Just to clarify that in the past Italian television used to impose singers to lip sync (it's well known that the Queen once protested against this during the most famous italian musical contest on national tv). I think Björk didn't want to lip sync here, as she tried to convey multiple times during the performance (by pointing at the microphone, for instance). I think she tried to execute the song in the most professional way while at the same time giving hints about what was really going on. It's actually quite painful to watch!
It is a great song, and BJÖRK is also great, but she is using vocal playback, and she is "too late with the microphone" at many places! Look at the the evidences here - 1:14 min / 1:54 min / 03:05 min / 03:16 min / 03:28 min / 03:49 min). It could be also, that the original live vocal was recorded in a bad quality, and then it was just replaced by the studio version.
