      Some people experience massive negative side-effects from ADHD medication. So I disagree with “there is no reason why they shouldn’t be taking Adderall or Ritalin”. There are massive reasons why they might not want to, be able to or need to. For specific people.
      Some people have ADHD but are able to manage quite well without medication at all. And they may not WANT medication. That works… for them.
      Finding the right medication (if you choose to go down that path as I have - and I have positive experiences in this regard) can be a long process which requires patience and a lot of trial and error. Precisely BECAUSE we are all individuals and what works for me may be a disaster for you.
      I am deeply suspicious of such sweeping, broad statements, because they tend to be wrong.

But now I just read what could be your most sympathetic comment you've expressed on the condition. Now I feel bad about reading your name and automatically feeling nothing but ire.

      @@alanberkeley7282 Someone that has studied the condition for decades? The mechanics of the medication appear to be sound as well: 1.The patient will generally have lower concentrations of neurotransmitters in their brain (hallmark of the condition). 2. The medication is supposed to upregulate these concentrations to a point where they can control their focus. Of course, it's more complex than that, as different medications effect different brain regions from the prefrontal cortex (executive functioning) to the limbic system (emotional processing i.e. "feeling zombie-like"). But if there are adverse reactions and medications aren't for them, then it's not the end of the world! We just have to focus on other methods. Medication is just more immediate in response most of the time. I've seen many people come to a realization of needing to be diagnosed after "popping an addy" at a party and find it it helps instead of inebriate! I just learned that lactate is a preferred energy substrate for neurons at higher firing rates, which is produced by physical activity. Thus, as people with ADHD and/or autism (a fairly common comorbidity, which I have) begin to stim through whatever repetitive movement, they are creating *more lactate for their brain!!!*

    I too, wish I knew these details years ago. It’s of paramount importance to point out, that not every individual with this clinical diagnosis, will exhibit all the behaviors he mentions. Even the degree to which a classic ADHD behavior is expressed will vary tremendously from person to person. With respect to another comment in this feed, that he is all “doom and gloom”, I ask you to step into his shoes, and view his presentation style as the product of decades of knocking his head against a wall, a wall comprised of hundreds of his most influential professional colleagues, who refused to open their minds to the fact that ADHD is real, and is a behavior-altering condition. I can appreciate how frustrating it’s been to advocate for your patients, in an environment where everybody thinks you’re making this all up. Eating disorders also fall into the same category, where, for years, psychiatrists believed their behavior was just a way to gain the attention of others. In both cases, the main issue is that health professionals are comfortable treating patients using techniques and methodologies they learned long ago. Most are extremely reluctant to do the hard work required to analyze if what they believe, will actually withstand scientific scrutiny. Kudos to Dr. Barclay for putting his patients’ needs first, and in so doing, put both his professional standing and reputation at risk.

    Teachers need to be taught the functionality of mental disorders, as they play a large part in the shaping of a childs future. Good teachers equal smart children, train a teacher how to properly teach a child with ADHD and he will be less frustrated and so will the kid.

      +Stahpitt Teachers also need to be taught how to spot it and that children can have ADHD without jumping off the walls and being consistently disruptive. Most, if not all, of my elementary school teachers noticed I had the classic problems caused by ADHD (forgetfulness, distractibility, slowness, failure to complete assigned work) but none of them realized that ADHD was the issue, probably because I was shy and quiet. I only got diagnosed at 23 years of age and I always wonder whether my life would be different if I was diagnosed younger.

      Parents should be taught this, ultimately we are responsible for the development of our children. And teachers should be paid more, I had teachers in SC with Master's or PHD's that barely cleared $38,000. Not in the 70's either, I graduated High School in 2008.

