      Ok I have been diagnosed as having mycoplasma genitalium and prescribed Levotop500 (levofloxacin tablets) for a duration of 30days. What can you advise me? Thanks

    • @DrBretPalmer

      @@brigitteebombo6131
      In the UK, we advise doxycycline 100mg twice a day for 7 days followed by azithromycin 1gram on day 8 then 500mg of azithromycin on day 9 an 10.
      If you have symptoms then this can be changed to Moxifloxacin 400mg once a day for 14days.
      But depending which country you are in, resistances may be different. Levofloxacin is a different choice from the UK, but if your doctor has had previous success then see how it goes.
      You will need a test of cure 5 weeks after starting treatment.
      In the UK guidelines we don't use Levofloxacin.
      But which ever you use levo or moxi, do not do heavy lifting and if your joints or tendons ache then stop using and report back to your doctor ASAP.
      Hope this is useful.
      Take care

    Hello doctor im 7 months pregnant and got diagnosed with this two months ago. OBGYN was only able to give me azithromycin since all other antibiotics to treat this aren’t safe during pregnancy. I’ve taken this medication twice already and I’m still testing positive: what other medication can I take? Or do I have to wait until after pregnancy?

    • @DrBretPalmer
      @DrBretPalmer  Před 2 lety +2

      What your doctor has done I agree with I'd do the same. You doctor may treat you again.
      After pregnancy you should be treated again but if you are breast feeding you must not use doxycycline as can cause discolouration of the babies teeth.
      Any antibiotics you take check with your doctor that ups safe to take either in pregnancy or during breastfeeding.
      I hope all goes well with the pregnancy.
      Take care and hope the advice above is helpful.

    So Doc, I tested positive about a 2 month ago’s, Did treatment (partner did also) tested again and was having symptoms (dysuria). The results came back negative for mgen and other routine tests. I then had intimate moment with partner, tested again two weeks later and it came back negative for mgen and also other routine test. The symptoms of dysuria are still going on. Is this a lingering effect?

    • @DrBretPalmer
      @DrBretPalmer  Před rokem +2

      Hi Kreed,
      Thank you for your question.
      Dysuria can stay for a while. Apart from STIs, other simple bacteria from the body can also cause dysuria.
      As its the summer simple dehydration can also cause it as well..... something I've recently rediscovered in the hot weather!
      My advise is what is the colour of you urine... is it dark? Or worse in the morning. If so up your fluid intake and see if it clears up.
      Guys can also cause dysuria due to over self examination, too tight a grip when masturbating, and over interpretating pain signals then would have previously ignored.
      You have good reason to be concerned as you've just cleared an STI, but that's the past. Drink lots of water, stop all self examination and milking and it should resolve itself...... easier said then done.
      Hope this is helpful and your symptoms resolve.
      Kind regards
      Dr Bret Palmer

    I was on doxycycline and moxifloxacin with an initial 3 day dose of azithromycin after a failed treatment using moxifloxacin alone for 2 weeks. After the doxy-moxi combo for a month it came back negative but I still have some discharge in the mornings and slight but diminishing testicular pain. My infectious disease doctor prescribed another 3 months of doxycycline and azithromycin as my test was negative. Thoughts on some lingering symptoms while others have disappeared?

    I am about 12 days post my doxy/moxi treatment.
    The symptoms are no where near as bad as they were, but they certainly haven't dissappeared fully.
    Still get a few random pains during the day, and it's embarrassing to say, but masturbation really flares it up and causes burning.

    Doctor, please help me with this question… would the presence of MGEN in a male who has a rare variant of Glucose-6-phosphate dehydrogenase (G6PD) deficiency be dangerous or deadly?
    MGEN showed in my blood test (I have no symptoms and must have had it for a year now) and I’m worried I past it to my new partner who has the G6PD deficiency.

    • @DrBretPalmer
      @DrBretPalmer  Před 7 měsíci +1

      Nope it doesn't.
      Also a blood test will not tell you anything apart from you have or had it in your life. You will need to do a urine test or swab test to see if you have it now.

  • @Rumblingbelly
    @Rumblingbelly Před 8 měsíci +1

    Question for doctor: MGEN was tested negative after first having symptoms for 4 months. Now it has been 2 months from the negative urine test but the opening of my penis urethra is still red and too sensitive to touch. How can the urethra still be inflamed if MGEN has been negative for 2 months? The redness around urethra opening doesnt react to any hydrocortisone or tacrolimus so it seems to be an urethra issue and not just skin redness. Is it possible MGEN caused permanent damage to urethra and that is why its showing symptoms? Is there any medication that can heal the urethra?

