    This isn't really a Let's Play, but instead I wanted to give a basic intro to how we stop and fix some heart disease and I decided to use the CardioEx cardiology simulator to demonstrate a few of the techniques I employ when unblocking arteries. It's a game developed *for cardiologists*, ie it's pretty detailed and accurate. I'm not a big gamer, nor am I trying to take on the Buzzfeeds and Dr Mikes with their 'Doctor plays' series, but there aren't many interventional cardiologists on CZcams (I know one other), so figured I'm the man for the job.
    To be perfectly honest, the last thing I want to do in my spare time is more cardiology, so I'm not entirely sure who will play this game. But it's certainly a useful teaching aid. With ridiculous music. I have no connection to LevelEx, the makers of the game. I contacted them saying I was making a video and they replied to say they'd forwarded to their head of PR who would get in touch but I never heard anything. You can find the game here: www.levelex.co..., it's free so why not give it a crack and maybe you can be sued for medical negligence too.
    I shot this quite a few weeks ago but haven't had any time to edit it until now (also, see above - I just couldn't be bothered to do cardiology after I'd come home from work). I tried to keep it short for CZcams, but left some extra bits in for the longer Nebula version.

    Fun fact: saying DES stents or PIN number is called RAS syndrome. RAS syndrome itself also suffers from RAS syndrome, as it stands for Redundant Acronym Syndrome ...syndrome.

      @@sensiblewheels If you use the wrist, and the patient dies, you get sued for millions. Never mind that it made no difference, the mere fact that the procedure used was different from the one other cardiologists in the USA use will be sufficient for a jury who is sympathetic to the dead patient's family to hand over the cash. In most other countries, a jury would not be involved.

    To be honest, I found this quite interesting as I am not in the medical field by any means. My Dad had a heart attack now 7 years ago, and thankfully can say he's healthy and well today. It was interesting to see the stint, the real one you showed and how you inflate it. I would be down for more medical equipment discussion in your expertise.

      FYI heart attacks hurt a lot. But the medical staff were all awesome

    Having paid into the NHS for umpteen years and hardly using it I decided to make a withdrawal and had 3 Stents at the same time. It really was the most surreal experience. The thing that sticks out is the pain suddenly going, instantly. The other thing that comes to mind is the Coronary after torture, sorry, care. Where an incredibly fit ( in the health sense ( well the other sense as well if I'm honest)) Nurse gets a pile of extremely unfit chaps to do exercises for a good hour a go.

      The 'fit nurse' is intended to get the patients working harder. They all think they will be 'in with a chance' if they work hard, but that they will look like ED patients if they don't.

    The one big impression I got here was from your description of the various arteries around the heart and how they all have names. I mean, of course they do -- that's not the surprise. What strikes me as amazing is just that in order to have predictable agreed-upon names, they most all mostly be the same from person to person. This branch has three sub-branches, then the middle branch runs a few centimeters and then branches off into two branches ... that sort of thing. But it all just looks utterly random to someone who isn't trained. The fact that all of these branches in arteries (and veins, and nerves, etc.) are predictable and mostly develop the same in most people just amazes me. It looks so random, and it's so ... not. I don't know how else to describe that.

      I think you'd be absolutely fascinated by Developmental Biology. Everything is coded and develops sequentially, so as far as the genetics are concerned the map can only develop one way. Well, unless something goes wrong, as it can.
      If we bring in some Evo to that Devo, we can see how that tracks over time through different species. A cool example is the Recurrent Laryngeal Nerve. In the embryo, thanks to having evolved from things that are a very different shape, the most efficient route for this nerve to take during development is to go under the aorta. This nerve, however, goes from the brain to the larynx, which are much closer to one another than either is to the heart, at least in the adult body. Meaning that, every time you speak, the nerve signals have to take a detour down to the heart for absolutely no good reason except evolutionary precedent.

I know it's just coded but it's still just damned impressive. I wonder what the balance is between how much is coded with simple algorithms and how much explicitly spelled out.

