What is Preservation Rhinoplasty? Before and After with Dr. Vila
A preservation rhinoplasty is a new technique for making aesthetic changes to the nose. In this video, I explain the philosophy and how it is different from traditional rhinoplasty - known as structural rhinoplasty among surgeons.
Not everyone is a candidate for a preservation rhinoplasty. But in the right patient, a preservation approach is a beautiful way to make changes to the nose.

    Marvellous explanation on whether to have surgery or not for a deviated septum. I have been told by a surgeon that i need one but i can breathe ok. Its hard living with it but the thought of having surgery makes me very nervous.

    As a patient who recently underwent a closed reduction: IT HURTS. There's no way this should be solely considered as "discomfort". It was as painful as the brake itself.

      I was not doing that. I have a deviated septum since forever. Getting my adenoids and tonsils out? My friends are painkillers? I’m yes very much. I watched Ren and Stimpy and passed out. I’m now an alto because… I woke up during surgery. I’m sure was freaky for everyone but apparently I was entertaining.

    Would you recommend an oral and facial surgeon as well? Or just plastic or ent surgeons?

      Oral and maxillofacial surgeons can do great work as well. I would just evaluate their results. ENT, Plastics, Facial Plastics, OMFS are all potentially great - just look at their before and afters and get a consultation with the person to make sure you're a good personality fit.

    Ive felt that the one side Inever breathe better has gotten worse with time and gets worse at night. Also Im always pickin my nose bc I feel like any little thing in that side obstructs the airflow! So bc Im always pickin that nostril Ive gotten so many cold 😢

    Hi Doctor Vila I have a question about scarring from a lip lift procedure as i've seen some surgeons explain it as pretty safe, while others only recommend it for older patients. Also, I realize that for men, the ideal length of the philtrum sits at 13-15mm, while mine is 16mm. I also have a decently full upper lip so I don't know if getting a lip lift would make my upper lip look unproportional to the rest of my face

      In general, scarring is more aggressive in younger patients and requires more interventions. Patients past 40 and up tend not to scar as aggressively. There is no ideal philtral length, as it is different for everyone and their facial shape. A patient with an already full upper lip would only need a conservative lip lift. Best of luck to you!

    Had my Septoplasty and Turbinate reduction done yesterday.. I have no pain and slight swelling. The worst part has been the stuffiness and cleaning up the discharge.(like you have with a sinus infection) I have the splints in, no discomfort or pain..Today I feel fine, still dripping a little.. For the best success and lowest risk of ENS/exterior membrane damage to the Turbinates have the microdebrider procedure done, the cauterize/radiowave methods have a history of issues (have a complete understanding of each method)...I don't regret having it done..Do your research on the Surgeon and the way they reduce the Turbinates and you'll have success ..:)

    Very interesting analysis of the faces' aging process. And great course on the movie's stars, too! Your own 'younger' face has thicker brows which is the most significant change when comparing. Your 'sharp eye' and detailed discussion regarding the face's change is expertly helpful when considering cosmetic procedure and choosing the write one.

    Peter, the results you showed are fabulous! And your explanation of what happens to our faces when we aged and what could be corrected is very informative. Unfortunately, my face's skin became saggy due to the excessive dose of Hylenex injected to dissolve the filler I was unhappy with.. Do you think it would be possible to correct/fix my problem, too? I want to restore my normal 'look' at least to some degree. Your video gave me a hope.

    My problem is, I live in Canada and work in the Arctic. I fly a lot. This past turn around, I realized a few things: I haven't been able to breathe through my nose in the winter for years. Too much fluid build up from the cold. And second: I got a sinus infection up in the arctic, I assume...because I haven't been able to pop my ears since my flights back a month ago. I bought one of those nasal valve things, and dang those are nice. Feels great. But you can't chew with them. Life's rough. (yes, I have a deviated septum, in case people were wondering). I also have a pretty small nose.

    Hello there, I’m a physician working in primary care. Here’s a question I really don’t know the answer to. If a patient has slight nasal asymmetry either bc of the tip or septum, are you ever able to achieve perfect symmetry? Are there any methods to ensure symmetry? Thank you!