      @@alanberkeley7282 I really do think big Pharma is evil in many ways but just like vehicles and butter knives and ropes Etc there really are drugs that are extremely helpful and also drugs that do more harm than good. Just like you can use a rope to pull a car out of a snowbank or hang someone drugs whether they come from Big Pharma or little Pharma or a mushroom can go both ways

    It's hard enough for an mental health outsider, layperson or family and friends to get bipolar 1 (rapid cycling), but when you try to explain ADD, they tell you to make lists and set reminders (all of which I continue to do, and they're of assistance, but Not in the way I profoundly need assistance.
    So when I am late somewhere, which is 99% no matter how early I start or how many 15 min reminders I set along with the final reminder... I'm STILL Late and apologising! I always feel horrible and know they think I simy have No respect for their time, which is So Far Away from the Truth.

    So what the hell am i just doomed to not complete anything in my entire life half assing everything. This does ake a lot of sense to me though thank you GOD for that. ADHD has bin devastating to my life. That a major understatement to 😑

    My life story. A huge project with a hundred others started every few minutes, become frustrated, wishing I could remember how to kill myself and then distraction again what's happening outside.....memory comes back abt the huge project, anxiety attack, forget it I am going to sleep in bed all day.....ending with everyone asking me why am I so weird and that I must be feeling so pathetic bc everyone else is successful but I am not so we don't want you around.
    Mention medications? Now I am not only lazy I am a druggy.

      Sonia Sohi.. Always remember the same people who would call you druggie will reach for their pain killers without thought! I’m 42 and I’ve had this my whole life just like you. I got diagnosed at 27.. When I was young there was virtually no such thing as ADHD... I was just a lazy distracted, inattentive, spontaneous, little shit that probably just needed a good whipping or two. Which I got and duly earned every one! I never understood why I was so different and had so much trouble at things others took for granted. Back then people would see me doing things in full ADHD mode and wonder where on earth my head was at. “He is sooo smart if he would just.. etc. etc.”
      Obviously this effected social interaction too which is problematic in the throes of puberty...
      Here’s how I have won..
      I simply OWNED it.. I knew way early that there were things about me that would probably ALWAYS be that way.. and then I very painstakingly observed EVERYONE.. ALL the time. I got “you’re weird” from many girls back then. But I embraced it as part of me! I somehow learned how to use the other characteristics to my advantage in my adult life as well and I promise you that if you can harness that which everyone else considers ignorance you’ll go further in your endeavors than they will in theirs! I’m a chef for example.. it’s constantly moving but NEVER monotonous. The hours.. that’s another discussion, but NOT everyone can do it. It takes a natural multiple tasker to cook high volume, high end restaurants. I’ll shut up.. you get it!
      People who are judging you don’t have enough going on in their own lives!
      Be who you are no matter what..
      I think it just took me a half hour to write this..

    I think just as unhelpful as “attention” in ADHD is the word “deficit”. There isn’t a deficit of Executive Function, there’s a disfunction or disregulation. As anyone with ADHD will tell you, there a plenty of tasks they can do where their executive function is not only not in deficit but almost supercharged (likely to the detriment of something else they were supposed to do). “Deficit” is merely a lack of something.

    In my case, I am quite fine with behaviors related to driving, spending money, etc. I’m not so good at inhibiting some behaviors that emerge during social situations. I don’t interpret someone’s body language well. I had no clue that ADHD was affecting me when I ran my own business for 40 years. I just accepted the fact that not everybody is good at managing people, and I countered that with knowing how much repeat business my customers gave me. Only after selling my business, and becoming someone’s employee, did I realize how real a disorder this is, and its potential for angry verbal outbursts. My only other comment, is that I have the benefit of hindsight, whereas a parent listening to what he is saying, may likely think their child is destined for an unhappy life. They will then try to prevent this by acting in such a way, that creates a self-fulfilling prophecy. That then becomes an additional problem. It’s important that both children and parents receive professional counseling. There is no doom and gloom here, provided both parents and children understand how to stack their deck for success, and how to avoid situations that are more likely to set them up for failure. Pay for professional help. Don’t waste precious time trying to do it yourself.

