    This is immensely helpful and informative. I’ve been thinking about my jowls and hooded eyes and been debating the best treatments. Facial fat loss worries me. Looks like surgery is the best for lift with some RF. Thank you very much.

    Thank You Dr. Kopelman for Your insightful and honest reviews and opinions. I have a question in regards microneedling RF. I am wondering if it does not have the same tendency to cause atrophy as Ultrafomer III ( HIFU) as it also delivers heat to the face. As far as I remember maximum depth for Ultrafomer III is 4,5 mm and for Vivace microneedling RF maximum depth is 3,5 mm. I had one treatment of Ultraformer III and as I noticed some pain and swollen face at the beginning after the treatment, I have not seen any results at all when it comes to lifting of the tissue. Unfortunately only afterwards I came across the information that it can desolve facial fat :/ which is an undesirable outcome for myself due to the facial structure (I have jowls but rather thin lower face). I am afraid that due to that fact microneedling RF can also be causing the same undesirable effect on a patient with already limited amount of facial volume. Would be greatefull for Your input on this topic.

    How often is it appropriate to get a CO2 laser? And is it appropriate for the neck and chest thank you so much for your great videos.

      How frequent you have repeat CO2 treatment depends on whether you had fractionated treatment, the depth and density of treatment and the degree of sun damage or age related changes
      In regard to CO 2 treatment of the neck and chest the treatment has to be light because there are few hair follicles found in these areas which means that the propensity to scar increases so the treatment must be very conservative
      Micro needling or erbium laser treatments are safer in these areas if performed in a conservative way

    Hyaluronidase is not without quite a bit of risk. In my opinion, fillers last a lot longer, and leave behind scar tissue that accumulates with repeated use of filler in the same area, as well as other (even worse) potential complications.
    Almost three years ago, a “top plastic surgeon” suggested a “bit of filler in my cheeks” when I went in for Dysport. I had been working in a stressful tech career, and hadn’t been getting great sleep. When she suggested it, I thought “Shit, I must be really falling apart for her to suggest it”, and went ahead with it. It was one of the worst decisions I have ever made. I have high cheekbones and I didn’t need filler (needed to quit my high stress job). The “cheek filler” turned out to be UNDER EYE filler - and although she initially offered/explained that she put “too much, too high”, when I got upset she back-tracked and claimed it was “malar edema that would resolve on its own”. She advised against the use of hyaluronidase, putting the “blame” on me, stating that she “didn’t think she could get me through it”. Almost three years later, I’m still “dealing with it”. It’s worse on my right side, as it has been for the three years since getting it. I would be very careful when getting filler done, anywhere. I haven’t gone near the stuff since. And I don’t care how this sounds, I will not trust a female plastic surgeon with my face after that (this) experience.
    Threading is even more risky. I would lose respect for a doctor who recommends threads - which, fortunately, Dr. Kopelman didn’t. I’ve experienced hyperpigmentation from a laser. It was the only spot on my whole face and it was very anxiety- provoking. Took many months to resolve. Will not go near another laser. Will not have filler. Will not have fat transplants (too risky).
    Strangely, I would consider a facelift (also risky) someday - if I could put enough trust in another doctor. Not sure that will ever happen, either.

    DO you think at home RF devices are safe to use? (I know they don't produce very strong effects, but overtime, they seem to be quite helpful).

    Doctor what about FDA approved bcs Althera is FDA approved but how come it can if it can atrophy mucked as you said ? Thank you in advance

      All medicine are fda approved and still have side effects so it’s the same for ulthera

    Is Thermage the same thing as ultherapy? If not what do you think about it. I've been considering getting it done

      Not a fan of either procedure .These procedures cause thermal damage that don't lead to long term benefit

      Ms. Bruno the best way to reduce melasma is to avoid sun exposure, plenty of facial sunscreen when out, topical drugs that include hydroquinone, azelaic acid, kojic acid, niacinamide, cysteamine, and tranexamic acid, and steroids. Sometimes combination therapy will work better than a single agent.CO2 lasers in general deliver heat to deep tissue which actually may make the melasma worse.
