    How do you handle prescription services for the expatriate community outside the USA

      Usually not an issue outside of the USA since medications are so cheap. I either work with a local physician locally to get someone their prescription or I've been able to have many foreign pharmacies honor my prescription though the patient has to take it there and they have to make an exception. If someone is living abroad long-term it's simple - they just find their own doctor there to prescribe things while I focus on all the other health related issues.

      The MPA under which your patient-doctor relationship will fall is determined by the originating site of the patient.

    nice video, how are you billing the pt? credit card , sqaure? is it built in in the EMR?
    how are you saving your website from hacking when out of 'usa,
    I am afraid of hacking.

      It's important to understand how websites work and how your location affects security risk. When in doubt you can have all of that handled by a third party. Hacking is likely inevitable; what information will be at risk, that's all that you can manipulate. Stripe is great and has served me well. And having my EHR company take on the risk of storing patient data adds another level of security. Purchasing cyber security insurance is an added layer of protection so that if you are hacked you can turn over the risk to that third party and continue seeing patients.

    Hello! Any idea on which commercial payers will allow me to practice from abroad or don't explicitly forbid it? I currently run a cash practice as a PMHNP and live in Colombia and Argentina for much of the year although I still keep my U.S. residence and pay taxes stateside. I would like to begin accepting private insurance if possible. It also seems like the DEA in their newly proposed rules included a clause that sates Telehealth controlled substance prescriptions can only be sent from within the U.S. regardless if the patient has had a previous in-person visit. Any insight into either of those issues? I'd hate to have to drop all my long established patients on controlled substances, but if the DEA goes through with those rules as I'm interpreting then my hands will be tied as I have no intention of staying on U.S soil full time.

      It seems like a complex move to make to go from a cash practice to a third party payer model because there will be many more hoops to jump through. Whichever commercial payer might be on the safe list today may not be tomorrow. I don't have any advice in this space though I suspect a dedicated insurance contract negotiator would be able to answer those questions for you.
