  • BASIC SET-UP: LACERATION REPAIR OF RIGHT ARM/ELBOW
    Alright so I did not see how big the laceration was so I kinda assumed it'll be something less than 5 inches and needed to be debride and cleaned up but it turns out that the actual laceration was a foot long lol and it's was infected and had some foul smell.
    So the nurse prepped the wound from upper arm down to the elbow. We draped the patient using half sheets, towels, stockinette and coban for the fingers, and an extremity sheet. Surgeon used the bovie to remove some infected skin. He also dug a little deeper so retractors were used. Ended up getting curretes cause this tray that I have didn't have it. It was a minor plastic. Usually I'd open a hand tray but this hospital don't have that. He gutted the yellow and green infected areas and used bovie for hemostasis. This took about 15 mins then we irrigated with baci and poly mix saline using a bulb and kidney basin. Once again, debrided the wound further. At this point I've used about 10 laps and opened some more. Surgeon asked for a jo drain fully perforated flat then we started suturing the skin with some 4-0 monocryls and nylons to finish it off. He used 0 silk to secure drain and a bulb was attached. For dressing he used 3 kerlix and a bandage to finish.
    Not all basic set ups are perfect since we can't really see how big or small the wound is. Sometimes you can ask the doctor but most times, you just adapt to the situation which is in this case, I assumed a very small tear but it ended up being longer than expected.

    Love seeing the similar supplies in the custom packs, also just for say, I hate that knife holder lol but love the huge mayo stand for the spine cases we do, also got way too long of a table but it’s all good for the 3+ trays that we typically open

    Do you usually keep your knife on the mayo? I’ve been told not to do that.

      Sometimes. I don't like it when a surgeon grabs it so I put it on my table but if I know the surgeon isn't like that I usually put it on. Its up to u how u wanna do it.
