Man, this makes me miss my old unit. Several of our core founders were first responders, Navy corpsmen, etc. in real life, so we went all out with the realism regarding medical mods (ACE Medical, KAT Med/Pharmacy). I thought it was great honestly. Medics actually played a critical role which also meant that CCT/TACP/FO actually had to maintain critical airframe deconfliction while coordinating with leadership on CASEVAC ops in addition to their other planned mission sets, and there was a HUGE incentive to not get shot or blown up, so even lower level leaders exercised more discipline tactically. The way we played, if you received critical enough injuries for you to have to be evacuated to a higher level of care, you could only be re-inserted at the convenience of aviation and ground leadership as a basic rifleman reinforcement. Depending on the operation though, you might not even get another opportunity to be re-inserted.
We emulated a SEAL platoon, so we did a lot of recon, recon in force, UW ops, highly coordinated DA raids, SDV/Dive/littoral operations that basically made re-inserts impossible until much of the OP was already over (if done properly). If you messed up, got shot, blown up, mixed your air improperly (ADE - Advanced Diving Environment), etc., it was pretty common to spend 20+ minutes unconscious while being evacuated, then spend another half hour on a Blackhawk door gun providing very limited ISR and mediocre support by fire while waiting for an opportunity for a safe LZ to open up after much of the action was already over. Most of us were pretty well practiced in our small unit tactics though and made efforts to avoid circumstances beyond our control (no Rambo BS), so at least among our core group, mass casualty events were pretty rare. We were pretty tight on our sectors.
I think it does really depend on the types of missions your group prefers to construct for themselves though, whether or not these highly technical mods are worth it. We were a small unit of about 20-30 people generally. And our operations were highly technical in nature, requiring either a whole lotta patience or a whole lotta precision in a short timespan. My absolute favorite missions were actually those in which I didn't even fire a single shot. I played for quite some time as a CCT attached to the SEAL team, so I spent many operations doing mostly nothing but chatting with with flyboys. For me, the coolest missions were those that involved logistical challenges like HAHO dropping in to conduct area reconnaissance to establish a hasty airfield in order to direct C-130 supply drops ahead of the main assaulting force. It's pretty damn cool to set up IR stobes in virtually zero light conditions on a really tight dirt airstrip and successful land multiple large airframes.
Recon and intelligence gathering in general is really my jam. When my old unit would build campaigns, a huge part of the beginning was always just how to establish a base of operations within the AO, gathering intelligence, establishing realistic supply lines, etc. Before our main operations on Sundays, we very often hosted weekly training and Recon pre-OPs in which small teams would insert ahead of the main force with the goal of just gathering information. We used Hate's Digital Camera and Electronic Warfare mods for collection of IMINT and Signals Intelligence, had multiple Zues operators role play for the gathering of HUMINT, had formal request forms we could submit to the mission builder regarding other forms of INTEL (OSINT, MASINT, etc.). We rotated mission building weekly, and creating those narratives was a TON of work, but man, it was pretty damn cool to see how people would compile information and utilize it in mission planning later on. We even sometimes created Twitter accounts for characters that players were meant to monitor for clues and include in their reports. It's pretty wild actually thinking about the many, many hours we spent each week building these campaigns. It some ways, it really did become like a second job, which is kinda why eventually we decided to take a break from Arma after over a decade of playing together. Sometimes, you need to take a break to find the simple joy of playing again without the pressures to provide a highly curated experience for others.

Ah man, something that I never learned was the advanced treatments that came with the KAT Advanced Medical "Pharmacy" update. It tweaks how you can defibrillate someone, and I just never learned the new conditions

Advanced flight model is like a drug, once you start even if you hate it you cannot stop. Literally as a little bird pilot when 6 people boards it that shit becomes so heavy i cannot fly straight with full throttle

lol my unit use Ace+KAT medical and I just did medic training JFC it hurt my brain but amazingly got the hang of it after a while. We are not hardcore but we have a bunch of paramedics and nurses in the unit so...