      There’s no such thing as perfect symmetry. 100% of people have some degree of asymmetry in the facial skeleton, which leads to the asymmetries that we see in the nose and other areas of the face. Rhinoplasty surgeons use a multitude of techniques to try to get the best symmetry possible, whether it’s changing the axis of the dorsum, adjusting the tip deviation, or some other asymmetry. Despite being a relatively small structure, the nose is a 3-D structure laden with asymmetries, and perfect symmetry is simply not possible.

    Breathing problems, facial pain, nausea, bony spur on the floor, that was why I had the septoplasty. I just had the septoplasty not the turbinate reduction. I also do the nasal spray and Claritin for allergies. And I had allergy testing and really need the shots. The mri showed me super blocked (due to allergies) and the deviation prior to surgery. Don’t wait 24 years like I did to get this done. And if a doctor pushes turbinate reduction and you don’t want that put it in writing. My doctor was awesome and did not push the turbinate reduction but I know some do. He is also the second surgeon because the first one didn’t believe me.

      Great question for your surgeon doing your facelift. In general I’d recommend at least a month or so to ensure the grafts and donor site are well-healed prior to doing another surgery.

    Several winters ago I slipped on half-frozen slush. My head bounced off a parked car door and i landed flat on my face. The nose bled some, but this stopped soon. However, in the ER waiting room two things were done. I was given a lockjaw injection. Then two "plastics" (attending and intern?) came over. The attending guided the intern's hands on either side of my nose, and she suddenly snapped my nose back into place. No numbing! I let out a sudden scream, and -one second later it was fine. Im guessing this may have been a displacement rather than a break...? CAT scan later to diagnose concussion and subdural hematoma. (Remember Ben Casey MD? Every episode seemed to include a subdural hematoma!)

    After rhinoplasty swelling is more challenging because your nose will look upturned short and piggy I'm two and half week post op and i can't see my face in mirror because i look like a pig but people who had this situation tell me it's just swelling don't worry so now it's a waiting game

    Thanks for posting this! I finally got my surgery date to repair my extremely deviated septum. I had major trauma to my face (centered on my nose of course) and now, do not breathe out of my left nostril. I also suffered some "tattooing" from hitting the pavement so hard.. There is a bump too. I'm just excited to breathe normally again. I'm 41,and would not be messing with this if I really didn't have to. Your video was very informative and I appreciate it.

      It doesn’t cause sleep apnea it just exaggerates the symptoms. Getting your nose fixed won’t fix sleep apnea which is a chronic illness

    Oh Man! I am waiting to hear the plan on my problem. I stuck a crayon up at 5 yrs old. After all the mothers on the block could not get it out a doctor thought he shoved it down my esophagus. Nope! It sat up in my nasal sinus, way up, for 20 years. At 25 I almost chocked on it. I am 67 and one side is completely blocked now from a deviated septum and hypertrophic turbinate. The sinus headaches are every morning. I pray it can be repaired as I have also lost my smell. On the sinus CT scan you could see one side is blocked completely. I am fearful of pain after. I have a tiny nose too so if it were to become smaller that would look very odd.

    My facial paralysis was caused by surgery to remove a brain tumor 2 years ago. I would explain the difference when someone would say I have Bell's Palsy. It's been quite a journey, but I'm happy to say now I'm a grade 2 on the House Brachmann scale. I was a grade 5 after surgery 😊 And I still see miniscule improvement to this day! Thank you Dr for this video❤

    Thank you for this, please my bell palsy is as a result of Ramsay hurt and I was initially diagnosis wrongly until in the second week of the onset. I was given Acyclovir 800mg and defal 30 mg for two weeks. I am in my fourth weeks now,I can close my effected eye now but my smile is still awkward. I still experience pains and ringings in my left ear and crawling iching sensation in my ear and throat. My movement is not balance whenever I am exposed to noisy environments. Please can I still take steroid tab in this fourth week? Thank you for your anticipated response.
