    Great video thanks for sharing! Do you think spironolactone causes a decreased AMH and a poor responder outcome to egg retrieval? I’m 35, healthy, tested .9 AMH (while on spironolactone), FSH 7.6, had only 5-6 follicles during stimulation and doctor retrieved 2 eggs. I am devastated and shocked. Do you think spironolactone has caused this poor outcome? Thanks!

    • @PhilBoyle

      I am not aware that Spironolactone affects AMH. Natural conception is likely a better option for you. Try the fertility app at www.chartneo.com and work with a provider listed there.

      I have seen successful pregnancy with undetectable AMH - there is no lower limit for natural conception. If the monthly cycle has stopped or female age is 45+, there is little chance of success.

      We treat Low AMH all the time. It depends on the individual but I commonly use Letrozole with HCG and low dose naltrexone 3 to 4.5mg nightly.... Treatment is a process over time and with NeoFertility needs a fertility chart with ultrasound follicle tracking in a specialised fertility clinic.

    • @PhilBoyle
      @PhilBoyle  Před 4 lety

      You are correct. Low AMH means a reduced ovarian reserve. Advancing age is the single biggest factor to predict egg quality. Natural conception with low AMH usually confirms the eggs are good.....But IVF clinics struggle to get success with low AMH as they need multiple follicles to get the best chance of obtaining a good embryo.

      AMH is only one aspect to consider for your fertility. IVF clinics recommend donor eggs with very low AMH - but natural conception is often still possible - even if AMH is undetectable. I suggest you track your cycle with www.chartneo.com and work with one of the fertility providers listed there.

    Hi Dr. Boyle. I am 34 years old. 1.5 years ago (at age 32), my amh was very high at 4.9 ng/ml. I had it rechecked a week ago and it is 2.08 ng/ml. I am devastated and panicking. Does this mean I will enter menopause early? Is there any way it could be inaccurate? I took two morning after pills two months ago, could that still be effecting things? I am so scared. I am single and not ready to have a baby right now but terrified I am quickly running out of time. I can’t understand how it fell so low, so fast. My FSH is 5 IU/ML

    • @PhilBoyle
      @PhilBoyle  Před 4 lety +2

      AMH levels can fluctuate a lot. I have seen AMH of 1.3pmol/l increase up to 10pmol/l over a period of 3 years..... so don't panic. I don't recommend repeated AMH tests if you are not trying to conceive. The single biggest predictor of fertility is advancing female age as outlined here www.neofertility.ie/single-post/2016/09/20/Fertility-treatment-age-40-plus ...... From 35 years old fertility is declining, so keep that in mind.

    • @t28khkaurharpreet90
      @t28khkaurharpreet90 Před 4 lety

      Why do they fluctuate? Based on online resources, vitamin d levels, birth control pills are the main reason. Are there any other reason? Please let me know ...🙏🏻🙏🏻🙇🏼‍♀️🙇🏼‍♀️

      @PhilBoyle  Před 3 lety +4

      Antral follicles produce AMH. The number of antral follicles decline in pregnancy and after taking birth control pills. Stress can also diminish the antral follicle number. AMH levels can increase over time and are a poor predictor of capacity to conceive naturally.

      Hi Nomin, It sounds like you are very close to menopause. You are in the menopause if there has been no bleed for 12 months. In that case we cannot help you to conceive naturally. An induced bleed from the hormonal birth control is not related to ovulation. I suggest you work with a doctor trained in RRM - Restorative Reproductive Medicine. See list of Providers and doctors hers neofertility.azurewebsites.net/ProviderMap?portal=True (Doctors marked with red cross.)

    • @PhilBoyle
      @PhilBoyle  Před 4 lety

      You need to look at the units of AMH - if ng/ml multiply x 7.14 to convert to pmol/l. I usually work in pmol/l. Yes you can conceive with almost undetectable AMH, especially if you still have a menstrual cycle. If cycles have stopped, conception is unlikely. In my experience natural conception is better than IVF for low AMH. But you need to see a doctor who can balance your cycle correctly.

    • @PhilBoyle
      @PhilBoyle  Před 4 lety

      1.76 x 7.14 = 12.5 pmol/l - absolutely , yes you can conceive with this AMH. AMH can be temporarily after a time of not ovulating post pregnancy and breastfeeding or after taking the oral contraceptive pill for several years. You should be ovulating for 6 to 12 months before testing AMH levels.

    • @PhilBoyle
      @PhilBoyle  Před 3 lety

      Yes it is possible to restore ovulation for about 50% with your clinical picture. Your doctor needs to prescribe ethinyl estradiol 20 mcg daily to down-regulate FSH and try and restore ovulation - If you do not start ovulating within 4 cycles, natural conception with your own eggs is then unlikely. see pubmed.ncbi.nlm.nih.gov/10920090/