Sniper here. ACE advanced ballistics is the best sniper mod everrr. It requires you to really learn how to set up a shot and what factors to consider. Different ammo has very different performance at long distances. Choose your weapon and ammo wisely. Consider atmospheric conditions (pressure, temperature, humidity), latitude, wind / gusts, ... , to set up your shots. Go on a shooting range and program the AtragMX BEFORE you use the weapon/ammo combo in a mission. Weapon performance will be very different at sea level vs. e.g. the Takistan map, which is at 2000+ m altitude. AWM / L118A3 is very gg. Learn your stuff and the reward of hitting the perfect shot is amazing. If you have never used advanced ballistics and did in-depth training you have not earned to be called a sniper. Honestly, one of the mods never reached and probably not even tried by any other game!
ACE medical is pretty fine, too even for softer milsim. Makes a hurt teammate something to consider in your missions. It can easily take two others out of a fight to save an effed up teammate from certain death. But then again it's super tense in a firefight when you have to treat a buddy and your other mates try and protect you.
Stay low, go fast. Kill first, die last.
One shot, one kill. No luck, all skill.

So a little comment on the “no hud” thing. Specifically in regards to not having a crosshair
I’ve actually been thinking about this a lot lately on whether having no crosshair truly is what is “right”. And I’m confident in saying that no it is not.
Think of the situation in real life. When I shoulder my weapon I have two points of contact. My shoulder. And my guiding hand. Both these together make an approximation in my head of where the barrel is pointing. Even beyond that there is alot more sensory info your body gets to make that “approximation” in your head and make an invisible outline what we call the “crosshair” in the game in real life.
Now in the game. You have none of that sensory info. And really. You have no idea where the barrel is pointing because you lack that shoulder and holder hand info.
Because of this I think having a crosshair as part of the hud actually make sense from a realistic perspective. Especially to make up for the games lack of ability to give you that sensory info and make that approximation for yourself.
Otherwise, you don’t really “guess”. You just end up spraying in hop fire and not adjust your sun off the weapon but off of where you bullets are hitting. And adjust your hipfire to that. Not actually how you are aiming the weapon

You mean like having a visual representation of "approximately in the middle of the screen"? Unless you play with the wibbly wobbly aiming in Arma where the aim isn't locked in the middle of the screen, you always have a way to make an approximation by just using your eyes and gauging where the middle of your monitor is.

That's Nametags from ace3 mod. There's some settings you can play with to get those to work as intended.
They already baked in well in Arma 3 Remastered on my Discord Server, too.

If you had basic medical training as a paramedic, military, or law enforcement then ace medical is a piece of cake, otherwise you’re going to be lost in the sauce 🙃 but me and my guys who I served with prefer it

For everyone interessed in these realistic mods but are afraid of the medical stuff, find a good unit that teaches it carefully, it's not that bad if you're playing with a good amount of players and if the basic of ACE and Kat have been teached

Have you seen the mod AWR - Advanced Wound Reaction? Developed by a friend to make ACE unconscious more realistic, meaning that it does not always happen. Instead people will fall to the ground and drop their weapons and in some cases they can still pull out their gun and have a last stand before bleeding out.

ACE med isn't controversial because of hardcorness, ace is controversial because it's basicly gives you immortality in compare to vanilla arma
That's why many ppl jump for more hardcore and immersive systems like Project injury reaction mod, where simple shrapnel can completly knock you off
Where in ace you could take 8 gunshots and still the only consequence would be bleeding that could be stop in less than a minute
What a load of crap. If that's been your experience with ACE medical then you either didn't touch the settings, or you just had no idea what you were doing.
Thats fair. Ace addon options does help with that.
@@nannertime I wouldn't be so sure about that, ace's options were really binary looking - you either go into a coma bc of scratch or be turned into a battle manga character, no in-between (and that was for the old version of Ace, the new one doesn't even have that).
Try KAT Medical. It's ACE Medical on meth.
