This is common in Latin America. The insurance is only for surgical procedures.
if you go to an ER, that's under catastrophic insurance which they have, just a lowest cost one
The US government can definitely reform Medicare to become a government health insurance program that would cover only catastrophic care and maternity care for every US citizen ( in which this new reformed Medicare would still be paid via taxes of course ). Then the majority of basic health care needs can be met by the direct primary care model through primary care health clinics competing with each other.