      @@jcortese3300 Oh, I get that! It's taken a *lot* of trial-and-error to get to consistently-shaped, large, complex structures. The sense of scale is mind-boggling.
      As to that balance, one sort of leads to the other. There's a *lot* that can be coded very simply, just through setting up extremely specific simple rules for processes to follow (like a Game of Life, if you know the concept). The memory isn't "read" so much as it is "executed", allowing iterative processes to take hold and develop instead of needing something that can interpret DNA in and of itself. When it comes to which amino acids are produced where, it's 100% spelled out, but pretty much everything after that is iterative.

    Seeing the different wires and tubes reminded me of something...
    I was sitting in a bar/pub once and heard some members of Slovenian parliament speaking about the number of different needles hospitals order "for no reason".
    The point was that buying more of something close enough to the right thing might save our health system a lot of money. While I understand trying to save money, there's a reason the saying is "just what the doctor ordered" and not "just what the politician/businessman/health insurance provider ordered".

      @@CED99 Nah, "chiropractor" doesn't mean the same thing in every country and even within places with effective science-oriented chiros they still vary frustratingly widely. Chiropractic isn't the right treatment modality for everyone or every situation, but when it's applicable and performed conscientiously, it's incredibly useful. I love this channel but my one gripe with people in non-manual-medicine specialties is their failure to understand the importance and usefulness of manual medicine. I had hundreds and hundreds of clients during my time in medical massage therapy, and many dozens of them kicked recovery plateaus that other modalities (including medication and surgery) couldn't kick, once they added chiro to their regimen. It's very, very useful. Just ... Also attracts people who happen to do a lot of loony garbage sometimes. It needs better definition/standardization through proper regulation, not to be mocked and mischaracterized to the point where it carries stigma and people who can benefit from it choose to avoid it needlessly.

    FWIW, the interventional cardiologists I've talked to here in the US (I'm an EMT) have all moved to radial artery access.

    I actually think you might find my heart interesting... I was born with a rhabdomyoma because I have Tuberous Sclerosis. They thought I had aortic stenosis at first so the prep'ed me for open heart surgery, but then found the tumour. The surgeon said it was enormous and didn't expect me to survive childhood, but it seems I am a stubborn bugger and I recently turned 40.
    Oh, and I had a femoral artery catheter when they tested the pressures in my heart.

    Its amazing how the technology changes. About 20 years ago my dad had a quintuple bypass. In the last year of his life he developed a disection.
    That is one cool app.

    According to the film director Kevin Smith, when he was being operated on for a heart attack the doc told him it was in the widow maker and something like 75% die(insert correct stat here) in a cheery voice and being a nerd KS found it very interesting. Afterwards lost a huge amount of weight by becoming a vegan.

    I'm part of the reason health care is so expensive in the US as I had a heart attack in 2015 "widowmaker" and recieved a stint through the femoral artery. Being poor (long story but you're not supposed to ever be poor in the richest country in the world but instead you should just died quickly and help the stats) I had no one way pay the staggering debt so the hospital had to write off the debt since the VA wouldn't pay for it because in my haste to recieve treatment I didn't get driven 70 miles to the VA hospital). But happy news, I've been getting steadily worse for five years and fully expect to not survive the next five years. And dont speak to me about socialized medicine because as an american I can be shot for saying that word and we can not have poor people surviving things. It messes up the success records. Cheers! (I love pretending to being English where I would eventually recieve treatment).

    So grateful for the training, dedication and research that has allowed for the treatment for my ongoing arrhythmia problem. Started as just the occasional heart skip and weird pacing, to full time A-fib. Shock treatments helped initially, but as my arrhythmia became insistent over 5 years, I had a couple of catheter ablations done. Zero pain associated with my form of arrhythmia but so wonderful to have all my energy back...

    Another great video!! When i had my ablation they went up through my leg, i asked the nurse why and she said "why take the side road when you can take the motorway"?? This was in Bristol about three years ago.

