If you are submitting a claim for PTSD, then yes, you will need to complete this form. I highly recommend getting a diagnosis for your exact condition first. Also, please be sure to complete the correct form, as there are two types of PTSD forms. Good luck!
@@VictoryforVets my form is VA Form 21-0781a PTSD secondary to Personal Assault. I’ll get the diagnosis before submitting. It is documented in my service record also so the stressor is there. I’ll get there one way or another.

@Panchitobear it is best to talk about each condition separately, if they are different. Example, migraines are very different from knee injury.... but maybe your knee injury is bad and also causing back issues. I hope this helps! Good luck
Thanks for your quick response, one more question , on Va 21-4138 , do I need to put in statement for example Insomniac or depression , a words “ this is a secondary condition due to tinnitus” for each form ( insomniac and depression?

You should ALWAYS have a personal statement. This is your voice to the VA rater to advocate for your claim!
I do have 4 different disability claims, do I need 4 separate form 21-4138 for each claim? Thank you if anyone can help
