I have the explanation You’re almost there Okay so when she showed the birthmark, someone or something had been watching them and had seen it (they might have been working for the circus) and kidnapped her before (I assume) torturing her making her the wrangled mess she was now.

So, basicly, someone or something saw the womans mark, which probably looked pretty strange. (Which already implies that they where being spied on) That "something" worked for the circus and kidnapped her because a weird birthmark+ some fake backstorie makes a great attraction. There they probably tortured her mentally and physicly until she became the soulles wreck she is now. This is how i understood it, hope this helped!
