A Functional Medicine Approach To Osteoporosis

  • We have been led to believe that drinking milk is the key to bone health. Unfortunately, the prevailing myth that drinking milk as children builds up a calcium bank that helps us avoid osteoporosis and fracture later in life is simply not true. Our bone health is determined by a number of things including what we eat, our stress and activity levels, our muscle mass, and other factors that contribute to the delicate balance between bone build-up and bone breakdown. Unfortunately, osteoporosis is a very common and life-threatening problem, it is also highly underdiagnosed. This is part of the reason why prevention is so important when it comes to osteoporosis.
In this episode, Dr. Hyman sits down with Dr. Todd LePine to discuss the Functional Medicine approach to osteoporosis. They get into how chronic inflammation acts as one of the main drivers of osteoporosis, why both bone density and quality are so important, and much more.
Dr. LePine graduated from Dartmouth Medical School and is Board Certified in Internal Medicine, specializing in Integrative Functional Medicine. He is an Institute for Functional Medicine Certified Practitioner. Prior to joining The UltraWellness Center, he worked as a physician at Canyon Ranch in Lenox, MA, for 10 years. Dr. LePine’s focus at The UltraWellness Center is to help his patients achieve optimal health and vitality by restoring the natural balance to both the mind and the body. His areas of interest include optimal aging, bio-detoxification, functional gastrointestinal health, systemic inflammation, autoimmune disorders and the neurobiology of mood and cognitive disorders. Dr. LePine teaches around the world, and has given lectures to doctors and patients at American College for Advancement in Medicine (ACAM), Age Management Medicine Group (AMMG), the University of Miami Integrative Medicine Conference, The Kripalu Center in Lenox, MA, and is on the faculty for American Academy of Anti-Aging Medicine (A4M). Dr. LePine is the head of the Scientific Advisory Board for Designs for Health and a consultant for Diagnostic Solutions Laboratory. He enjoys skiing, kayaking, hiking, camping and golfing in the beautiful Berkshires, and is a fitness enthusiast.

I'm convinced that all the calcium they have been telling people to take has contributed to dementia and heart issues. That, and some of the other medications. My mom did everything right. She took long walks with friends every day, cooked, cleaned, played cards, sewing club, and ate pretty much a Mediterranean diet as we are Italian. She also experienced chronic lower back pain that was debilitating, then dementia. No family history of any of this. I have recently read that some blood pressure drugs have the potential for all these side effects. With all the doctors she saw, NOT ONE, thought of switching out her meds. But some, although rare, do. I learned this too late. Doctors still do not believe that K2 redirects calcium, and they still prescribe a ton of calcium without it.
Going through this journey with my mom, then more recently during COVID with both parents, I can truly say that I have lost complete faith in our medical system, and that includes the doctors. I have found them to be puppets. Relying only on what they were taught in school, and lacking any kind of critical thinking. Anyone with good recall can be a doctor, but how great it would be to be able to see if your doctor possesses true passion for healing? That should still be number one. And that is what has been lost.

💯me2! Doctors are the most uncaring people ever! Sure, there’s exceptions but the vast majority actually do not care if u r dieing (everyone has to die) even as you stand right in front of them, as my precious father did. My docs response to my new osteoporosis Dx was - paraphrasing - no drugs? Then no tests. What does he care if I suffer fractures in my back in my old age? He doesn’t! Resent that I have to pay Medicare premiums!

