    Dr Parthasarathy talks about hernia. Many patients want laparoscopy while others don’t want laparoscopy, there are many opinions for both pro and against laparoscopy whether it is better than open operation or open operation is better than laparoscopy. The same surgeon can do laparoscopy for some patients and open surgery for others, it is based on many factors such as :
    1. Whether it is a small hernia or large hernia
    2.Where is the hernia located? is it a bellybutton hernia or groin hernia
    3.Has the hernia failed previously, if somebody has done hernia operation before and has come back after ten years.
    4.What is the overall physical condition of the patient, whether he is a fit young person or an old person.
    Thus the operation choice is based on individual patients depending on their state.
    An important reason why patients postpone their hernia operation is the fear of pain and lengthy recovery from it. The amount of pain depends on the nature of the cut, as more and more hernia operations are done using laproscopic technique which are not done with large cuts but with small keyholes the pain has largely come down, more or less close to being pain free. When an open surgery is done with a large incision we generally take adequate measures to make sure the patient doesn’t have excessive pain, such as:
    1.Placing an epidural catheter which is a very fine catheter placed at the back through which a daily dose of pain killer is given which directly acts on the nervous system and decreases the pain.
    2.We give focussed nerve blocks at the end of operation so that patient doesn’t experience much abdominal pain for the initial three or four days and the patients recovery is smooth.
    Generally the pain after an hernia operation is very much tolerable and the patients are usually able to walk the next day after the surgery.