      I put it to you, the only thing worse than having ADHD is not realising that you have ADHD.
      Knowing that you have a disorder is the beginning of recovery.
      Like Allen Carr says about addiction, understanding that you're in a prison, is the first step towards getting out of it.

      You are not useless. You need treatment; just like someone that has any organ in their body out of alignment. If the heart needs something then we give meds, if the lungs need something we give meds, in ADHD the person needs something to lift the fog and veil for the brain. Too often ADHD is misdiagnosed as depression and the brain is given the wrong medication. This video and Dr. Barkley are trying to teach medical providers to recognize ADHD and actually treat it and treat it appropriately.

    Seems to me like he’s thinking of it in the wrong direction. “To hierarchically organize behaviors to meet the complex structures” of thinking just seems backward.
    Evolution would have made the ability to act based on instinct first. The more complex modes of thinking would have come later.
    I have no issue with complex thinking. I’m happy to talk abstract philosophical theories with the best of them. I have fantastic and wild ideas that I think through in their entirety. I have an almost obsessive interest in efficiency, and love to get my hyperfocus on organizing things into the most efficient way it can be. It’s the basic ability to get the complex ideas down into the simple actions that I struggle with.
    Get up, get moving, shower, do dishes, call people back, pay bills, clean closet, etc.
    Top down problem, not ground up.

      I thought he was referring to complex structures in our society not complex, abstract topics. ADHD actually is correlated with high IQ and, therefore, the abilities you speak of.

      I thinks the talk sounds too good, and DJ makes a point. Not because a bottom-up approach is the answer to everything as Kristian says, but because tracking the logic biology solved is the way to undrstand what the actual brain does. The idea that the brain describes every behavior in time and in it its complex structure, sounds very nice and powerful, but it is not entirely true. Biological solutions sometimes look nothing to do with what expected, or wise at all.
      The same applies to how ADHD develops. I really value this talk and others from him because it is a brilliant attempt to put together and makse sense out of lots of resaerch and clinical work. His contribution is enorous and probably is true about many moments of what someone with ADHD goes though. But research does not stop. The way each organism develps, attends to the enviroment, organizes behavior, responds to different modalities or multisensory modalities, can be very different; and of that we know very little.

    The irony is that I am excellent at project management and also have ADHD. My ADHD is comparatively mild now and seemed to improve until perimenopause, but day to day, PMP education & meds make a good quality of life possible.

    Anyone here who wants to better understand ADHD should read Driven to Distraction by Edward Hallowell.

    Even if a fancy word like Self Regulation Deficit Disorder (SRDD) might be more appropriate, and if it would replace the “good old” acronym ADHD, you might find an entirely new bad mouthing of the disorder based on will full ignorance, or simply inertia. Self regulation disorder could be connected just as well with stupid remarks about people who can’t contain themselves, don’t know when they are off the beaten path, or even can’t hold on an urge and pee in the pants. I’d say, don’t change the name until a correct understanding has become mainstream in common discourse, and not just among the educated. Because otherwise we might soon enough find yet another undesirable battle under the rubric of “political correctness.”. Think about it: it’s now not too horrible if you tell a friend or boss you have ADHD. Now tell’em you have a self regulation issue, and they might get scared that you can’t contain yourself and get into a rage easily - Or that you just can’t stay away from slot machines etc. Rather continue fully on finding the actual nature of the disorder and educate the public. The name isn’t the real issue. If at all then a name like Executive Function Disorder if made official sounds certainly better. But then imagine someone mentions this in a job interview where it’s about an executive position of some enterprise. I can hear the buzz in the head of the head hunter. Never underestimate how much damage can be done with naming of categories.

