As someone who worked in linens before, to clarify for those unaware of what these are:
Duvet- a insulated blanket designed to keep heat in and the cold out. They come in different materials and thicknesses for different purposes and type of washes.
A bed sheet- a bed sheet can come as a fitted sheet or as a regular sheet, fitted sheets are made to cover the mattress and fit around the corners of mattresses whereas a regular doesn't fit around the corners but tend to be larger to compensate.

As a Housekeeper in a 5 star hotel, this makes me intensely uncomfortable.
I remember having one of those crappy cheap bed frames- when I finally threw it on the curb, some old guy came up to me and said "You're doing it wrong, when I was your age that frame would've been broken!" (implying from sex)

Btw for hotel duvets and blankets: Those things don’t get changed out unless they start to stink or they have stains on them. It takes too much time and adds too much weight to the linen cart/bags to include them in. -From a former housekeeper

My own set up is thus, fitted sheet w/another sheet on top of that because I find that they break down far to easily other wise, like they will go from smooth to those absolutely tiny balls of fabric like its ablating and then the holes form witch means its time is limited, and then just a comforter and a couple of pillows

My dad and my mom still sleeps in a futon/mattress on the floor. So my mom is normally the one putting on the sheets over her and my dad’s futon. But that very day, my mom was feeling exhausted so me and my dad had to put the sheets over the futon and it’s the most chaotic thing that ever happened in my life.
My dad had to turn the futon sideways and I had to pull the sheets up but because the futon it’s like wonky and wriggly and it keeps shaking whenever upright, the both of us had a hard time slipping that futon into the bed sheets. I kept toppling over and my dad started sweating while my mother witnessed the both of us suffering.
From that day on, I have shown a lot more respect to my mother who had no issue whatsoever changing the sheets and shoving the futon back into the sheets everytime she washes it

Many duvet covers have strings in each corner that allow you to secure the duvet inside preventing from shifting around.

I need a day off when I do my sheets. I will not clean my sheets on a day I’m busy. No homework, no school, no work, just me, the bedsheets, and a scheduled 30 minute cry break when I’ve got the three corners, one pops off and the others come off with it. I bought these bungee cords for your bed and they take an extra 10-15 minutes to put on, but they make any bedsheet tight with no wrinkles. Worth every penny and minute.

I'm pretty sure I've never in my whole life had like a real, proper duvet, since childhood we've always used those sofa blankets/covers so ever since then I just use that to cover myself with, of course I cover it with a duvet cover which surprise surprise the cover is nust a tad bigger than the blanket but I know no other way now, gotten used to it xD

I wonder what is "normal" to set up a bed? For me: Bed frame > box spring > mattress > mattress pad (an egg crate kinda thing that roughly fits the footprint of the mattress) > mattress protector > fitted sheet > sheet > blanket > comforter. It seems to be normal to tuck in the bottom of the sheet and blanket in the northeast of the US and a part of the bottom sides. I dated a German girl for a while and after a few nights she said she hated it and asked if I could not, I was wondering about her not doing it as well. Seems tucking like that is an American thing that I have stopped liking, the freedom is much better to me. I have never messed with a duvet/duvet cover before, maybe someday.
