If ultimately approved, it would be the first antipsychotic treatment that does not target the D2 dopamine receptor.
      This following is what info that I got off the internet. They are rolling this out this year!
      A new medicine for treating schizophrenia-one that appears to help reduce both positive and negative symptoms of the illness-has passed a first hurdle in phase 3 clinical testing. Phase 3 is often pivotal in deciding whether a medicine is effective and safe enough to obtain FDA approval.
      The medicine, xanomeline-trospium, is called KarXT by Karuna Therapeutics, the company that is developing it and which paid for the initial phase 3 trial. The drug has a novel mechanism of action that distinguishes it from all previously approved antipsychotic medicines.
      In December 2023, the pharmaceutical giant Bristol-Myers Squibb Co. entered into a deal valued at over $14 billion to purchase Boston-based Karuna. The announcement came just weeks after positive results of the first of two positive KarXT phase 3 trials were published in the journal Lancet. Senior author of the paper reporting the results was Steven M. Paul, M.D., a BBRF Scientific Council emeritus member who is currently Chief Scientific Officer and President of R&D at Karuna. One of the paper’s co-authors was 2007 BBRF Young Investigator Christoph U. Correll of Northwell/Zucker Hillside Hospital.
      252 patients with acute psychosis requiring hospitalization were enrolled in the randomized, double-blinded, placebo-controlled trial. Half received KarXT for 5 weeks and half received placebo. KarXT was observed to significantly reduce both “positive” and “negative” symptoms of schizophrenia compared with placebo. In addition to reductions in both kinds of symptoms, patients receiving the new medicine in most cases were able to tolerate it well, reporting only moderate side effects. Larger and longer phase 3 clinical trials are now underway.
      KarXT is the culmination of research begun decades ago to find a new way of treating symptoms of schizophrenia. Since the first antipsychotic medicine approved in the 1950s, every antipsychotic approved to date targets a cellular receptor for the neurotransmitter dopamine called the D2 receptor. Some “atypical” or second-generation antipsychotic medicines, including clozapine, also have important therapeutic effects related to their impact on receptors for serotonin. Both first- and second-generation antipsychotics are often very effective in reducing delusions and hallucinations that are the chief positive symptoms of the illness. But they have essentially no impact on negative symptoms such as blunted affect, anhedonia, lack of motivation and asociality, and no appreciable impact on cognitive symptoms that are also among the core symptoms of schizophrenia (reduced executive function, difficulty in sustaining attention, impaired long-term memory, among others).
      An estimated 30%-40% of schizophrenia patients are resistant to the therapeutic benefits of current antipsychotic medicines; others derive only partial positive-symptom benefits. Short- and long-term side effects associated with approved antipsychotics are also an issue for many patients, and range from motor impacts, such as akathisia, Parkinsonism and tardive dyskinesia, to cardiometabolic effects including weight gain, lipid and glucose abnormalities, hyperprolactinemia and sexual dysfunction, as well as somnolence and sedation.
      The idea that led to KarXT began with the aim of developing a drug with a novel mechanism of action-one that would not block D2 dopamine receptors but rather would stimulate cellular receptors called the M1 and M4 muscarinic receptors. These receptors are part of the cholinergic (acetylcholine) neurotransmitter system. The theory was that agents targeting the muscarinic acetylcholine receptors might indirectly impact the balance in the brain between the dopamine and acetylcholine systems, including in the brain’s striatum, which in turn might help therapeutically address pathology that gives rise to psychosis.
      For many years, preliminary tests of medicines targeting the muscarinic M1 and M4 receptors suggested that they had excellent potential for reducing schizophrenia’s positive, psychosis-related symptoms. The problem has always been side effects: the early candidate drugs had significant side effects in the body’s gastrointestinal system, including nausea and vomiting. To potentially overcome this obstacle, developers of KarXT have tested the idea of combining a compound (xanomeline) that stimulates the M1 and M4 muscarinic receptors in the brain with a compound (trospium chloride) that blocks the M1 and M4 receptors in bodily tissue outside the brain, including the gastrointestinal tract. In phase 1 and 2 trials, KarXT appeared to demonstrate antipsychotic efficacy while reducing the frequency and severity of gastrointestinal side effects.

    Don't judge Lauren until you have done your own research. After a month of using Metabooic therapy, my life has completely changed. The debilitating depression is gone.     I am the biggest skeptic there is about the keto diet due to the increased risk of cardiovascular disease and cancer and for the majority of the population, the risk outweighs the benefit. However, because of your videos, I spent time on pubmed learning about the specific psychiatric and neurological conditions that can benefit. Only the patient can decide that what they are willing to risk and sacrifice. You have done a fantastic job of warning the viewers that your plan is under professional supervision and to not try this alone. Well done.

    People are saying this video is irresponsible, but I don't agree. I see this situation as a possible hopeful and relieving option for possibly, a lot of people, even it doesn't help most. This needs to be talked about. It's great news. I understand that it can feel hurtful to see people like you getting better and you are "left behind", but you are not behind... you just struggle with something that you have no control of. It's painful but the solution is not wanting to gatekeep those who get better. We have to create a sense of community with each other, so we can improve each others lives. In this society is hard, but we can slowly change things through awareness, empathy and altruism.

