. However, if you continue to irritate it and it starts dragging on for several months, then it enters a disrepair and eventually degeneration phase where the tendon starts to lose strength - then you have an increased risk of rupture.
      In your case, you don't yet need rehab exercises - you need to let it rest first and then start with super gentle moves.

    Thank you for that excellent video. Currently suffering from Achilles Tendonitis (right leg)) four weeks after a 10mile race. Have been running for about five years, normally 5K park runs and 10Ks, however my new club encourages entry to a multitude of events (with very little training advice or personal considerations) and the 10miler was extremely hilly (almost 1000ft of ascent), although I posted a decent time (for a 56 year old man), 1hr 14 mins, I didnt give myself the recovery time required. But you have hit the nail on the head with everything you have said in that vid, confirming all my thoughts and current scenario. I have previously have had a full achilles rupture (left leg) about 9 years ago while playing squash, so I know what that is like. But onward and upward, as a non drinker, non smoker, I'm hoping to keep running competetively for at least another ten years. It is amazing the amount of differing opinions I have received from various physios. I hope they all get to watch your videos, sometime soon! I have tailored my exercises accordingly to the morning pain/stiffness level of my foot. One question : do you agree with collagen supplements to assist with recovery?

    I have achilles tendinosis on both ankles...swollen and painful for months. Stretches or loading makes worse and affects hips and so on. Is laser therapy good to help heal so i can gently sttetch? Thanks

      Stretching is the least helpful exercise - it usually just makes it worse so I would avoid that until you're built good strength. Laser may be useful but it is a bit hit and miss with regards to results. Strength training exercises are important but they should be set at the right intensity and be done in a position that your Achilles tolerates - if yours are making your pain worse, it usually means that they are not the right ones for you.

    Hello my friend (Dutch or South African? That accent sounds like mine). I am a mountain biker who rides in quite an extreme way. Lots of jumps, drops, stairs, impacts and a lot of force going through the ankle joint as I weigh about 100kg and ride very often. Two years ago, I sustained a heavy impact to my left heel during a crash, resulting in (what I suspect is) a degree of compression in the joint between my calcaneus and tibia. A few months later, I partially tore my ATFL. Both healed and I was riding again, although with low grade chronic pain (especially in the cold). Recently, I have developed insertional Achilles tendonitis in the same ankle and I have been forced to take time off the bike to rest. What sort of rehab exercises should I perform, if any? Just elevated heel raises like you said?

      Hi John, if the pain is on the heelbone then it sounds like an insertional Achilles tendinopathy. It would be good if you could consult a physio to make sure that it is that and then prescribe a specific treatment plan for you. But in the meantime don't do any calf stretches as that can often make it feel worse. It would also help if you can wear shoes with a heel or place a heel lift in your shoe to take strain off the Achilles. This is also something that we could treat online via a video consultation.     I dont run or jog, yet ive have archilles pain in heel several times a year.nit just comes on and pain lets up in a week. The painnis awful like a knife stuck in back of heel. I stretch and ice but nothing, take advil ..nothing.

    fantastic info! would have loved to know is there are differences in treatment depending on where the pain is. I have pain at the insertion, like you mentioned it hurts when pushing on the back heel, after using ice the pain gets better, but walking brings it back again.. how to heal it completely ?

      Those types of sensations are usually due to nerve irritation - so in your case something must have irritated the nerve that supplies that area.     I have mid portion achelestendonosis! My heel hurts every morning there and I can barely walk!!
    But on my right heel, wrapped all the way along my bottom side of my back half is bruising!!
    Up my legs, like if I was wearing socks pulled up like soffit socks, is red coloring, but it's not sun tan...
    Do you know how to help me, please!!

      I think you need to show that to your doctor because that is not normal for Achilles tendonitis to cause changes in colour and going up your leg.

      It may be something to do with your shoes then - are they perhaps pressing on the tendon or something else in the shoes that are irritating it?

    I'm obese and I've had an old injury in Achilles tendon that is long ado healed, but every time I try to walk every day or jog (to lose weight), the tendon gets painful again as the exercises accumulate until I either tone down or stop or get another injury... any advice?