      Hello, thank you very much for your thoughtful firesight of what is possible with the possible changing of name from Attrntion Deficit Disorder to any of the other discriptive sentences. I too agree that this does need to have a lot of considaration into the possible negative and to into more possitive conertations of what a new disxriptive title of the dissorder called ADJD could cauase society inclyding academics and nit only us the laymen to think aboit ADHD

    Is it common for those with ADHD to believe they can learn self-regulation without meds, or is that generally a behavior more exclusive to bi-polar disorder? I'm aware that there are overlapping symptoms with both.

    I want everyone to know that since Dr Barkley has given these speeches there's evidence that Meditation and CBT help to improve the working memory in people with ADHD, thus becoming a real way to chronically treat ADHD without the need for medication.

    Maybe because I was raised with strick parents And when I was being told to do something.I had to do it right then and there ..STOP WHAT I WAS DOING AND DO WHAT THEY ASK OF ME...UNlike my bf he is mamas boy. And he is so slow in doing anything.....And he has slite ocd...

    "Some hyperactive children can develop tics or even Tourette syndrome in response to methylphenidate and perhaps to other stimulant drugs as well. In some of the 20 cases reported in the literature and in two cases in my own clinic, hyperactive children with a prior history of tics developed Tourette syndrome after being treated for only a few weeks to a few months with methylphenidate. In these cases, the Tourette syndrome did not disappear after withdrawal from the stimulant drug. The children are now being given haloperidol for symptom control.

    Awesome information. My thoughts:
    I think that we have to start by not calling it a "disorder" first. Just a different way of being, a different way of thinking, a different approach to problem solving. ADHD is only a "disorder" in contrast to what Western society expects of the individual. I think telling people, specially children, that something is wrong with their brain, sabotages their growth as Human Beings and it hurts their self-esteem in the long run. Instead, we should be honest about the fact that the economic system just wants you to be a production-consumer bot and A LOT of people need chemical assistance to be a bot since we are Humans not robots.

      In my opinion, ADHD is not just a disorder because of western society. In any productive society, planning, time management, working memory, self-motivation, impulse control, emotional regulation, and attention for long periods of time are all required to function properly. All of these are significantly impaired for people with ADHD.
      Personally I don't see my disorder as a disorder because it makes me different from everyone else, or that I'm just not compatible with society. I see it as a disorder because I can't read anything longer than a few sentences without my mind wandering off if I don't take my medication. I see it as a disorder because I just can't do the things that I tell myself to do. I see it as a disorder because it keeps me from exploring things that I genuinely love. I don't see it as a part of me, I see it as something that prevents me from being myself.
      I don't know, that's just my two cents.

    When Keith Conners found out about Richard Fee's addiction and suicide he by all accounts was shaken, stunned and upset. I heard on the grapevine Barkley was indifferent like he'd heard his neighbor had got the cold. He is totally lacking in ethics. His own research in 1978 showed that kids who take Ritalin don't do better in school than those who don't. Yet he accuses parents who don't drug up their kids with stimulants of being child abusers. He is unforgivably arrogant! Connors like Allen Frances, seemed to have retained some sense of his humanity over time, and back tracked on a lot of things he said earlier in his career and ended up thinking this whole "ADHD" thing had gone way too far. Barkley, on the other hand, is utterly without redeeming qualities, he is completely remorseless and he continues to lie and cheat because his reputation depends on him being "right" about things he knows are not. He knows he's talking shit, he's lying or telling half truths, he knows about Richard Fee committing suicide and rehab being full of Adderall Addicts or people committing stabbings and shootings whilst on it and still he carries on. That's a serious lack of moral compass there. I'd argue he's the one who should be committed if anybody. To protect the public from him. Conners at least showed he had a conscience, did feel guilt and remorse, and has spoke out in the last few years. Same as Allen Frances. Looking at Barkley I can see sociopathy and narcissism there. He's unmoved by what's happened in the US, with stories such as Fee's, still says it is under diagnosed, and is somebody who just can't admit he's wrong or back off from making the bullshit statements he's made the last 25 years or more. Morally and ethically as well, how can we tell kids Heroin, Cocaine and MDMA are bad and dangerous which they undoubtedly are, without question, when in the same breath we are giving them Amphetamines and Stimulants for them to treat a dubious condition. It's absolutely appalling and immoral to drug kids because they fidget or look out of windows or talk a lot or run around. It's outrageous. All because a Teacher who drones about George Washington or Algebra bores them. And there's evidence it could set them on the road to drug addiction as they will seek out further drugs as they believe it is acceptable to take drugs in the first place. I have long believed that Athletes should be banned for life if caught taking steroid enhancers, so kids taking Adderall to pass exams and get higher grades even though it has been proved to be bullshit, is just as bad.

