Fillable Document - How to make any Google Doc as Fillable Form

- This video shows how to make any Google Doc in Google Drive as live Fiillable Form.
Once the Google Doc is present as live fillable form, users can submit the form. The form responses are stored into a Googel Sheet. Users can generate document by merging the submitted data with the Doc and generate merged Google Doc or PDF. Finally the document can be sent out as email using a preconfigured email template.

THANK YOU SO MUCH. I had to keep pausing doing the steps and it worked perfectly. Your screen cuts off so doesn't show that when you opened the folder towards the end you clicked back on the previous tab to see the list to edit.

This looks great. Does the person filling out the form need to have this add on as well or just the person creating the form?

Hi Dietra Jones, Fillable Document saves the form responses into a destination Google Sheet (as per your configuration). If you are looking at publishing individual form responses into Google Sheet template, (similar to how it does Google Doc template), that is not yet there. It is in our TODO list to implement in the future.
This is amazing! I'm using it for my 7th grade science classroom fill in notes. Is there a way to default remove/uncheck "Show as placeholder" on all field list entries?
Andrea, thanks for your feedback. Currently, there is no bulk update for this on all fields You need to open the field and uncheck this if you do not want to show the name as a placeholder. This being a one-time setup, I guess this is not a big concern.

Thanks. You can configured the selected answers to be shown as bulleted or numbered in the generated Google Docs/Slides. This set up can be done in the field configuration.
Could you please do another demo where you feature a form? This is an application that I would use to fill forms where the critical features are lines & spaces. I'd want to see, how to adjust the size of the underline( the size of the field). Formatting the document is the critical topic. Thank You

You can simply use slider in the field configuration to increase height and width of your field size. Please check field settings

I wish I knew that add ons were required before I started. Not sure if you mentioned that in the beginning and I just missed it. My job won't allow add ons.
Yes, this requires add-on to be running. You could reach out to your admin to add this to allowlist. (many do that in the restricted environments)
Instead of filling de document, can I fill the spreadsheet? Or a xml file?

But are the users only those who are part of the company's Workspace, or can the forms be sent to anyone? If I wanted to create a form for our suppliers to fill out, can it be shared with them with no issues?
Yes, the published form can be done in two modes - anonymous and logged in user mode. If you are publishing anonymous mode, and share the link, anyone on the Internet can see the form. You can see more details at fillabledocument.jivrus.com/features/publish-form

The fillable document is great. When sending the document how do I change the content of the email. From having to change your info to my company info within the body of the email.

I hope you can still answer my question. So here it goes, I created a form and I'd like to send it to my suppliers for them to fill it up. My problem is, I can't share or publish the form unless it's all filled out.

I created a template document, and added about 21 fields. I filled them out on the Fillable Document and all went well until I Saved. A "Saving..." popup showed that "Packaging Form Response" was fine, and "Saving Form Response" was fine. But "DONE" never completes and it repeats the animation forever. Is there a limit to the number of fields, or are there any problematic combinations or formats or bugs we should know about.

I have tried reloading the page and filling out the form several times. (A pain because so many fields are required.) I have also removed several combinations of fields to find if there is a problematic one. I notice that the name of the form is at the top of the fillable form underlined with a red line, even before I start (which is, of course, produced by the "Open Web Form" button from the Add-On). If this an indication of a problem, I can look for a problem in the document itself, by recreating the doc/template. I'll start a ticket.

I've been using this for two months already and I can't seem to figure out how I can generate the documents in PDF file only. There is no option to generate PDF only but instead, it always generates both pdf and word which takes up all my generated document allotment for that month. DO you know where I can find that setting?
Hi, Yes, Fillable Document generates doc/slide first and uses it to generate other files like PDF, docx, rtf etc. If you do not want these doc/slide files, you can simply delete these

QUERY! FILLED DOCUMENT NOT FOUND IN THE DESTINATION FOLDER. After I clicked on Publish Form, generated an Anonymous Link, opened the link in a another browser, filled and successfully submitted the form, I went to the destination folder but the submitted document wasn't there, neither was it in the spreadsheet. But I found the generated documents in the folder, exactly as it is in the video.
The response gets stored into FD Output sheets of the destination spreadsheet. Please check if the generate doc/pdf switch is on to get the generated documents in the destination folder. If you still need support, please raise a support ticket at fillabledocument.jivrus.com/support

No, the form respondent (who submits the form) does not need Fillable Document when you publish the form. It will just appear as a web application. They won't even know that you are using Fillable Document.
How can I use this app on my business email account and not my personal acccount????
Hi Celia, if you are on GSuite, you can use your business email directly from Google Docs. If you are not a GSuite user and do not want to use Gmail but want to use your business (custom domain) email, you could register your business email with Google using accounts.google.com/SignUpWithoutGmail

How do I set a default value for a dropdown list?
Hi Alexander, Currently Fillable Document doe not have a way to set a default value for the dropdown list. We have this in our TODO list and we will implement it in the near future.
This would've been THE perfect add-on, buy I'm concerned about confidentiality (HIPAA compliance) since you're able to track use / submissions each month. Sad!
Fillable Document operates within G Suite permission control and hence users have full control. Only usage count is tracked for analytics and quota restrictions. You can read more on the privacy policy.

Fillable Document takes only the permissions required for it to operate. And it can operate as per your configuration. For example, you can generate a merged PDF and send as email through Fillable Document. To generate PDF file on Google Drive, it requires drive permissions and to send email, it requires mail permission. how to unpublish a form once it is published?
This one is coming with easy option in the new web app (currently in beta). In old web app, you can remove sharing from Google Drive to unpublish.