      It is just an anecdote, not a scientific study. Furthermore, she is claiming her symptoms are in remission when she hasn't completely weaned off medication. That is reckless and is giving people false hope. People who go down this path, especially without support are going to end up in terrible places.
      If her doctors are recommending a step-down from 40 to 20, those doctors are incompetent. That is too much, too fast. In my experience, doctors don't understand the pain of withdrawal. It is something that they read in a book.
      I hope for the best, but she is ignoring that keto is dangerous and I fear a crash that she might not recover from once the meds are completely out of her system. That would be awful for so many reasons. I would be devasted to see her relapse and I am afraid of what it would do to her spirit. It is her life, but she is being reckless.
      It would be great if it worked for her, but she is risking harm to others and she doesn't seem to understand that. I have similar diagnoses as she does and have been med-free since 2017 because I am very sensitive to psych meds and I hate them, but even I am not this reckless.
      The Keto movement is filled with charlatans, there is some promising work but there is also tons of junk science done by what are basically third-grade dropouts. Anything that attracts "intellects" at the same level as antivaxxers, covidiots and urine-drinkers is something to be skeptical of.

      Also, going from 40 to 20 isn't a big jump if you started from 100. 🙂 Your body is already used to receiving lower & lower medication, so it isn't a big shock anymore. As she's shown with her withdrawal ending quickly.

    A few years ago I did a med taper too. It was also under supervision, but I was so intent on going off meds that by the time they were becoming concerned there was no way I was going to listen to them (I was also becoming delusional). I did end up in the hospital again (for a very long time) and meds were reinstated. HOWEVER, I am still very glad I did it. Because I hadn't just thrown them in the trash. I did a very slow taper, the correct way, and still ended up psychotic. I really needed to know if I could live off meds and to me it was (sadly) proof that I needed them. I can't stand being on meds and feel they are toxic. They are certainly overprescribed and a lot of people take them who don't need them. But I tend to doubt a psychotic illness can be controlled by diet. It just feels too strong and would also depend on stability to be maintained.

    Reading about this made me shake my head so I googled it and I saw Stanford had results of a study they did which showed successful results. That’s enough for me. I’m happy to see you doing well and you seem better than I’ve ever seen you. This must be completely life changing. Much more I’m interested in understanding but I’m thrilled for you.

      thank you for being open minded!! medication is not the only solution/treatment for mental illness and it’s really important that other people are aware of other effective options, especially considering how rough antipsychotic medications are for most people. side effects are a major reason for non-compliance, so if there are other options with much fewer risks/side effects people need to know!!!

      The Stanford study is a good start for the research, but they didn't use a control group so it is difficult to tell (at this point) what caused the improvement. It certainly could be the keto diet, but it could also be just an improvement in diet from the typical North American diet, and it could just be the intensive support from the group doing the research as they supported the study participants in their switch to keto.

    Dear Lauren, I've found so much comfort and inspiration through your videos! Suffering from schizoaffective disorder can be a lonely walk, especially if you don't have mentors and inspirational stories on how to treat it. So thank you from the bottom of my heart. Please be super aware of how your body responds to the lower medicine doses! I have previously tried to cut down on meds which worked really well for a period of time and then had horrible episodes

    Congratulations Lauren! I have been following your videos for years and it have been not only extremely informative but also inspiring to watch your journey as I have struggled with my own diagnosis of Bipolar Disorder. It has given me a lot of hope in managing my own symptoms. In the initial video where you decreased your medication from 60mg to 40mg you mentioned withdrawal side effects but not any reemergence of perceptual changes or paranoia. I was hoping you would share more about when those symptoms emerged during that decrease in the setting of withdrawal and how long they lasted before going away. thank you so much for creating these videos and sharing this part of your life!

    If someone is saying you’re being irresponsible that is own them not use. Most of us have common sense and understand what works for one person may not work for the next person. I for one am encouraged by your videos. It does make me wonder how much our foods effect ANY of our illnesses.

      Really most of the people watching should be people with schizophrenia, are you? Are you even related to anyone who has psychosis? Are you aware that relapses are one of the biggest issues with people who have schizophrenia because relapses can cause treatment resistance. Have you ever had someone you know end up in a hospital because they stopped taking their antipsychotic? Just wondering.

    Congratulations. You're doing very well. Great support team. I started my son's journey around the same time that you did and I took the course from Dr. Georgia Ede. He is on clozapine And we are down to half slow tappering schedule. Clinical keto metabolic therapies do work must be taken seriously. It's not simply just starting a keto diet. Clinicians and healthcare professionals need to take the blinders off. And take this seriously. So, they can offer this as another therapy

    So glad to hear you have found something that is working for you. Each time the doctor has tried to taper me down on the Seroquel I take for my voices and hallucinations I get horrible side effects.

