  • Tom Maholchic suffers from hyperacusis with pain: almost any noise is akin to stabbing icepicks or molten steel in his ears. At Mass. Eye and Ear, Dr. M. Charles Liberman, director of the Eaton-Peabody Laboratories, and Dr. Bradley Welling, chief of otolaryngology, are searching for a cure.

I find, I have severe hyperacusis x10 it seems some days :-( - that I put the volume down before I start a video, and then turn it up as is possible, rather than just.. start it not knowing how loud it would be.. speaking of icepicks.. I don't always remember, but.. mostly I do, and it does work..for me anyway.. fwiw..

Just like J Burrows - I too have chronic or severe Hyperacusis. I start all videos on low and then take care to turn them up too. Blame is of no use. Even with a rare chronic disorder I believe that all people need to take responsibility for looking after themselves, ultimately. Perhaps a warning would be nice. I've done that before on one of my videos for instance. Those of us with Hyperacusis sooner or later need to accept that we have to learn the hard way, it sucks, but at the same time it just is what it is.

I consider myself so lucky. I had a severe case of this for 10 months. I wanted to kill myself. The only things that made me hang on were my two young children. It felt like someone was sticking hot, metal sticks deep into my ears. The pain radiated into my scalp and it burned, and sounds were like knives shoved into my ear canals. After 10 months it started to diminish very slowly. It took about three years to where I could go to the movies and go to loud restaurants. I've had maybe two minor relapses since (but not with the pain, just the sensitivity). The only remnant of this horrible condition that I have left is tinnitus, but after having hyperacusis, tinnitus isn't the worst of it. I'm completely attenuated to it now and barely notice it. Nine years later, I am 95% healed. I think the biggest thing for me was not caving into people that were telling me to wear ear muffs/ear plugs. I refused and it was hell, but it worked for me. I wouldn't wish this on anyone.

I'm right there at his level and it's truly a nightmare that has halted my life for the past 7 months. The burning that comes everyday even when in silence is intolerable. It often gets a bit more tolerable when in complete silence for weeks but any type of movement to return back to a functional life (like trying to watch videos without captions or take showers without earmuffs) brings it right back to intolerable levels again. This affects many veterans who were exposed to loud noise, same as tinnitus, but it doesn't affect quite as many people so the Department of Defense doesn't fund anything for it and nobody really knows about it until it happens to them.

@@liamlecool I've seen very small improvements. The burning, thank god, has reduced a lot and is gone most days. I have gradually been able to wean off earplugs, starting with in the house, and then I just wear them partially in when outside (Except while near traffic) and I only go outside 20 minutes or so at a time twice a week for food and thats it. Still no showers without plugs or social life or working and I cant talk long because that seems to make it much worse. I believe part of this is physical, but there's also a very strong psychological part that makes it so much worse as well, at least in my case. I had to work on the psychological part to see any improvements. It's like if the physical part made me a 4/10 as far as quality of life, the psychological part made the burning worse and brought things down to a 0/10. By working on the psychological part and trying to calm my nervous system, I've gotten back to a 4/10.

I'm certain some neuroplasticity took place and calmed the nerves and muscles in the ears. Most people are not receptive to this, so that's all I'll say and that it's a very personalized approach one has to take so not something that can be easily taught to someone else, even if they are receptive.

I still flinch at hard syllables (plosives) S and T sounds when people are speaking indoors or in close proximity but my tolerance has improved. My tinnitus is as bad as ever, literally sounds like a dentist drill in my left ear. The pain is like what you feel if you look at the Sun or a bright light. Not physical pain but extreme discomfort. I just had a family reunion and I had to wear hearing protection to be around my people. It was exhausting and difficult but I managed to get through a week of partying. Spent most of the time outside, keeping distance between myself and others so that the sound dissipates. I cannot carry on a conversation if someone is closer than ten feet away. It’s just too loud even with hearing protection. I’m ten years into my ordeal and trying to keep myself as healthy as possible. I can’t, but I can enjoy a new life. You can too!

I’ve had a case of hyperacusis (with no pain) for several years now. It has mostly been fairly mild. I work as a chef so dishes clanging really sets it off for me. Other irritants include dogs barking, tannoys, unnatural sounding speaker systems and so on. Recently on lockdown it has become much worse, and I think I exacerbated it by wearing ear plugs which in turn made it more sensitive. About four weeks ago it lead to tinnitus which I am still hearing now as a very high frequency pitch which initially was in my left ear (my most sensitive hyperacusis ear) and has moved to more inside my head. This was initially debilitating but i have grown used to it. Through research I learnt that ear plugs can worsen hyperacusis. I have stopped using them and my ears have become less sensitive. However, if i am caught off guard by a dish clanging etc. it is worse for the rest of the day. For example, I was watching a CZcams music video yesterday and my ear was fluttering the whole way through after doing the dishes, but I tried again and could listen to it today. Speaking on the phone is also an issue even when I hold it to my better ear. Two and a half weeks ago I had an appointment with my doctor over the phone and explained my symptoms and he suggested I had a narrow left eustacian tube which can be exacerbated by fungal spores in the area I live in (he has heard similar stories from other patients before). He prescribed me Mometasone Furoate nasal spray which I have been using at double dose (his direction) ever since which should clear it up in 2-3 weeks. I was fairly doubtful of his diagnosis but continued on. My hyperacusis has become better but I’m sure it is because I have abandoned ear plugs and avoiding noisier environment, and feel I have adjusted to tinnitus. Does anyone else have any similar stories and words of advice? Should I get antidepressants? Does perseverance almost eliminate it? Does tinnitus finally subside or get worse? Please help. Thanks.