    • @DrBretPalmer
      @DrBretPalmer  Před 8 měsíci +1

      Thank you for the question. MGen is negative so we know its not that. From what you have said, it sounds like there maybe a fair bit of self examination going on, would this be correct?
      If so, then stop all self examination for 2 weeks and see what happens.
      Also stop all coffee and alcohol as these can irritate the urethra and don't use any soap, just use water, can use an emollient.

    Hi there, I am a male currently on treatment for MG taking a week course of doxycycline and then a week of moxifloxacin. Unfortunately I have transmitted this to my female partner. I will be seeing my long distance partner 2 days after I finish my full treatment. My partner may still be on treatment for her MG. My question is, if we use condoms for safe sex will be okay to give unprotected oral sex to one another? As it’s mycoplasma GENITALIUM I assume the bacteria is only in the genitals and not in the mouth? So my partner wouldn’t be able to give me MG again through oral sex? And I wouldn’t be able to contract MG by giving her oral sex despite her potentially still being positive. Only through vaginal sex? Please could you clarify thanks.

    It was only later that I started to receive drug treatment, and now the doctor prescribes the drug with 10 days of Doxycycline, then with 7 days of Moxifloxacin , But I still haven't cured it, and the test is still positive, My symbol is that every morning when I go to the toilet and squeeze the glans, a drop of yellowish liquid will come out.

      Hi there.
      The antibiotic, moxifloxacin, will treat Mgen. It should kill all of the bacteria. However, the body take a little time to clear the dead bacteria from your system.
      So any test of cure for MGen, should be done ideally 5 weeks after the start of your treatment. There can be residual inflammation which may last longer. But also important to stop self examining and keep hydrated.
      I hope this is useful

    Hello doctor
    I live in Spain and I was disgnosed with Mycoplasma genitalium this January (2024) and I got it via oral sex because the person has it in the pharynx( or somewhere in the neck I don't really know).
    The symptoms were pain when peeing, discharge and inflamation on the prepuce (I still have a scar from it).
    After 4 months I am still fighting with it and the urologist told me that most probably I will have to live with it and this makes me feel really bad!
    So far I had 3 treatments:
    22 Jan-28 Jan 》moxifloxacino 400 mg 1/day
    After 3 weeks and 2 days:
    21 Feb- 1 March ( 10 days) 》 doxiciclina 100 mg 2/day
    After 6 weeks and 4 days:
    17 April-30 April (14 days)》 moxifloxacino 400 mg 1/zi
    I don't know if I still have the mycoplasma but I still have pain when peeing (not all țhe time) and I have discharge in the pee ( like a white liquid).
    I made an appointment for the next week to redo the analysis/tests.
    The urologist told me that the next tratament (if I still have the mycoplasma) will be moxifloxacino for 1 month.
    Do you think that the Mycoplasma already has resistance to these antibiotics?
    What should I do?
    I am feeling really frustrated and I hope to cure it!

      The absolute risk of tendon rupture is very small and rare, and more common with other antibiotics.
      I'm assuming your not going to the gym and just done legs and calfs?
      They may just stop it and see how you go. Tenderness should go after a couple of weeks, may be prolonged 6 weeks but if you rest and swim, should recover quickly.
      Must discuss with your doctor, see what he says. Not many other treatments, may just wait for 5 weeks and retest. Other meds I'm the same class may also make the tendons tender.
      No gym, cycling, running. Swimming should be okay as takes the weight off the tendons.

      Hi Steve,
      No problem.
      Most people can natural clear it, this is most acceptable to people who don't have a partner and not in pain.
      You could go onto an extended course of Doxycycline. Various reasons why they have put you on moxifloxacin. They are probably hoping the Doxycycline will get rid of the ones resistant to moxifloxacin, then given moxifloxacin to get rid of the ones resistant to Doxycycline.... thats the idea. Of course what do you do if some is resistant to both!
      From what you have said, I'd suggest speak to your clinic straight away, give them a call.
      You should stop the moxifloxacin if causing tendonitis, you may be called in to be examined.
      Let me know how it goes.

