  • Side effects of Finasteride used after hair transplant worry patients endlessly and is one of the most commonly asked question-
    "Is Finasteride Safe'?
    "Does Finasteride have side effects that are sexual"?
    Over 300 million people have received Finasteride till date. But only 39 have reported permanent erectile dysfunction after taking Finasteride. Is this percentage even statistically significant?
    In my experience, much of which is borne out of observations since the past over 18 years in hair transplant practice, only 1.19% reported erectile dysfunction but on stopping the medicine, they returned to normal. Contrast this to a person who insiduously searches the net for side effects, and has an incidence of 23%. This makes me wonder how much of this is purely psychological overlay, the mind playing tricks with the body! As you know the human mind has complete control on the body.
    Does this occurrence keep us away from the benefits of Finasteride in hair loss?
    Did you know in the age group 25-35 (and this is the age group which has maximum hair loss), the prevalence of ED (erectile dysfunction is as high as 50%?
    Bearing all this in mind and the drawbacks of a hair transplant without Finasteride, I would advise you especially if you are extensively bald and available grafts are limited, to stay away from a hair transplant procedure.
    Only Finasteride provides a protective umbrella to the growing grafts to save them from the affects of DHT.
    IThe bottomline is that Finasteride is a must for growing hair that are susceptible to DHT. You must know that grafts from the so called permanent zone are only relatively permanent and they do carry on them DHT receptors, though to a lesser degree. This is the basis of use of Finasteride in hair transplant.
    Since you do not get a hair transplant every other day (nor is it possible)- why not give this chance your best shot!

    Hello Dr Bhatti and thank you for a very important video. The one problem with a hair transplant is that it does not 'cure' or 'stop balding' as some uneducated potential patients may belive. It merely rearranges the hair that a patient already has. So it would make sense in today's age to take Finasteride to stabilize one's hair loss in the first place. Once this is achieved then the planning of a hair transplant/use of grafts, etc is to the patient's advantage (as well as the surgeon).
    The problem with people who are 'Google doctors' who refrain from Finasteride from what they read is that there is 'no control group!' How does one know if any sexual side affect is 'directly related' to the drug itself and not just a self fulfilling prophecy or a host of other extraneous variables? Finally thank you as always for your honesty in advising people (such as myself) who have such advanced hair loss/poor donor area, etc not to have a hair transplant. Patients need more HT surgeons like you. I trust you and your family are well and thank you for sharing!

    I got fue 6.5 months ago my results I would say are 9/10 I have decided to take nutrafol but I am considering taking finasteride now that you mention this.

      I also noticed some additional thinning in areas like crown that haven’t been worked on so I hope 🤞 I didn’t mess up by not taking finasteride my surgeon recommended it but I only use Irestore helmet and just bought nutrafol. Maybe topical finasteride???

    So my honest experience with fin ( 10 months so far) is that my sex drive has gone down a bit and my erections are slightly softer as well.
    Although I've also noticed if I weight lift or/and do some cardio these side effects go away, so that it's another good reason to hit the weights.
    The pro is my slightly reclining hairline has some growth back, I have almost no acne although I do break out often enough still, I don't pee nowhere what I use to (god bless) and my body/facial hair doesn't grow as fast. Which is quite nice, less trimming and shaving is always welcomed.
    In my experience the pros out weight the cons.

      @@DilshadAlam-gr4qt I dropped down to .25mg like 10 months ago, sex drive still is a bit down than not being on fin but better than on .5
      Erection is better too and I still have my fin hairs (I only regain a small amount of hairs from fin to be fair). I think .25 mg works for me and i don't plan on changing unless my hair loss gets more aggressive.
      Also added a micro needle stamp to my routine that I do once a week and it has really good results so far, hope this helps.

But I plan to start taking finasteride after a hair transplant. Unlike Rogaine, does finasteride work on most people? I tried using Rogaine but it didn't help me well!

      It depends upon age and speed of baldness. Start with 0.5 mg on alternate days for 3 months and then scale up.