I worked in dementia care for over 30 years, it is a vascular disease they now consider it to be diabetes #3
gluten and wheat carbs are top of the inflammatory foods list and it should be considered a sugar as it creates the same spike in glucose meters if not worse than sugar. On the vascular cellular level every time we consume a high gluten carb or sugar there is damage being done at a micro level.
Dementia actually starts 10 years before symptoms. Parkinson’s dementia Multiple sclerosis are all the effects of diet. Watch MS for hope. Buddy went to the Canadian MS society wanting to do a talk about the proof and finding and they refused to give him a minute. As when we cure disease we eliminate the need for a society that makes money that depends on disease .
The then 24 year old and his MD father did the documentary and he went into remission forever after. His diet is strictly lean meats and leaf veggies that’s it. As far as these disease go because it takes years to be symptomatic it makes sense that children don’t get these diseases unless they are obese because it’s years of an accumulation of micro cellular damage. I believe dementia can be stopped and even reversed with keto or carnivorous diets. As a care home worker 30 years.. I’ve seen the food quality and diet in Canada excel all disease I don’t doubt USA the same. No money is cure only profits in disease but more importantly attacking the cause is key.
I think it’s time the FDA start grading food! A+ top healthy to z- completely toxic. But what would that mean for fast food industry? Cereal manufacturers? My grocery had a whole full long top to bottom isle dedicated to just cereal that’s all man made fortified toxin. Highly GMO cheap to produce over priced Corn or wheat. The next isle is all top to bottom end to end potato chips and candy. Most of our snacks in Canada in the class of healthier granola bars and fruit fibre snacks are actually glorified candy.

I will be 75 next month. At 50 I had my first DEXA and had osteopenia & some osteoporosis; the doctors instantly gave me a prescription for Foxamax. They scared me & said at 60 I would be hunched over & have weak bones. I never took the drugs & read every book on the subject. I have ALWAYS eaten a very healthy diet, never smoked & always exercised. I walk, bicycle, ski, do yoga & lift weights for years. I weigh only 99 lbs. I have had a few DEXA test throughout the years & I feel my bones are strong yet the density doesn't show that. I put off having DEXA test because doctors keep pushing the Drugs!! I am curious though so may have one to see if my numbers are better. I feel terrific & do take a plant based calcium & collagen. So many people are misinformed. It's wonderful that these doctors are spreading the word out. Thank you so much! Stay well everyone:)

My husband drinks gallons of milk every week and he has the worst osteoporosis. He was shocked by his diagnosis when he developed compression fractures in his spine. It has been devastating to his quality of life (and mine). Milk does not do a body good unless you are a calf.

    • @GiGi-fd6om
      @GiGi-fd6om Před 3 lety +36

That is because milk products have protein as well. Since protein is acidic, the body uses calcium to neutralize it. (Tums or an antacid are used when people feel like they have too much stomach acid and Tums is calcium). Excessive animal protein also leaches minerals out of the body as does alcohol, and acid forming foods. You're correct that milk is good for babies like a calf. Adults should not be consuming another species' breast milk. It's insane. It's just industry lying to us since our grandparents were young that we need it. Food that grows from the ground is made for us and has all the minerals we need.

All milk is not the same. For years now, milk has come from cows that are crowded into factory farms, never go outside, get shot full of hormones and antibiotics. They don't get to eat grass, their natural diet. If cows are not allowed to eat their greens, of course people who drink their milk won't get the benefit of those greens either. Drink organic 100% grass fed whole milk and see how that goes. Or better yet, try making that organic grass fed whole milk into kefir. (Also, some people's ancestors did not evolve in an environment with cattle so never adapted to be able to digest milk.)

I have below the chart osteoporosis. 12 years ago I had a liver transplant because of cirrhosis caused by hep C. I had a dexiscan when I was put on the transplant list and was told I had osteoporosis. I was on so many meds, something like fosamax as was out of the question. I had 7 teeth pulled they were afraid could get infected when I had to take immune suppressants. I had my first stress fracture in the hospital for an infection a few weeks after the transplant. A back brace over healing staples wasn't fun.Since then I have broken both wrists, at different times, my elbow is wrapped in titanium with 13 screws. I had a shoulder replacement and 18 months ago I broke one leg right under the hip from a light fall. I haven't been able to walk unaided since.I recently graduated from a wheel chair to a rollator/transport chair which I love. It forces me to walk, painfully.
So what have I done about it? I have studied food for 40 years. I am 69. I was body building at the gym when I started learning. When I started getting symptoms from the hep C in 1986 I didnt know why I was sick. In 1998 I got a diagnosis. I started researching everything I needed to know about food and supplements for better digestive, too. Today I eat organic lettuce and veggies for breakfast. From my last dexiscan to one I just had, my bone health was 16% better. I liked what I heard in this video. I learned new things. Life has been a struggle since the transplant but I'm still here!

