    Thanks alot for your effort.. 10 y old girl CO poor vision for far . VA 0.2 ... Auto refraction s _ 1.25 C 1.75 in both eyes.. I tried first according to this and vision improved up to 0.9 .. then I did cycloplegic refraction and it was s+3 c 1.75 . In next visit When I put plus sphere the vision worsen even with small amount.. now should I go with pre cycloplegic refraction..or with post cycloplegic refraction despite vision become more pad

      The spasm of the ciliary muscle is responsible for this disturbance ... try to relax the CM then give the maximum tolerated plus and full astigmatic correction.

    Thank you for the interesting video Dr !
    I have a question about hypermetropia correction for children : 8 years old boy (no squint, headaches) with refraction before cycloplegeia OU +1 and after OU +5. Shall I give him directly glasses with +3 (frome the autorefractometer reading), or try a subjective refraction when he is dilated, or tell him to come in few days and check if he is comfortable or not with +3 glasses ?

      No squint, no headache with visual acuity 6/6 = No glasses.
      The correction of hypermetropia is prescribed in cases of squint or defective vision.
      If his vision is defective, you have to give him the maximum tolerated pus lens in trial few days after cyclopleia.

      For the maximum tolerated plus lens I begin with small + lens and increase until he reaches BCVA or invers I begin with high values and decrease ?

      @@tahachabchoub2250 I usually start with the smallest lens and increase it to reach best vision 6/6 then I increases it until the vision start to blur then I decrease the power to get rid of this blur (maximum tolerated plus lens).

    If the adult patient has accommodative spasm , i have to treat this spasm by cycloplegic drops and then give him the maximum tolerated plus lens ..right??
    How long is the period of treatment accommodative spasm ...tow weeks??

      If accommodative spasms lasts for a long time, then treatment can be considered. Simple treatments like meditation, relaxation, or taking a vacation can help. For students, reading glasses can be used (temporary in urgent cases). Another treatment is eye drops that can be used to break the focusing cycle. This is usually for few days.

    السلام عليكم
    Please I need your advice.
    I am 28 years old, I am hyperopic and astigmatic,
    I have been wearing glasses since I was 14 years old.
    During these years I always wore prescribed corrections after
    instillation of drops. The last measurement in 2019 I had
    OD +3.75
    OS +2.50 (-1.00 at 180 degrees)
    in 2022 I visited an ophthalmologist who gave me corrections without
    instilling the drops (without cycloplegia):
    OD +2.75
    OS +1.75 (-1.00 to 171 degrees)
    When I suspected these values he gave me the drops and found the measurements
    of the glasses I was wearing
    OD +3.75
    OS +2.50
    He told me to wear the ones taken without cycloplegia.
    I wonder if this could damage my eyes or accentuate my hyperopia in the
    medium and long term, knowing that I have always worn full optical corrections
    and for years?
    Do you think it is better for me to wear the new corrections
    (without cycloplegia) or to continue with the full corrections as before?

      Do not worry .... there is no correction that could damage the eye ... even the incorrect one. In hyperopia in adults, we advise the maximum tolerable correction, not the cycloplegic refraction.

      There is also the concern about amblyopia since my right eye isn't as good as the left one even with glasses. (Is that means I have amblyopia or lazy eye?) I've read on the internet that full correction is needed in this case. Sorry if i'm bothering you with my questions.

      Horopter is a line or surface containing all those points in space of which images fall on corresponding points of the retinas of the two eyes (See this picture en.wikipedia.org/wiki/File:Horopter.png).
