Well, the whole Romeo and Juliet concept works because the song is about being in love with drugs or alcohol, and it slowly killing you. Romeo and Juliet died from being in love with each other. Even though the drugs are killing the Patient, he still does them knowingly, which fits "lather the blood on your hands".

The music is just so raw and lustful and sexual (without actually being sexual) if that makes any sense. It kind of tickles that animalistic urge in us and the chaos ensues.