      He has said the opposite of what you just posted. Perhaps why this video is specific for health care providers (I can understand this being hard for non medical people to follow). he is actually bringing light and awareness to the REALNESS of ADHD.

    Conners has expressed remorse and in 2013 apologised. Allen Frances similarly has expressed regret and remorse. Even Ned Hallowell as awful as he is, and as full of shit he is, has expressed some sort of remorse for what has happened even though it didn't last long and he's gone back to how he was 15 years ago, but Barkley is utterly remorseless, totally shameless and unapologetic. Despite the evidence there in front of him, 15% diagnosed if not more, rehab full of Adderall addicts, Richard Fee, ER having Adderall users present, he thinks not enough kids are diagnosed or adults, thinks more should be and is pushing another bullshit disorder called Sluggish Cognitive Tempo which the DSM rejected so he can get more kids being drug addicts.

    Barkley has said Adderall and Ritalin is like eyeglasses to the person with ADHD. I wasn't aware of there being a black market for eyeglasses on college campuses

      As a provider than cares for those with ADHD I can tell you that ADHD is very real and treatment is life changing. In healthcare we give the green light to treat DM2 with insulin, and teach dosing based on blood surgery at that moment (very similar to how ADHD patient's take their medications- when educated properly by the provider), we give ibuprofen for pain when studies show the damage NSAIDs and Tylenol have on the liver ...but not one eye batted. Yet you get on here and damn treatment for the brain, which like the liver and pancreas is also just an organ. However the brain is the organ that allows the human to download and deciphers data..something people with ADHD can't always do (some days better than others). Do you feel they should be tossed around mentally like rag dolls? Or live with anxiety and depression and be placed on SSRIs that make their symptoms worse, often to the point they can't function. Perhaps instead of criticizing someone's life work, or judging a medication that one has no clue why you have such a distaste for, I suggest you try talking to someone (an adult) with ADHD that takes a small dose of a stimulate find out how their life is changed. Please stop posting on this video with useless opinions. Thank you in advance.

      Two diabetes drugs - Ozempic and Wegovy - are now being sold on the black market as weight loss aids for those who don't have diabetes. Does that mean diabetes doesn't exist now? Or that these meds are ineffective at treating diabetes?

    Barkley is also a total hypocrite. His own research back in 1978 showed that "ADHD" kids did not learn more on stimulants. Yet he made his career claiming that not "medicating" "ADHD" kids was tantamount to child abuse. To promote something that you know could be harmful when you know it doesn't really improve outcomes and make money off of it - sounds pretty sociopathic to me.

    The effects of Ritalin don't last very long in the body? That's absolutely wrong. It's incredibly disheartening that people are willing to drug their children in order to not face their irresponsibility as parents. They feel that it's much easier to just stigmatize their child with a lifelong illness and then drug them senseless - denial is comfortable. Drugging should automatically qualify as child abuse.

      Christ what a fµcking mor0n. It has been shown TIME and time again that ADHD is not caused by bad parenting, or even diet choices. It's been researched since at least the late 1700s. Acquire some knowledge instead of trying to deny a neurological disorder that thousands of people are suffering everyday and of whom many HAVE tried therapeutic approaches for, to no avail.
