I still think they should do an ep for the 50th Annv Where a cosmic event brings DOctors from several different Universes together. Each played by one of the surviving Doctor actors. In each's universe they still had all the same companions, and for the most part all the same adventures. THe difference being each survived the circumstances that led to the next regeneration, and they all aged. So that way they can return to their characters with an explanation for them being older.
They sorta did that ish now with the 50th being 3 Doctors (Which I guessed you watched) and now they are doing Tales of the Doctor Where they have it ad the memory of the Tardis of a Doctor Gets to meetup with a companion and they chat about old times, so far I know theres Tegan and 5th with Peter Davidson being of current age and Talking to Tegan in currdnt age. I think this is also the same with Ace and Sylvester McCoy, because of the recent 60th where the old companions pop up to have an adventure.

"The Doctor" didn't come until the relaunch. This was prior to that and it very accurate to call the character Doctor Who because that is how he was credited throughout the series.
The credit was changed from "Doctor Who" to "Doctor" during the Davison era. He was never seriously called Doctor Who in the series, though. Everyone who knew him referred to him as The Doctor.

In ''The war machines'' he is actually called 'DOCTOR WHO' even though I know this may have been a mistake. well at first people called him doctor who. the credits an unearthly child says doctor who and so was all the episodes until Davison, so people who are a bit older and don't watch new who call him doctor who because they probably only watch it when he was referred to as doctor who. The characters obviously didn't say stuff like watch out doctor who, they called him the doctor or doctor. But I do think that saying so and so played THE doctor is the correct way of saying it

In every episode, from 1963-1987, characters always called him "The Doctor".... plus Peter Davison was "The Doctor" in the credits...
The 5th through 8th Doctors, and every Doctor since and including the 10th has been credited as the The Doctor
