For those who dont understand, the parents took the organ of her, to give it to her sister. Kidney removal scars are located over the hip bones left and right, you can see the bandage at the end on that exact spot. Also it's named "the first accident" showing, that this was maybe not a real car accident, it was plannend, so they can take her kidney. There probably will be another accident bc of the eyes and voice issue from the sister. And bc the hospital doesnt exist, it's possible that they maybe removed the kidney themselves (the parents) and bc of that, she has severe stomach aches bc it wasn't made professionally Hope this helps. It's a psychological horror movie Well she finds out that her dad was actually testing her blood and stealing her organs so that his true loves daughter will live because she has a disease. He had a ton of kids with the girls mother and keeps the mother chained up so he can keep having kids with her because she has a rare blood type, he takes vital organs from the children and dumps their bodies.
