This is Yukios vision in this timeline, before the events of Days of Future Past. Logans death scene in Logan is her vision coming to pass in another timeline. It's really quite poetic... and heartbreaking.
No actually him "dying on his back holding his heart" is at the end of Logan when he dies against the tree holding his "daughters"(heart) hands
