  • Eye health is an area that often presents new challenges and discoveries, and one condition that has received special attention is retinal vein occlusion, a serious complication that can significantly affect vision. This article explores how the epiretinal membrane can influence the treatment of venous occlusion, based on recent studies and clinical practice. What is Retinal Vein Occlusion? Retinal venous occlusion occurs when a vein in the retina is blocked, preventing proper blood flow. This leads to fluid buildup and swelling (macular edema), resulting in blurred vision and, if left untreated, can lead to permanent vision loss. The Role of the Epiretinal Membrane The epiretinal membrane is a thin film that can form on the surface of the retina. Although it can appear idiopathically, it is common after vitreous detachments or in conditions such as diabetic retinopathy. The presence of this membrane can further complicate the condition of patients with venous occlusion, influencing the effectiveness of available treatments. Current Treatments for Venous Occlusion Historically, macular edema resulting from venous occlusions was difficult to treat. However, in recent years, effective therapies have emerged such as intravitreal injections of antiangiogenics, which help to reduce swelling by interrupting the abnormal flow of fluids into the retina. Antiangiogenic injections Medications such as Eylia are often used to treat macular edema. These injections are applied directly into the eye and work to normalize fluid levels in the retina. Use of Corticosteroids In some cases, agents such as Ozurdex, which contains dexamethasone, are used. This treatment is also administered by intravitreal injection and slowly releases the medication over several months, offering an alternative for patients who do not respond to antiangiogenic drugs. Impact of the Epiretinal Membrane on Treatment Recent studies indicate that patients with an epiretinal membrane may require more treatment applications to achieve the same results as those without the membrane. This suggests that the presence of the membrane may interfere with the effectiveness of venous occlusion treatment, complicating the control of macular edema. When is surgery necessary? In approximately 10% of cases where the epiretinal membrane is a significant factor and there is no response to conventional treatment, vitrectomy can be considered. This surgical procedure involves removing the membrane and can improve vision, although it is a delicate option and should be carefully evaluated by the specialist. Conclusion It is crucial that patients with retinal vein occlusion and an associated epiretinal membrane have close and personalized medical follow-up. The choice of treatment should be based on a detailed assessment of each case, taking into account all the variables involved. Remember to discuss all options with your ophthalmologist to ensure the best possible outcome. Staying informed and seeking early treatment are the best strategies for preserving eye health and quality of life.
