Why crown area transplant is not always great? Do you know that crown or vertex transplant is most demanding and technically most difficult? Even doctor thinks many times before crown transplant.
so if you are thinking about crown area transplant then this video is a must watch for you..
Lets start this video with a case study. One army officer mr anup has approached me for hair transplant, his age was 31, baldness grade was 6. one fut was done in 2013, which failed, now he was using wig.
i called him for consultation. in my opinion consultation is most important step before transplant.
Why, because when patient come for consultation then many tests, trichoscan, scalp examination can be done. by seeing image a doctor can understand only few things about bald condition. any way he came for consultation, i did test and trichoscan of different part of body like beard, chest and safe donor area. his hair quality was good. we planned for 6500 grafts. bio fue no touch technique and body hair extraction techniques were used and in 3 days 6600 grafts transplanted.we extracted hair from safe donor area, bearded chest. his front look has improved just in 4 months, while crown area was still growing even after 4 months. it took complete 9 months for crown area to grow. now this a question to ponder upon, if the patient, doctor, team was same then why front part took 4 months while crown area took 9 months.so now you can see why all doctors do crown transplant after a lots of thoughts.lets understand why?
A.difference in inbound vascularity- it is observed that crown area vascularity is different than frontal and temporal area.this is one reason for slow and less growth percentage in crown area.
B. gradual progression- in crown area baldness keep progressing in circular shape.so if transplant is done in immature age then after some time area of baldness will increase , and create an unnatural looking halo around transplanted area.this is another reason for hair transplant after 30 years, because by that time either hair loss got stabilised and progression either stop or slow down.
C.because of gradual progression that person may need second or third transplant in short time.this is 3rd reason for hair transplant after 30 years.
D. Presence of whorl- creation of whorl is not easy for inexperienced doctor.
E. Poor visual density- in crown area hair is in two direction horizontal and vertical, so hair do not overlap in this area and even small baldness appears big.
F. Hair in whorl splay , which further expose bald area.
G. Whorl creation and excellent result require extensive number of grafts.
H. Insufficient graft- frontal transplant is always priority.so if a person needs frontal and crown both to cover and if graft is limited then frontal transplant is done first, not crown. These are the technically difficulties in crown area transplant.
then who is ideal candidate for crown transplant.
A. if age is more than 30.
B. crown area baldness is stable for 2 or more years.
C. medical treatment tried but failed.
D. genetic history of baldness in crown area.
E. donor area is more than sufficient. so that if in future transplant in frontal and mid scalp required, it should be doable.
PRECAUTION
1. Take medicines sincerely, because shock loss is heavy in vertex area, if possible start medicines few months in advance.
