Maybe habit or historical appreciation? Placing the first aid kit at the middle back of the belt was very common with Marines in the late 50s to the 70s or 80s.
Because the IFAK is intended for someone else to use on YOU. That's why placement of it is S.O.P. Everybody knows where it is to save looking for it. Back of the belt is out of the way too.
I use the grenade pouches for my rifle cleaning kit, mini binoculars, a compass and a small radio.
