  • Value-Based Care Was First Put Into Place Almost 40 Years Ago by Medicare with Its Prospective Payment System.
    The Prospective Payment System Changed Medicare Payment for Hospital Inpatient Services from 'Cost-Plus' for the Hospital Services to a Fix Amount Per Diagnosis Related Group (DRG) Regardless of the Length of Stay.
    The Result:
    Length of Say Decreased 17%
    Admits/Discharges Decreased by 3.5%
    Readmissions Remained Unchanged
    30 Day Post Admission Mortality Increased by 0.6 Percentage Points
    Medicare Inflation Decreased from 10.1% Per Year to 5.7% Per Year.
    Why Was Medicare Able to Bring About So Much Value-Based Payment Change in 3 Years ('83-'86) Compared to the Relatively Little Change That Has Occurred in the 11 Years Since the Affordable Care Act?

      Biggest impact of Medicare DRG on surgeries is to move them from inpatient to outpatient… which is not subject to prospective payment.