      That is actually easy to overcome - you just need to take more time to slowly increase activity.
      Why it happens:
      1. At any given time your tendon is strong enough to cope with the loads (type + how much you do) of the typical activities you do in a day - this includes your day to day walking etc.
      2. If you now suddenly start to walk or run on it and does it every day - this is a large increase in load and your tendon is simply not prepared for it so it gets overworked and flares up.
      How to fix it:
      - When you start your fitness plan, only train every 2nd or 3rd day so that your tendon has enough recovery time between sessions and it can get used to that load
      - Start with only a bit more than usual e.g. walk at easy pace for 30min - speed walking for 30 minutes may also overdo it if you're not actually used to going for continuous walks.
      - And then you build it from there

    I went on my tiptoes and felt a rip in that tendon, i cant go back on my tiptoes cause it hurts and hurts to walk for too long but when i feel it and poke like the video says its not painful. So im confused

      That can happen - you then adjust the treatment and treat it as if it is insertional (because that is usually most easily irritated) e.g. start with the exercises just to floor level and avoid calf stretches etc

My main pain is at the heel like you showed but I also have the bump higher where you also pointed. That’s tender to touch but it’s the heel pain that hurts when I walk. Can non insertional cause that heel pain too? Or can you have bad luck and get both at the same time? I just woke up with pain last week no injury etc or heavy activity. :(

    I recently played basketball two days in a row and on the second day at the very end my Achilles was feeling slightly sore. The very next day my Achilles was hurting only when standing and walking but if I lift it, it doesn’t hurt. I never had any pop noise or bad landing while playing bball so is it just a strain? Thank you❤

    i workout at home ani workout at home so i started HIT withot wearing shoes and suddenly this pain comes up , i neglect that and it increase , i feel pain early in the morning and the time my leg is in the rest and then i started to walk and it goes away in few minutes, and when i stretch its all good and i also realize when i stretch second time at night and use some balm it reduce inflammation and relieve pain so i feel much better in the morning and pain like get better,This pain is from several weeks so do i have to stop doing cardio or weight lifting or HIT? and how long does it take ?

    Hi there, I have had sensitivity in my Achilles tendon, from the lower insertion point up to the upper insertion point. I don't really get any issues walk or hiking, but my issue isnwith wearing tight socks or any shoes or boots that touch off the tendon. It calms down of i leave it for a couple of months, bit it flares straight back up again as soon as i try to put on a new pair or shoes or boots. Ive no idea if o should just avoid wearing things that annoy it, or am I missing something. Any advice would be greatly appreciated 😊

      That doesn't really sound like tendonitis because the pain from tendonitis is linked to intensity of activity. Yes, it can get irritated if something presses on it but usually it would also be painful with activity. I am wondering if you have something else going on there e.g. varicose veins or something that is more sensitive to pressure. If you want to really figure it out, the best way is to get it assessed and see what is causing the pain - an ultrasound scan can perhaps also help.

    Wonder if you can help? I don't have all the typical symptoms (mid portion) but it hurts most to jump/hop - the main aggravator. The pain came on gradually in the weeks that followed from a race and I felt at first that I i had strained my calf muscle. The location of the discomfort for the most part is very much the upper limits of the tendon, as if where it branches into the wider calf complex. It made me wonder if it was an Achilles issue at first? There's a slightly puffy feel to the main part of the tendon and stiffness in the morning but I can walk fine and typical calf exercises to help it aren't hurting. However running is still a no go after around a month (especially as hopping is an issue). This feels like a 'minor' version of an achilles problem in the grand scale of things but yet it wont budge. Hugely frustrating. Looking to try shockwave therapy on it soon as I've had success with this for other issues. Thoughts on all of this?

      It sounds like you may have tendinopathy in the very high portion of the tendon, where it starts to merge with the muscles. The treatment for this is similar to that of mid-portion tendinopathy. Yes, it sounds as if you have a mild case but even mild cases often take at least 12 weeks to get back to running, so hang in there.

    Hello!! I have pain in heel bone like a knife.. It started 3 years before and continue until today (im profesional football player and nobody until now can understand full what is going wrong) before 2 days in the game i felt one small crack and now i cannot walk.. In the next days i will go for MRI.. But i feel the pain only in the heel bone when i strecth and especially when i touch the bone. (crazy pain)

    you never said why it matters where you feel it like what causes one or the other and if there are different treatments
    both mine are fucked they feel like theyre shrinking and stretching makes them tighter later in the day

      The reason it matters is that you have to do different exercises for the one type vs. the other. When it is injured at the heel bone, it doesn't like being stretched so any calf stretches or even doing calf raises over a step can make it a lot worse. Whereas for mid-portion it is usually OK and better to do the calf raises over a step.
