Grenades in Tarkov aren't the most consistent thing, but it is possible to get it down with enough practice. and uh.. try not to ruin my favourite map too much (please do)
[IF YOU HAVE A HIGHER STENGTH LEVEL]
Either aim at a slightly lower point on the ceiling (since you throw grenades further) and stand either on the "X" or touching the back wall below the stairs. Preferably the was if you can throw it far enough since it's easier to stand in the same spot repeatedly if you're against an or stuck on a piece of terrain.
[GRENADE CHOICE]
I mainly recommend using either F1 Grenades or RGD grenades. VOGS are abit inconsistent with there explosion radius and sometimes the PMC can survive the blast but can still work. Impact grenade are pointless since a normal nade explodes as soon as it lands on the otherside of the window anyway.

Wierdly enough i got this recommended, its pretty cool that line up, but i tried to test it with my pmc, i started wondering can you get line ups on the other windows.
and i found one thats WAY EASIER to line up and falls in the hallway between the doors of the safe room.
1) go to same exact stairs, in this video, but go to right side,
2) let your left "shoulder" be aligned with the hand rail and strafe left so they touch (behind you there is a black line on the wall, also an indicator)
3)when aiming with your hand if your thumb's tip is flush with cilinder >| , then depending on your strength go up or down (with 16 strength your thumb should cover the window also)
4) happy fragging

Posting another reply. Finished Grenadier in less than an hour using this method. Got 2 double kills and a triple kill. Definitely worth resetting after a couple minutes of no activity if you have factory exit key.
If you have a crosshair built into your monitor, grenades do not seem to suffer from the "sway" of your hands or weapon, so using the cross hair results in perfect grenades. Used it earlier to throw a grenade directly between a 4 inch gap and get a flash grenade kill. Was hilarious.
I almost hate to like and comment on the video as it will promote this to more people. This needs to stay a secret. I'm gonna go get grenadier done today when I get home, lol. This should be super easy for people that have a monitor cross hair. Just turn it on and use it's fixed position to throw grenades. I'm not sure, but I wouldn't think grenades have the same type of sway that normal weapons do.

How do you know if someone is up there at the exact time you throw a nade? Or do you just randomly throw one up there and hope someone is up there at the same time
I'd usually just get to the spot, stay quiet and listen out for them heading up. But If I got a bad spawn, by time I got there I'd just throw one up just to check. Plus If someone's fighting up there you can see their muzzle flashes. And people just like generally stand there and hold the other side of the hallway.
