Maybe in the earlier seasons, but he does change. He always has empathy, but for some reason chooses to hide it in a shell of sarcasm and weird humour. Chase's stabbing, Wilson's diagnosis, Cuddy's relationship, etc. I'm sure there are more examples

Almost all therapists Ive worked with had me perform something like this, some physical release of pent up emotions. Usually it was hitting a pillow, I’m not sure actually breaking anything (like in this scene) adds anything. However, I think the key is to channel your thoughts before you begin, so you’re targeting the specific pain and not just being destructive for destructions sake. I hope that helps.