Apart from the SAD that ups inflammation, other living factors are the shoes we wear that disconnects us from grounding with the earth . What percent of our population, live in high rise buildings, work in high rise buildings? These heights take us farther from the grounding earth. Our modes of transportation too. I have had a stressful life, didn’t realize it until lately that it the undoing of me. That’s where the inflammation comes from. Poor sleep for me.

    Gut Microbiome Impact on Bone
Gut microbes produce metabolites that influence bone remodeling, including short-chain fatty acids (SCFAs).
Commensal bacteria create SCFAs like butyrate, propionate, and acetate through anaerobic fermentation.
These compounds regulate both osteoblasts (bone-forming cells) and osteoclasts (bone-resorbing cells). Each SCFA has specific signaling functions in the bone. Butyrate stands out for promoting osteoblast differentiation, mineralization, and bone formation.
It does this partly by enhancing osteogenic growth factors and stimulating regulatory T cells to suppress excess osteoclast activity.
Butyrate also enables parathyroid hormone to stimulate bone-building, again by boosting regulatory T cells. Overall, SCFAs help balance bone resorption and formation, central to healthy remodeling, by modulating immune-skeletal system connections. Supplementing with individual SCFA nutrients has limitations. However, diets that nourish SCFA-generating gut bacteria through resistant starches and fibers represent a promising nutritional approach for preventing osteopenia. Further research on microbiome-skeletal links may reveal additional microbial metabolites like SCFAs as targets for nutritional therapies to control bone health.
The gut-bone connection extends to influential hormonal signaling from intestinal regions. Researchers theorize gut-derived peptides help coordinate bone cell homeostasis beyond basic nutrient effects. Small clinical trials have begun investigating this concept in humans. One 2021 crossover study (n=14) examined associations between two bone remodeling blood markers and satiety gut peptides GIP, GLP-1 and PYY after meals. They found one bone biomarker positively correlated with GIP and GLP-1, yet negatively with PYY. While larger trials are needed, these preliminary data support the hypothesis that gastrointestinal hormones communicate with skeletal systems, potentially directly modulating osteoblast and osteoclast activity. Uncovering the specific mechanisms in humans that show enteroendocrine secretions controlling bone resorption and formation rates could spotlight digestive health as a contributor to osteoporosis. If further research confirms particular gut hormones do reliably indicate, or even drive, imbalanced remodeling toward net bone loss, targeting these signals could open new paths for nutritional therapies promoting skeletal integrity.

Most folks who include cheese, yogurt, and milk in their diet get plenty of calcium,
too much probably. Low calcium intake is not the problem. Absorption is. The absorption
problem is because most folks are deficient in vitamin D3, vitamin K2 and magnesium.
The Standard American Diet, as recommended by the FDA food pyramid may be part of
the problem. Too much calcium and carbohydrate, not enough protein, good fats,
minerals and vitamins. About 80% of Americans are D3 and magnesium deficient.
One answer is to get yourself a vitamin D3+K2 (not vitamin K1, that one is for clotting) combination capsule of about 5000 IU
D3 with 100 mcg K2 (as MK-7). To this add magnesium citrate (mag oxide is useless),
    about 200-400 mg per day.
The D3 & magnesium facilitate calcium absorption, the K2 directs the calcium to the bones,
otherwise too much calcium will circulate in the blood. The K2 in the combination will prevent hypercalcemia.
A book by Kate Rheaume-Bleue, “Vitamin K2 and the Calcium Paradox” describes this
    in some detail. Public knowledge.