Yeah, I have a tinnitus after taking a shower (high pitch, water sound), subsides after 10 minutes. When we exagerattes, we blown everything. I used to play a podcast every bath I took, summing bath water fall noise + speaker podcast play louder than waterfall + everyday = abuse of hearing. Recently, I got little sensitive to noise just like you described (dishes clanging, opening old sliding door with particles under the wheel), I have no idea if thats going to subside or get worse, but mine is not aggressive (still lol). I am getting aggressive with others that produce noise and its not worth be aggressive, we can't control others, only ourselves, so keep calm and accept the suffering noise as part of life

I can understand the need for headphones but it's more of a band-aid like effect for the problem. He needs to stop using them and let his ears relearn how to hear again or the problem will never go away. Like a band-aid it's job is to stop the bleeding, but by stopping the bleeding your causing infection. the real healing happens after you've taken the band-aid off. By wearing the headphones your over protecting your hearing and making the problem worse. I have Hyperacusis but the way I solved it was by forcing myself to go out and hear again with out hearing protection. it does go away and can if you just quit letting the sound get to you. pink noise is a great help for that too. listen to that as much as you can and it helps build up your hearing back to a normal level.

I have this problem and it started in NZ, where people rarely moderate the volume of their voice. It is normal here to have loud talks everywhere, not considering if the other person is able to do the work or hold a conversation of their own. Not sure if this problem started or aggravated with me renting with an elderly couple who had hearing problems and would turn on the tv with loudspeakers each night from 7 to 10 pm. It would cause me pain and anxiety. I would run away to the garden or garage, even if cold. Unkind people. Also, some people are just overly loud, and talk too much and too fast saying little. It is not just the problem of sufferers. When I complained about the noise one day, they said it's my problem, until another guy moved in and said he couldn't do work under the noise the other person/manager was making. Yes, it lingers onto the head and neck and even causes stomach pain. Sometimes, a weekend is not enough to recuperate.

Hello :) there is always ways to help hyperacusis. Doctors rarely understand the condition and even very experienced specialist will dismiss your condition as hopeless. However there are some ear doctors that not only understand hyperacusis but can offer treatment. For those of us, like myself, who do not have access to these doctors, we still have hope. Research tinnitus retraining therapy. If you have a phone or a computer you can do this at home. Your on youtube which means you have access to certain therapies right at home. Research tinnitus retraining therapy and you'll find a huge amount of research on the subject that can assist people with tinnitus, hyperacusis, or both. Things you should avoid while you have hyperacusis would obviously be dangerously loud sounds. That would mean any sound that would be too loud for somebody WITHOUT hyperacusis. If you avoid all noise that causes discomfort or pain you will make the condition worse. The biggest and hardest step is continuing your life dispute the discomfort. It's not easy but literally anybody can do it. I did it and I was very severe. Learn to venture out, learn to do or use forms of tinnitus retraining therapy, dont hide from noise, and try to reduce your anxiety. Anxiety can be reduced from meditation and relaxation. It's a hard road but with research you can recover. I reached remission without the direct help of any ear doctors or specialist. I saw many doctors but none of them could help me and none even tried. I was able to do my own research and found there is help available from many places and I pushed forward and found many successful ways to cope. Don't expect doctors to help you directly. No doctor will give you a pill or a shot and magically cure you. That said, there are doctors that have found treatments and have published their information online and are willing to help people like you. Read, research, and reach out and you will get better. If you low and need somebody to talk to try messaging me or commenting. I'm willing to talk to you about stuff and help out any way I can.

I related to the ice pick stabbing with loud noises and the hot molten metal he described. It feels like a steel ball that has been in the fire is pushing against my ear all the time. My own voice hurts to the point that I can't even whisper before the pain is intolerable. I miss talking to my family. I am learning sign language but not many people sign.
It is debilitating, to say the least. It is so nice to know that other people are going through this as well. People think I'm crazy,

