Case 108: Manual of CTO Interventions - Dual lumen microcatheter for bifurcation at the distal cap

- A chronic total occlusion of the left anterior descending artery had a bifurcation (LAD/diagonal) at the distal cap. Antegrade wire escalation resulted in crossing into the diagonal branch. The main branch (LAD) was wired with a second guidewire advanced through the over-the-wire lumen of a dual lumen microcatheter (TwinPass Torque). The bifurcation was successfully stented using the DK crush technique with restoration of antegrade flow in both branches.

how do you do trapping balloon technique when dual lumen catheter is difficult to remove?
