  • Although doctors have their reasons for being judicious about prescribing SSRIs and the maximum dosages they will take their patients up to, in some circumstances (particularly when treating anxiety and OCD) psychiatrists do exceed the FDA "max" dosages limits.
The American Psychiatric Association (APA) has a separate maximum dosage recommendation to prescribers for treating OCD, which it calls the "occasionally prescribed maximal dose." In practice, higher than FDA dosages are also sometimes used when treating GAD and MDD .. although how far prescribers will go is very much a matter of individual preference.

If it’s not working at the highest recommended doses, it’s pretty unlikely it will work at above the maximum dose