Hello :) try not to reach out too much for help on the internet. You'll find alot of really negative people who have no idea what they're talking about. There are no treatments that completely cure tinnitus. However, there are MANY things you can do that can help you sound threshold and bring your ears back to normal. Loud noises can bring you right back into hyperacusis so dont think therapy is a cure. It is a treatment. That said, with constant treatment and monitoring and good ear hygiene ( avoiding concerts and loud music ect) you can kick hyperacusis and lead a very normal and VERY happy life. Many doctors and specialist do not know about hyperacusis and will tell you its hopeless. You'll find alot of people on the internet and t.v. telling sad stories about their condition and how it has disabled them. Try to avoid these things ok? It's not healthy and it not the entire story. For every 1 person that says they are helpless there is another out there that went through great lengths to treat hyperacusis and did so successfully. Research and ask an audiologist about tinnitus retraining therapy. Next I would see a counselor or a psychologist and ask about CBT therapy and tell them about your fear associated with your condition. Being counseled is a huge step. Reduce your stress. Stress over hyperacusis can actually make the condition worse and lead to serious anxiety. The thing that helped me most was meditation. It reduced or eliminated suffering associated with tinnitus and hyperacusis and allowed me to push through going outside and having fear with hyperacusis. This ultimately lead to my condition getting better. I have a severe anxiety disorder. I had visual static, hyperacusis, tinnitus or ringing in the ears, dizziness, tingling, and a myriad of other conditions. I became suicidal and very depressed. This was right out of highschool when I was very young. I got no help from my doctors but a psychologist helped me with my anxiety. She did not directly help me but did show me meditation and pointed me in areas that gave me hope. With my newfound hope, I researched and researched until I found way to help myself via audiologists online. I was able to return to life, kick my anxiety, reduce but not eliminate my hyperacusis, and have ultimately moved on to start a family and lead a successful career. I went from being suicidal and not being able to continue life to being the happiest I've ever been even with my myriad of medical issues. It is possible. Not only is it possible, but with support, treatment is likely. Stick in there and stay away from the negative comments. Most people suffering from these conditions have not tried every option and have not meditated and learned to control there thoughts. Meditation was the single biggest help for me. Meditation taught me to change my relationship to my health problems. So rather then feeling fear and dread over my pain with hyperacusis I learned to feel indifferent. I recognized and accepted my hyperacusis and this allowed me to reduce my suffering and start treating it. 10%better, Calm, and Headspace are all fantastic meditation apps. Research research research. Spend hours researching everything I've mentioned. Understand it and apply it to your life. It has changed my life and I've learned to not only live, but to live happily with my health and to lose my anxiety. If you need someone to message let me know. I can walk you through it. I'm not a doctor, I'm a normal guy with a normal career. I cannot cure you. I can however give you free advice on how to get hope back and help you understand how I got off anti-depressants and found ways to treat myself while facing waves of anxiety. Hope this helps :) hang in there. You got this.

@@TheHondarider35924 I think I'm suffering from this or have very close symptoms. I'm a musician and earlier this year I finally got gear to record my music... and then 2020 happened. I still persevered despite it all, and now I have this totally debilitating issue going on with my ears. It started when I tried cleaning out my ears with q-tips (bad idea). I shoved some wax down deep in my right ear and had muffled hearing for a few days, that's when the ringing started. I went back to doctor and she recommended using debrox to remove any earwax blockage. Well I did it a few times at home and I think I over did it. The ringing is intense. So I sought an ENT to check my ears and really give me some direction. Well it's not earwax he said, but it could be TMJ? I doubt it. After I avoided loud sounds the ringing sort of calmed down but as I slowly exposed myself to normal sounds again they were just too loud accompanied with the ringing and now I'm terrified. It's like everything is too loud now, I can't play electric guitar, def not drums, watch movies or even play video games or sing anymore. I'm hoping to god this is temporary

See a doctor and try to get help from this first. I would discuss your concern with a hearing specialist and explain to them that you think you have hyperacusis. Do not wear ear muffs and try to avoid dangerous levels of sound. Anything over 85 decibels sustained would be too loud. Google what sounds are roughly 85 decibels to get an idea on wear you can and cannot go. Wearing ear muffs or hearing protection will make the condition worse. Tinnitus retraining therapy, CBT from a counselor, and attempting to continue daily activities are the best treatment. I am a musician. I played electric guitar, bass guitar, and acoustic and I damaged my ears and developed a ton of different ear conditions. I developed severe depression, anxiety, and suicidal thoughts. I was very low but then I looked to meditation which retrained my brain and helped to reduce the suffering. I then ventured out more and forced myself to deal with the pain and discomfort in my ears. After that I started therapies for my ears, played low level acoustic instruments, reduced volume of music (but importantly did not cut out sound. I continued sound.) And things got drastically better very quickly. I no longer suffer from hyperacusis. Its changed my life but I live a full and happy life. Feel free to comment and message me if you have questions on how to cope. It's very easy to have your life back once you understand it.

Others say use sound therapy but don't over use ear protection for normal day sounds as it becomes more sensitive... Obviously in loud places use protection. What do we do? Example being here..:
"Wearing hearing protection making hyperacusis worse is a bullshit lie made up by TRT enthusiasts. You absolutely MUST protect your ears and be in silence to recover. Sound therapy makes hyperacusis worse. STOP doing sound therapy, PROTECT your ears as much as possible, AVOID loud environments at all costs, and most importantly give yourself time. The reason your hyperacusis is getting worse is because you aren't protecting enough".
