Hubris was working. The first double kill was giving you 19 AD - 15 + 2 per kill means 2 kills. Then, in the second double kill, you were getting 23 AD - 15 + 2 per kill means it registered 4 total kills since you had Hubris. If you think it should be 1 more, Hubris only counts kills since buying it, so your first kill made before purchasing Hubris is never counted.
Edit: didn’t realize the item had a tooltip counting “champions vanquished”. The item was working but had a bugged kill counter
He got a double kill after buying the item and it was only showing one stack, but 19ad so it was just a visual/ui bug.
The thing was it said "Champion Vanquished: 1". So the math part was right but the flavor text/helper was wrong. 10:42 for reference

My theory about the missing stack is that they happened so close together in time that the item's extremely low cooldown caused it to not get the other one.

Day 1 of asking please play gucci belt Tahm Kench. Heartsteel into 1 bami and 3 giants belts. The early extra health from all the belts will make you stack your hearsteel faster. And together with the bami you have full upgrade ability into better items afterwards. Bami and 3 belts is the cost of one full item plus component but a lot more health. Also possible on Skarner and Mundo