    I was on antidepressants for years due to chronic severe major Depressive Disorder. Then I had a rare case of Serotonin Syndrome last year. I was no longer able to take antidepressants and was put on a few mood stabilizers. I got to feeling so good that I thought I could taper down and become med free. So I did. Within a month, I fell into the worst depression of my life. I wanted to be med free so badly that I waited to get help. Then it got so bad I realized I CAN'T be med free with this mental health disorder. So I was started back on an antidepressant I had taken for years in the past that didn't give me a case of Serotonin Syndrome and it's been slowly increased. I went through 4 months of severe depression and dropped out of life all because I wanted to be med free. Some people with a mental health disorder CANNOT be med free to function at a normal capacity. Diabetics need insulin, stomach issues need treatment, brain tumors need what they need, and I realized that my depression needed meds. I'd rather be on the meds than suffer that severe depression again. I hope that it all works out for you, but know that being on meds is not a bad thing. I know how you feel about wanting to be med free, but remember... you were feeling so good before weaning down BECAUSE of the meds. You may not feel so good after you quit the meds. If you need meds, it's ok. I wish you so much luck and love as you taper off the medication. Just know it's totally alright if you realize you need the meds. We need what we need to feel normal and functional and stabilized in life. If that need is meds, so be it. I'd rather feel good on the meds than be miserable because I was adamant about being med free. Being med free did not work for me.

      @@liamcurran5612 I was on Effexor for 18 years with no problems. Then I switched to Cymbalta. I was starting the Cymbalta as I was weaning off Effexor (under doctor supervision), but it caused an overdose in Serotonin, which threw me into a severe case of Serotonin Syndrome, which nearly killed me. Serotonin Syndrome is no joke. It caused me to seizure, stop breathing, and lose my memory for 3 days. Didn't know who I was nor did I understand the English language. It was terrifying. Doses of both drugs? I was coming off 300mg of Effexor and was on the starting dose of Cymbalta (don't recall the dose). But I had come down to 150mg of Effexor and was still on the starting dose of Cymbalta when I started to have the beginning symptoms of Serotonin Syndrome. I went downhill extremely fast. Called the ambulance just in time. The ER doc said if I'd waited 15 minutes longer to call 911, I wouldn't have survived.

      I hear what you’re saying. Some people need meds to function. Unfortunately for those on antipsychotics there are a lot of risks for permanent movement disorders, diabetes, and increased risk of cancer heart and stroke. I think for some class of meds the possible side effects are really scary and I can understand why people would want to find alternatives to avoid them if possible.

I hope she finds a holistic way that works for her long term. I really do. If she does, she needs to share it with the mental health community.

    I'm not even stable enough on Keto to go off or down right now but holy cow when I am in optimal ketosis I feel great!! I finally bought a ketone blood monitor and I go out of ketosis in the morning and I'm not sure why. I need to do more research and keep tracking my macros. I don't have a dedicated nutritionist but I do have a supportive care team and nutritionist on hand if I need it. I somehow got onto public and it's a good system for the selected few who can get in. Thanks for the updates it has helped me a lot.

      Look up "the dawn effect" This is a natural phenomenon where your blood sugars rise in the morning when you wake up. Jason Fung and Dr Boz both have worthwhile videos on this.

    Brazil Nuts have Magnesium but don’t eat a lot in one sitting. My daughter is off all antipsychotics for years. Symptoms are present but the good outweigh the bad in our experience. Eating well and staying active is vital.

    Truth be told, I was a little jealous about your recent remission. But, am a grown woman and I am so proud of you. My adult daughter was diagnosed with schizophrenia and has been in and out of hospitals for almost 5 years. Clozapine and injectable have failed.. She is not aware she has schizophrenia and not able to adhere to medical keto. I have not given up hope.

    Thank you for sharing your story and emphasizing the necessity of medical supervision. People don't realize that experimenting is a part of fine tuning the theraputic protocol for any patient. When your doctor gives you a protocol they are not 100% sure it's the one that's gonna work and eperimentation *under medical supervision* is something many patients go through. Lowering the dose of antipsychotics under medical supervision is not that controversial.
    Take care of yourself and be careful.

    I am super pleased about your progress and your journey with all of this. What you’re experiencing with the ketogenic diet amplifies a need for more research into how B12 and folate help our neurological processes so fundamentally that they can mitigate schizophrenia and epilepsy (which is what the ketogenic diet was developed for in the first place). I’m so excited for you! I know you’re taking magnesium, which is fantastic. I think further supporting this journey with the adenosylcobalamin form of B12 and Folinic acid or L-methyl folate would create an even stronger foundation for you to help guard against any regression of psychotic symptoms during medication decreases and ever after as continued support. Regardless, I hope things continue to improve! I’m super excited for you and so grateful that you have been willing to share your journey with all of us.

    Havent finished the video yet but 40mg to 20mg sounds like a lot! Its 50% of previous dose. Have you heard/ read about lived experience recommendations for tapering at 5% of previous dose?

    Good job. I take an Abilfy injection 💉 every four weeks for schizophrenia. I use to take five 💊 pills a day and now i only do the Abilfy. For me once I accepted my diagnosis and went to therapy sessions and even a couple of conferences to learn about mental illnesses I started getting better. Also knowing where to go and what to do for less triggers and stress but most importantly don't listen to anyone except the mental health Professionals 🙂