      Yes but these are very rare. In my clinical experience side effects are psychological since the negative impact stops immediately on stopping Finasteride

      @@mertcalskan7676 Initially when you start to take the medication the body needs time to adjust. This may vary from 4-6 weeks.
      If a person continues to have side effects he needs to stop.

    Hello doctor, I have been thinning for a time now and been using minoxidil 5% twice a day. I‘m currently 19 1/2 years old and want to start finasteride soon. Now this is where my problem begins: Some say it‘s safe to take after the age of 18 (after development of primary/secondary male characteristics) and that it won‘t stunt my growth/maturation until 21 even though it blocks type 1 alpha5reductase enzymes, which is known to develop male characteristics. Is this true, or should I wait until I‘m 21 just to be sure that I‘m fully matured? Now some people on youtube, doctors like you ,mention that taking finasteride at the age of 18 won‘t stunt anything, and they say that it‘s better to start that early in order to as much hair as possible. Now I‘m thinking whether I should start now or wait... But what if I loose many hair until I‘m 21? Will I hate myself for not acting earlier?

      Finasteride as per label can be used after 18 years. However it is better to wait till 21 when secondary sexual characteristics have fully developed.

      Thank you for the message. Finasteride is a prescription drug in India. You will need to go to a local doctor. Please get Total testosterone done before that. The usual dose is 0.5 mg to 1 mg a day.

    Dr pls tell me when baldness genes switches off so that we can stop taking finasteride as it is evident that over the period of time finasteride tolerance happens in the body due to upregulation mechanism of the body. From your own video I knew about customized dosing ,switch on switch off method and combination of dutasteride once in a week.       With growing anxieties about use of oral DHT blockers, what everyone is hoping for is a better topical Dutasteride. Which can be delivered in controlled titrated doses at the root level efficaciously and safely in doses which do not saturate this environment causing spill into the bloodstream and elevated serum levels and sexual side effects.
      Many clinics have started making their own versions by co-opting local pharmacies. However, for them to make topical dutasteride that is effective is a pipe dream. Why? Because the science of topical medication delivery is fat too complex to be casually experimented with. Most are churning out placebos. These cannot be substantiated scientifically. A very large company Polichem has devoted years of research in perfecting its drug delivery system. Local pharmacies use half aqueous and half ethanol solution as solvent unmindful of the fact that most of Dutasteride and Finasteride precipitates and is not deliverable) besides chitosan to improve drug permeability into skin but it cannot take care of the problem of solubility of Dutasteride or Finasteride for that matter. There is more chemistry involved than simply mixing 2 solutions.
      Polichem has been tight lipped about their vehicle but there is a rumor that they have been using nano-carriers alongwith chitosan in form of a molecular complex with polymersomes.
      These newer vehicles of effective delivery are the future of hair loss treatment.
      We want reliable formulations and nt magic cures any more fro hair loss. And the present ethanol and polyol formulations that are irritating, greasy and difficult to apply really suck.
      So be happy since the potential of present DHT blockers has not yet been exhausted!

    I'm a 27 year old with androgenetic alopecia. I took finasteride for 3 Months 1mg 3 times a week. After two months I have started to feel tenderness in my breast. After stopping the treatment for another 2 months the tenderness was gone. Would you advice to stop the treatment completely or continue on a lower dose for example?

      @@TheFreeskier01 Levels do not have any value once Finasteride is initiated. You could reduce it to 0.5 mg on alternate days in consultation with your doctor. Monday-Wed-Fri-Sat (this covers the weekend testosterone spike well )

I took it for around one week and had got its side effects then the doctor told me to stop it so the side effects went away. Now, I’m still experiencing hairfall so should I take it again or what?

      Yes the only thing that will help you at this stage when you are very very young, is Finasteride.
      If you experience side effects, you will need to stop. But get a baseline total testosterone and estradiol done at the lab and let me know before you start.

      Side effects happen if Finasteride gets absorbed. And it doesn't get absorbed through the skin.

      Finasteride does not cross the skin barrier in sufficient amount to be of any value for hair loss. It;s only a marketing gimmick. Just take topical Minoxidil if that is all you wish to do.