      Ablations are almost done via the leg as they go in the vein, not the artery! Much less chance of severe bleeding...and the veins in the arm are just too small for all the catheters the electrophysiology cardiologists stick up there! Sometimes 5 separate ones are needed. Hope you're well!

    My 22 yr old had a operation early jan and ended up in a coma because of blood clots going through his heart. So thankful for dr's who managed to come together to fix him (brain surgery team, heart team, then vascular team) they went in through his groin because they also put like a umbrella in to try and catch all the other clots.

      Yes that's very different - when we need to put bigger kit in, like new valves, or the umbrella thing you mention, or devices to grab things, we go via the leg for sure. Hope he did and is doing ok! Best wishes :)

    It looks like there are 3 types of operations: in vivo, in vitro, and computer simulations. As long as you are good in vivo, I would not worry about losing ’patients’ in simulations.
    I am sure most, if not all, the patients whose lives you have saved would gladly tell you that you have done a great job, and to send pyrotechnics to you if you wish.

    When I wake up in the morning I usually have a list of videos to watch that had been released since I went to bed. Whenever I see a Medlife Crisis video that is the first one I watch unless there's a This Old Tony video.... (yes I have a weird range of interests ranging from machining to medical to music) ANYHOW I got my new T.O.T. video yesterday so now I'm off to try this new game then maybe I'll make some hash browns. Thank you for everything you do and pointing out the ridiculous use of the term PIN number. I have been berating my friends for saying that at ATM machine for years.     So I stumbled onto this channel a while ago, I must thank you for putting me onto the show Succession excellent show. Then I went and had a heart attack a month ago so now I must thank you again for answering a heap of questions I didn't want to waste the Cardiologists time with. Cheers. The sections regarding putting stents in were truly uncomfortable to watch maybe its still a little too soon. You guys are amazing thank you.

    This turned out to be more interesting than usual!
    As it turns out, I am living with an Abdominal Aortic Dissection with Aneurysm, descending into the left iliac artery. It was discovered by accident (in a 2012 CT with contrast) at the Atlanta VA after years if me complaining about lower back pain and an occasional bout of Throbbing Kidney pain. In 2012 it was just the dissection, but at some point between then and 2018, it became "with aneurysm". Treatment for now is to try to keep my BP low (I am on 4 bp meds)... which, for me there are always complications, is difficult because I have hypertension, I am HIV+ and I have other problems like Osteo-Arthritis in many parts of my body, like both hands, wrists, knees, ankles and feet... Osteo-Arthritis in my lower and mid back and neck, with bone spurs pinching nerves in my neck and lower back, degenerative disks in my lower back, and a bunch of other things.
    My elderly mom had a Heart Valve replacement that worked kind of like a stint (She had a Tavr Procedure earlier this year and is doing great!).

    I really liked this video, taking a relatively realistic casual game and explaining extra bits - I liked for instance how you explained it's not easy to push stents through other stents, and I'd love if you explained how drilling an artery blockage (like you did at the end) is actually more complicated in the next episode :-)

    My interventional radiologist positioned the screen where i could watch during my last PICC line placement and it was facinating to watch the guide wire being threaded up my arm, across my chest and into my heart and then when the PICC was in place i had IV contrast flushed to confirm correct placement and seeing the the inside of my heart suddenly appear on the screen and then fade as the contrast was pumped back out was a strange combination of 'wow, thats my heart with stuff in it' and 'oh sh*t, thats my heart with stuff in it'. The interventional radiologist knows me well and has done over 10 procedures (PICC lines, NJ tubes, and PEG-J tube placements) on me and said to me while grinning to keep my head turned to the side so the PICC line didnt end up in my brain instead of my chest, and ironically 4 weeks later the PICC ended up migrating from my vena cava to my jugular vein pointing upwards.

      @@userPrehistoricman haha, i just have a lot of medical problems. Those 10 procedures are just the ones ive had done with that particular doctor. Ive had probably over 50 procedures of some sort and a total of 22 surgeries, as well as a ton of other medical stuff i have to do on a daily basis myself but i wont get into that. But, yes, my health likes creating problems faster than we can fix them 😂
