Depends on which time sugars are high.If PP are high then 50:50.otherwise 30:70
With Lantus 10U and linagliptin 5 mg CBG is > 400 . Post-CVA patient at home. Should we add human actrapid or increase dose of lantus ?

@@dr-arvindkumar Thank yu sir. I have a 48 yr old diabetic who was on sgl2 inhibitor with sugars in 400+ range HbA1C 11, who reqd all the 3 drugs as well as basal insulin. With proper exercise, diet & above regime, her sugars were ialmost in the normal range. With her slacking on the exercise, her sugars are slightly out of range now, necessating an increase in the insulin dosage.

Sir if we start basal insulin on Monday evening, and check the Fasting glucose on TUESDAY morning,, and suppose Fasting is still high, then on which day we will have to add the extra 2 unit of basal insulin??? Would it be WEDNESDAY evening as per ur explanation sir?? 🙏🙏🙏