    Is it ok to take alcohol once or twice in a week post one month of hair transplantation will it have bad affect on the results.

      Dear Mr Sanjay, Yes you can consume alcohol after 5 days of the procedure. Unlike smoking it does not affect the procedure. Only thing is that excessive connsumption of alcohol can decrease availability of Oral Finasteride to the follicles since this drug is also metabolised in the iver. Moderate consumption like yours is not harmful, either to your overall health or your follicles.

I have a small question though, would you still recommene baldy men( Noorwood 7) above 40 or late thirties to take this medicine? Would it worth the risk?       A bald patch in a person at 40 may still not be stable. In this case DHT blocker is advised.

      It interferes with the detection of prostatic cancer since it interferes with PSA test. However most prostatic cancers happen late in life and are mostly benign. Hair transplants are done in younger age.

      Rarely yes if you are oerweight, or take too much Finasteride, or take Dutasteride which blocks 95% DHT and importantly genetics is a huge factor- gynecomastia gets unmasked or it appears earlier than it otherwise would.

      There is no alternative to DHT blockers which deal with the root cause of baldness. Otherwise you can in consultation with your treating doctor use Minoxidil twice a day. Biological functions are more important than esthetics and body image.

      The transplanted hairs are 'permanent'. Once they have grown at 6 months to 9 months they will survive without Finasteride. This is if the permanent harvesting zone was where the grafts had been taken from and Finasteride had not been used to thicken miniaturised donor hair before the hair transplant.

    topical finasteride, the results are practically the same as the oral with less sides efects, the problem with the oral is when it work is because the drug is in all the system and arrives to the follicle and arrives the side efects, aga is not a endocrine problem is an autocrine, with topical finasteride the action is in the folicule and in the system but not at the same intensity

      Topial Finasteride cannot reach the follicle directly yet. The carrier has not been discovered that will allow this. Till then topical Finasteride will continue to be absorbed into the blood stream and then reach follicles.

      Not likely due to Finasteride. But spillage of Minoxidil into eyes if you do not apply carefully as instructed in the pamphlet in the carton can cause problems in the eye. Plaees show to an eye specialist.

    Hello Doc, i'm taking finasteride since 4 months ago, i experienced testicular pain in the first days, so i decided to only take 0.25mg daily which seems to be fine for me, if i try to take 0.5mg daily i get testicular or lower abdomen pain again ( i checked total testosterone and estradiol before using it, all fine). After these 4 months i had a good regrowth of hairs ( my barber said it is already an exceptional result). Should i try to increase the dose to 0.5 or 1mg daily to see more results or i could do fine with only 0.25?

      @@elxelx441 If you do not get it in the first few months, you never will. We have patients who have been using it for over 15 years. Also remember hat 45% people between 25-35 years age have erectile dysfunction which can be unmasked with Finasteride. So before starting Finasteride get blood levels of various hormones tested as a baseline.

    Hi Doc, I am 25y old and took 1mg finax daily for 5 weeks and had experienced mild side effects like reduced libido, weaker erections which persisted during the last two weeks. I decided to stop but after two days experienced adverse effects. My total test is 300 after 12 days of stopping. After about 20 days my morning erections are back and I am able to get 6-7 erections in a day(some improvement over the course). I had normal sexual function before finasteride. Can you suggest the next course of action. How long does it take for the side effects to go away? I had not done hormone levels before starting as my doc didn't suggest.

      You can take 0.5 mg on alternate days and see if it helps.
      If not, you may have to increase the dosage.
      At this age, baldness when it happens is very quick and aggressively progressive and hence by playing around with low doses you may be losing precious ground.

      @@kaustoobh1 Dear, transplanted hair never shred off but sometimes they may b get thin over the years. But those hairs who r not transplanted, may continue shredding. So to prevent further shredding of such hairs, doctors advice medicines n PRP. Every doctor every clinic has it's own way of treatment. So better would be to go & meet the doctor.       @Gary Its the best and most ideal situation if a person has history of extensive badness to take Finasteride for longer. If they do not choose to do so, and this is the topic being covered here, taking it for the grafts to grow is mandatory for a minimum of 6 months.