My doctor already gave me azitromycine antibiotica, but it didn't work, now he wants me to use moxifloxacin 400 mg for 10 days. I am so scared to use it , because of my pregnancy. My midwife says do not use moxifloxacin, but my doctor says mycoplasma Genitalium will damage the fetus more then moxifloxacin. I really do not know what to do. Please can you help me. @DrBretPalmer  Před 2 lety +1

      Okay, in the UK moxifloxacin is not used in pregnancy and most guidelines around the world say it is contraindicated as can cause arthropathy. However some medication websites say it can be used if the benefits out weigh the risks.
      The UK guidelines tend to just use Azithromycin and then leave it at that.
      Also general advice is if no symptoms then leave until after the birth.
      If you are in Holland the do have sexual health centres.
      I'd advise to contact:
      The Centre for Sexual Health (Centrum Seksuele Gezondheid Haaglanden) offers tests and check-ups for STD free of charge. A referral by your GP is not necessary and they should be able to assist.
      In the UK the harms of mycoplasma genitalium to the baby are not considered great as the evidence is very poor for such a common infection.
      I hope this is useful.

My partner probably also have it, if we don't treat it and still have sex, would it make the mycoplasma worse? Like more bacteria and maybe in that way, it can harm the baby? I hope you understand what i mean. Hahha my English is very poorly.

    • @DrBretPalmer
      @DrBretPalmer  Před 2 lety

      Nope your English is better than my Dutch.
      Some people have extended Azithromycin course for 6 days.
      You won't make it worse but you are probably giving back to one another.
      There is also a possibility of naturally clearing the infection if no sex but this can take 6 or so months.
      The evidence is weak about mycoplasma genitalium affecting your baby, but there is a risk of pre-term deliver and a low weight baby.

      Resistance usually comes from incomplete courses. Levo and Moxi are both good antibiotics and resistance in one may not guarantee resistance in the other but this also depends on the bug as well. The smaller the bug the quicker resistance can form (very general rule and depends on how many proteins are on the outside compared to the inside, gonorrhoea easily becomes resistant, syphilis with no outside proteins is still sensitive to the same antibiotic discovered in the 1940s).

    What is Sexually Acquired Reactive Arthritis, and what are the symptoms?

    • @DrBretPalmer
      @DrBretPalmer  Před 2 lety +1

      Reactive arthritis is a condition that causes redness and swelling (inflammation) in various joints in the body, especially the knees, feet, toes, hips and ankles.
      It usually develops after you've had an infection, particularly a sexually transmitted infection or food poisoning.
      In most cases, it clears up within a few months and causes no long-term problems.
      Men and women of any age can get it, but it's more common in men, and people aged between 20 and 40.
      Symptoms of reactive arthritis
      The most common symptom of reactive arthritis is pain, stiffness and swelling in the joints and tendons, most commonly the knees, feet, toes, hips and ankles.
      In some people it can also affect the:
      genital tract - causing pain when peeing, or discharge from the penis or vagina
      eyes - causing eye pain, redness, sticky discharge, conjunctivitis and, rarely, inflammation of the eye (iritis).
      You should get a sexual health screen ASAP, if you think you may have SARA.
      I think I'll do an episode on this in March.

      @DrBretPalmer  Před 3 lety +1

      Ideally you should have Moxifloxacin which would be better as would treatment Mgen and PID. Then repeat test 5 weeks after to make sure you are clear of infection. Also need to make sure partner is treated. The longer you have PID the increase in chances of infertility but everyone is different.
      Make sure you have recovered from PID then try for children. If no luck in falling pregnant after a year then speak to your GP and/or your fertility service (depends on which country you live in).

Then in January I tried to contact the sexual health clinic that referred me to the GP. I went to the GP, and he said not to worry about ureaplasma, devalued the infection and said that everything was fine with me, even though I said I had pain (similar to menstruation pain). Thank you for getting in contact.
      Usually ureaplasma urealytium is completely harmless and guidelines say not to treat it unless there is no other cause.
      I'd go back to the sexual clinic and talk to them about the pain your in. If you've had a full sexual health screen including mycoplasma genitalium and its all negative and you have been cleared for PID, then go back to GP to discuss further investigation of the pain you are in.

Both trestment helped but then symptoms came back 2 weeks after. Now 3rd treatment is 7 days of doxy followed by 7 days of moxi, hopefully it works. I was wondering if I should ask for 10 days of moxi instead, to make sure. Still waiting for the resistance test, should take 1 month, I’m in Canada. If treatment fail, next one should be Pristinamicyn if I follow Canada’s guide. I’ve also heard of minocycline and clarith have a good success rate. There has been studies comparing different regimes. The main thing is that you will have dome Moxifloxacin.
