Last month, entrepreneur and venture capitalist Bryan Johnson released a CZcams video detailing his trip to Prospera, Honduras to receive follistatin gene therapy from the Minicircle Gene Therapy clinic. Bryan describes Minicircle's gene therapy as "a pioneering technology with the potential to improve muscle and strength, slow the speed of aging, and many more benefits".
In this episode, Matt reacts in real time to his first watch of Bryan's video and responds to Nick's questions about various topics that come up along the way. He provides a primer on gene therapy and its potential as a longevity intervention, discusses the clinical evidence for follistatin's effects on humans and model organisms, and goes through several things one might want to consider when deciding whether to partake in a new therapy. He also gives his honest take on "offshore medicine"-medicine performed in countries that lack the regulatory oversight of the United States Food and Drug Administration-and discusses potential mechanisms for safely offering experimental therapies in the United States.
Follistatin is a glycoprotein that functions as a potent inhibitor of several members of the TGF-beta superfamily, particularly activin and myostatin. It is involved in various physiological processes, including muscle growth and development, reproduction, and inflammation. By binding to these proteins, follistatin prevents them from interacting with their receptors, thereby regulating their activity.

    I wish there were places where you could tell a biochemist what you need, and they could develop a therapy, gene edit, or molecule to fix it or replace what's lacking. Maybe AI will be the answer. Nothing is ever going to happen if relying on the FDA the process way to long and expensive.

    To elaborate on the "cancer killswitch" point: If you've got cancer, that means that a bunch of other genes in the cancer cells have gotten mutated/switched on (oncogenes), while a bunch of others have gotten switched off (tumor suppressor genes). Moreover, the cancer cells that are driving the tumor growth have to have had repeatedly mutated in such a way as to successfully evade the immune system's innate anticancer defenses, in a malign version of natural selection. Thus, like Matt says, at that point the plasmid gene that initiated the cancer cascade likely wouldn't be playing any role whatsoever, so switching it off would be a useless endeavor.

    If Follistatin increases muscle, how many older people would not die from falling if they had more muscle? Didn't Dr Attia did a podcast on the percentage of people that die from falling due to lack of muscle approximately one year later?

    There are drugs where untold people have died just waiting for the FDA approval! A DISGRACE! With offshore protocols, who knows when they would get approved for anti aging because FDA didn't even consider aging a disease that we ALL die from. THE FDA EITHER NEEDS NEW LEADERSHIP OR DISBANDED! The FDA has made raw milk illegal!

    Bryan Johnson is playing an extreme game and he's definitely passed on the risk. However, I like guys like that in the sense that they can teach us a lot with their results. For people like me who want to tread a safer path, I'll keep an eye on developments. Assuming the people involved are honest on CZcams then what is happening is pretty cool!

    I did some digging on population levels and found the average follistatin level from various cohorts was 11-13.5 ng/ml. We don't know what assay they used, but on face value Bryan's pre-value seems low, and its perhaps no surprise they found beneficial effects in someone who was low to start with.

      The workout routine seems to be on the high rep side, with a little more emphasis on the upper body. Its not specifically focused on bodybuilding. I'd expect to see some updates to the routine over time.

    Thank goodness for the wisdom of MK! I actually have friends who are considering this gene therapy. Now I am armed with necessary information to forewarn my dear friends and potentially save their lives. After all, if someone wants more muscles, they can get a personal trainer and hit the gym and do the necessary work. Food, exercise, sleep!

      Consistent training would be the way to go before this if increasing muscle was the desired outcome. Bryan Johnson has of course being doing this, and has lots of imaging and strength measures to show that was very effective for him.

    I have myself studied, kill switches. When these are introduced into cells, they give one the ability to cause the cell to commit apoptosis. There are a number of animal studies, as well as in prokaryotes. I understand that Liz Parrish also had a genetic therapy. I believe it was used to extend the length of her telomeres. Your podcast are excellent. Love to hear your thoughts on the Liz Parrish therapy attempt as well as others.

    I'd like this format on a now-and-then basis, but don't turn it into a "reactions" channel. Follistatin gene therapies have been tried in various clinical trial settings in the USA. The delivery systems were different, but surely there is more info out there about the safety and efficacies measured so far.

      That's proven, so it works on thymus but a new thymus alone is not enough to counter the inflammation that accumulates over a life time. So there still needs to be a way to remove the trash from the body.

    I've heard you mention that a low-carb diet can be healthy, but a whole food plant-based diet can also be very beneficial. The latter often includes 60 to 70% of its carbs coming from healthy sources like beans, chickpeas, lentils, rice, and quinoa.

    I have no experience with mice, but it seems self evident to me that putting mice on a complete and sufficient exercise program is simply impossible. And thus any testing in mice related to an intervention that increases muscle size and strength is at best relevant to only sedentary humans.

    This new Nature study is bonkers, I'd love to see a video discussing it: " Inhibition of IL-11 signalling extends mammalian healthspan and lifespan." The study used knockouts and IL-11 suppression in late middle age. Very interesting results: The first genetically engineered mice so they were unable to produce interleukin-11 The second waited until mice were 75 weeks old (roughly equivalent to a 55-year-old person) and then regularly gave them a drug to purge interleukin-11 from their bodies The results, published in the journal Nature, showed lifespans were increased by 20-25% depending on the experiment and sex of the mice.

    I've been a follower, but pretending you know little about Johnson's practices is downright disengenous. Asking if he's 60 (when you know he's 46), is sadly juvenile. I'm sensing a degree of 'sour grapes...?'

      To be fair, I think he meant to say how many years he has to live till 120, "60 maybe 70 years".

      Think there is a miss commutation here! We both know Bryan Johnson is 46 years old. Just was a moment of bad mental math. Fun fact, I was a pretty early follower of his back and 2021 and we even briefly emailed. I still love his "Typical Blueprint Morning" video and some of his longevity suggestions/framework. The point of this video is to highlight gene therapy and Minicircle.
