You seem either to have no needle-tip anymore, or having the counterweight set to high. Set it to approx 1.5 grams, and the anti-skating to roughly the same value.
The bottom shell is warped. You can test this by removing the bottom shell and place the TT on blocks, See if the problem still occurs. If not, you have to straighten the bottom shell with a heat gun
