If you slow it down you can see that the box, once it hit Percy, fell off to the side. But (not shown in the video), after the scene with the driver, we find the box on top of Percy. You have to watch the episode Percy's Promise in order to find that scene.

0:55 Clarabel is facing the wrong way, and the art director didn't have enough time to switch it the right way while the show is starting

After the rain covered all over Percy's boiler and everything you can see Percy that already dried his boiler and his smokebox

Maybe they thought that the kids would think Clarabel would feel pain if they chopped her floorboards.

I really love how you explain about these goofs keep it up mate, but there’s a goof you missed “when Harold drops the hot drinks (on top of Percy Boiler) it landed in the river.... but in a close up it’s on Percy’s boiler..
When Percy plunges into the flooded tracks, Annie and Clarabel have swapped positions and the water appears to come above his firebox. However, in the next shot, the water is below his buffers.
When Thomas is on Toby's Old Tramway, Clarabel is facing the wrong way. Then in the next shot, Annie is facing the wrong way.
Despite the fact that the water only reached Percy's footplate, he has a water line across his boiler when he meets Thomas at the end of the episode.
In the shot of Percy's driver and fireman during the scene where Percy is in the flooded water, the fence behind them is submerged; but in the close-up of the fireman, the fence is completely above the water. This changes again in the shot of the driver, fireman and guard carrying the wooden planks through the water.
When Thomas talks to Percy at Tidmouth Hault, a red mark can be seen next to his left eye.
When Percy says "I wish I could see," his roof is lifted.
In the Polish dub, Edward's English name was used by accident.
When Percy rounds the last bend the puddle near the tracks contains black plastic.
The narration on the Trust Thomas and Other Stories DVD is severely out of sync.

The mud on Percy is definitely wrong because if the water was up to his nose he would probably drown though I'm not entirely sure if engines can drown or not

Furthermore, this story should've came to pass before Percy takes the Plunge, for this episode's events are mentioned in that episode

Why is there a brake van in percy this
Passenger train when Annie or Clarabel has a brake compartment
@General squirrel In the novel version of the episode, Percy’s train merely consisted of Annie and Clarabel, meaning Percy’s crew had to chop up some of Clarabel’s floorboards to use as firewood. In the TV series adaptation, a non-sentient brake van was added to the train so Percy’s crew could get floorboards from it instead.
