Serrapeptase Protocol for addressing pain and inflammation - with Dr. Soroush Zaghi at TBI
The Serrapeptase protocol, originally developed by Jennifer Tow, is designed for post-op wound care following tongue-tie release.
Serrapeptase is an enzyme derived from silkworms that is commonly used for its anti-inflammatory properties and its ability to help break down non-living tissue, making it beneficial for reducing swelling, easing pain, and enhancing the healing process after surgery or injury. We are currently exploring this method in our clinic to effectively manage healing, particularly in patients experiencing excessive wound tightness during the contraction phase.
The treatment involves applying a quarter of a capsule of Serrapeptase under the tongue and to any necessary areas, helping to ease contraction and reduce pain and inflammation. Additionally, we are exploring this as an adjunct to myofunctional therapy exercises and stretches, as well as bodywork and 1064 nm laser therapy, all aimed at improving patients in wound healing integration and maturation.

      Thank you for your comment! Tonsils can become enlarged due to various reasons, including recurrent infections, chronic inflammation, or other factors. In some cases, allergies or exposure to irritants can also contribute to the enlargement of the tonsils. This issue can start at a young age, with many children experiencing enlarged tonsils during their early years. However, it can also persist into adulthood or develop later in life.     So do adults with large tonsils with sleep apnea have to get them removed? Are there other options that allow you to get off from using a CPAP machine

My husband has sleep apnea large tonsils, narrow palette, possible tongue tied too, and every dentist and ENT he's been to never mentions any of this stuff or looks at him like he's crazy for bringing it up. I would love for him and I to talk with Zaghi, i do think I have major oral function issues too. Also do you see infants? We have a 6 month old, her chin seems recessed and we also got her tongue snipped at 2 months but did no exercises before or after and it doesn't seem like the silnip made any difference. Unfortunately I only just started going down the my functional rabbithole and found out that tongue releases need prep and aftercare, her facial bone structure also seems more narrow (she was born a month early, don't know if that had any effect) how early can a child/baby be worked with?

    We agree with the midtongue restriction. It's accurate and easy for patients to understand too. I loved the explanation of how to use the palatal flutter in an assessment, we have added it to our assessments! This update will make us better partners with our referring docs and make it easier to explain treatments to our patients.

    does not look a fine work! why was nose opened from front, why not simply cut mucosa and take out the cartilage? how long did patient take to be symptom free of pain? lots of concerns!

      Thank you for your feedback and questions regarding the procedure. The choice to open the nose from the front was due to the septal deviation being too anterior. In such cases, open septoplasty has several advantages, including better visualization and access to the deviated structures, which allows for a more precise correction. In terms of recovery, the patient experienced 1-2 weeks of discomfort, which is typical for this type of procedure. The best results are observed around the 2-month mark.

      Not all tongue-ties are obvious. Some can be more mid-tongue restricted, and thus are not often not discovered by providers or patients who are not privy to the more recent advancements in screening and diagnostics and treatment protocols being developed and advocated for at The Breathe Institute and amongst the ever-growing community of TBI Affiliates and Ambassadors.       you can speak and do everything just as before, but easier after the procedure. Of course there will be some discomfort for a week or so, but so long as you do not perform extreme exercises with the tongue that could cause sutures to pop out, you should be able to speak and carry on as normal, of course considering any specific considerations your provider may discuss with you pertaining to your specific case and physiology.     For anyone nervous about the procedure, and this is coming from a person who's filed with nervousness and anxiety. I had issues swollowing food, and while I didn't have any pain it was scary each time I ate. That drove me to the point to face my fear and force myself to go get checked and eventually get the procedure done to get checked to see what the issue was. While I can't recall the medical term of what they saw, they widened something in my throat like 3mm, which has made eating again a joy. I was nervous while they were prepping me, but didn't even notice once the anesthesia hit, and then just woke up with my wife in the room... Go get checked and just do the damn thing, I'm 100% sure you'll thank you did.

    A jaw thrust helps collapsed / relaxed jaw breathing? Who would have thought? Everyone I hear in borderline agonal or even 'I snore like a madman / madwoman' when you thrust their jaw forward, they breath like a questionable marathoner. I am not a doctor, hell, I am not a certified forklift operator. Why is that complex result? Chiropractors (modern day witches posing as doctors) create appliances for this issue. Why is this a thing? Is this new age 'psycho-trauma' therapy? I am retarded (certified) but help me understand.

    More dentists and MD's should be recommending to get their patients to get a Lingual Frenectomy at an early age to prevent the speech impediment that most tongue tied patients have. I refer my patients as young as 1-3 years old.

    You did an fantastic job with the Lingual Frenumectomy with scissors and hemostats. I usually see this procedure done with the Diode laser.       Hi, thanks for reaching out! 1. It is of course a surgical procedure (albeit a minor one), but in skilled hands, the only discomfort should be relatively minor and will fade away within a week or so as the site heals following the release. 2. One of the things that makes our world such a beautiful place is all of the creative ways us humans adorn and decorate ourselves and the world around us. However we would be amiss not to also recognize that if ones goal is to have optimal tongue posture (resting at the roof of the mouth to enable easy, natural nasal breathing), attaching weights to the tongue, is not going to necessarily help in this. Also, having what is in effect a foreign object attached to the top surface of the tongue, will to some extent impede on it's ability to achieve suction against the roof of the mouth (of course the severity of this would depend upon the shape and weight and location of the piercing. Ultimately, a tongue piercing may to some degree counteract the benefits gained from the release. Potentially removing the tongue piercing at night, so that your breathing while asleep is not as affected could be something to consider as well. We are not here to tell you how to live your life, just to share what we have learned so that you can make the best decisions for your health and lifestyle :). Hope that helps.       thanks for the question, basically, the tongue is used to perform many daily functions (speaking, swallowing, eating etc, and when it is restricted, the body will often over-use other muscles to compensate for the lack of mobility in the tongue. this over-use can result in the neck and shoulder tension/pain etc you are asking about. So by releasing the fascia and/or mucosa/tethered oral tissues, the tongue is able to function optimally, and thus not require the overcompensation of the other muscles. Hope that helps.     As an adult how does getting a buccal tie release affect your physical appearance? Does the smile change ? Thank you

      Pre upper buccal tie releases I’ve had constant tension in my cheek area, a slightly sunken in cheeks look even at high body fat percentage, smiling hurt (without even realising until after release) and what I didn’t expect the cause of the majority of the tension in my neck on the sides reaching down to my shoulders (sternoclaidomastoid and levator muscles mainly). Also slight eye asymmetry and outer corners being pulled slightly downwards on one side. Now close to 2 months post release my eyes are more open and symmetrical, less eye strain and blurry vision issues. And with my relaxed cheek muscles they appear slightly more wide, round and not as sunken in (it’s subtle but looks better) and interestingly my nose looks thinner and more narrow now in a good way and while my outer nostrils appear slightly lower giving my nose tip a slightly more upturned look (this might also be from lip tie releases as I no longer have lip incompetence now). And breathing in air is so effortless now, my sinuses so much more free, my ear pressure issues improved. My mouth and lips look slightly more wide and full now and I’ve still got my dimples when smiling. But I can move my lips and laugh more independently from shifting my whole cheek area now. A lot of my tmj clicking and pain gone now as well. Definitely worth it for me.
