  • Interstitial granulomatous dermatitis is a rare condition which develops in patients with underlying systemic disease. Autoimmune, auto-inflammatory, lymphomatous, drug-related and many other conditions have been implicated. Patients characteristically present with a dense cord-like lesion. The histology is very variable. Most often it looks very like interstitial granuloma annulare. In some cases there may be conspicuous neutrophils with karyorrhectic debris. Eosinophils can be conspicuous even with fame figures- one cause of Churg Strauss granuloma. Elastopahgocytosis and engulfment of collagen fibers can been seen. Histiocytes wrapping around collagen is said to be a characteristic appearance. The diagnosis is really one of exclusion. Often special stains for micro-organisms are necessary before coming to this diagnosis.

    This video was very interesting. However you say that IGD should be know to a dermatologist. Not so far here in the Cleveland, OH area. I first noticed the rash June of 2019. March 2020 it was finally biopsied and diagnosed as IGD. 1 PCP, 3 dermatologists, 2 RA's, 1 endocrinologist and 1 allergist had never heard of it. My current dermatologist has prescribed Plaquenil, 50mg, which I have been taking twice daily since 3/10/21. So far no results. It's spreading over my body and is very depressing to me. I wish I could find a doctor that could help me.

      @@phillipmckee9913 Thank you for responding to my comment. Currently I am seeing John Anthony, MD, Dermatology at the Cle Clinic. He and Allen Laurence Kennedy, MD, Endocrinology and Elizabeth Kirchner,CNP, Rheumatic and Immunologic Diseases (all associated with the Cle Clinic) were not familiar with IGD. I keep searching for a doctor to help me. The rash keeps spreading over my body and makes me very depressed.
