I've actually had two tow bar failures while Towing. This one, I believe, was due to an R-key failure - allowing the main hitch pin to come out. So, that means the end going into the hitch receiver came out and it must have shattered when it made contact with the ground. We were on the highway, so it would have been around 65mph, with a 6500lb truck behind it