That’s the problem with the medical community these days. Bring a retired RN I believe in being a partner with my doctor so we can work together. I give them all my significant labs and reports from MRI’s, CTs etc. I have had osteopenia in my left my femoral head in my hip for over 20 years and have never been questioned or given any advice or diet or vitamin d levels. I’m the one who started taking Vit D3 and K2 and have had to ask drs to check every year when I get my thyroid tested each year. I didn’t ask this year because I figured this new Dr would test ( same practice) but they didn’t. I’m now osteoporosis 2.5 in that left femoral head and osteopenia in total hip and spine!!!! Could I have been helped if my drs knew my history and cared about my health? Probably. I live outside of Tucson, AZ and none of the Functional medicine doctors here take any type of insurance especially Medicare! Not only no insurance but most are now Conciege doctors/medicine. No one cares to be a doctor any more. I’ve never been touched by my doctors since on Medicare except twice once when I had them feel what I wondered was a lump on my thyroid and the other time my doctor actually listened to my heart because I had complained about not ever having my heart listened to. Also I just found out I had regurgitation of aortic, bicuspid and tricuspid valves and that’s becsuse I had a pain my my chest one night and I told my last Dr and I went to cardiologist.

Excess protein? Bones are around 40% by weight. The collagen matrix is made from consumed protein. Consumed protein bioavailability decreases with age so over 60 year olds need to consume MORE protein not less. Protein restriction is a one way trip loss of lean muscle mass which includes bone mass.
2 yearRct data shows highest protein intake (with calcium citrate malate and vitamin d supplement) showed increase in bone density on dexa while lowest protein intake did not or even decreased bone density in both men and women over 65.
Bones are mineralized protein matrices. Leafy greens with their calcium bound up by oxalates do not bones make.
And the zinc, copper, manganese( not to mention bioactive vitamin A, vitamin d, vitamin k2) he mentions....are all found in beef liver which he did not mention....sigh.

In my opinion that’s good advice you’re giving ( on upping magnesium intake). Most people look at me aghast when I tell them I stopped taking calcium supplements ( on a doctor’s advice ) years ago .
I wonder whether my experience might be of help to other osteoporosis sufferers . I have had very severe osteoporosis for many years now & have had fractures in both ankles and wrists . Much worse & extremely painful have been the vertebral fractures - 14 of them up to now. I haven’t really been on osteoporosis drugs as I’ve always been afraid of the ‘rebound’ effect & interestingly the only ‘spontaneous’ vertebral fracture I’ve had occurred after ending a 2 year course of Forsteo.
I then went to a doctor who advised taking a higher dose of Vit K2 (400 mcg daily )along with 4000 a day of Vit D . To that he added Silica, fish oils & curcuma.
To that I added 100 mg Nattokinase daily( fermented soybean which is rich in Vit K2) . I’ve seen great improvement , my spine seems a lot stronger ( & so far no more fractures! 🤞🙏)
I’ve since found very interesting articles on how Natto & K2 can benefit your bones.

I heard Dr Berg talking about the importance of Vitamin K2 and Vitamin D together. It is worth you checking this out. He said Calcium Carbonate supplements aren't so good.

People on long standing Warfarin are prone to osteoporosis as well. In fact I’ve just had 7 vertebrae fused in groups after being on it from 1988.

Excellent! Thanks so much. I plan to begin weight bearing exercise as my key. My D levels are already up. Using a supplement that gives my collagen from grass fed beef sources. I have a-Fib and am on a blood thinner - doc said I can eat leafy greens (but lectins can be a problem) - but not take supplemental K, which I had been doing. Hmmm.... need more info on the K issue. Dr. Mobeen Syed in a lecture on K, explained that, yes K is involved with clotting by way of some liver mechanism. However, K also has another effect in the blood itself, which prevents excessive clotting and if K is under that amount you should have that preventative mechanism cannot work. I have to find out more about the mechanism of K. Oh, my! So much to learn...However, I love learning! 🙂

K2 may not have that clotting effect that k1 has. Maybe research that👍🏻

I took Actonel a long time ago and not for very long but when I had a root canal tooth pulled a few months ago, strange bone grew in the area and I had to have it surgically removed. I also have TMJ that's worsening and I'm thinking it's due to that terrible drug. More studies needed, so dentists know about this. I have been studying and working on healing my body naturally now that I have been awakened.

See a biological or holistic dentist immediately and give a full health history. Root canals and oral health issues are usually overlooked holistically. Do an internet search.

THANK YOU VERY MUCH for this enlightening podcast. The sad truth is that many doctors do not keep up with new information about osteoporosis and keep on prescribing high doses of calcium wirh vitamin D3. I found out that you also need Vitamin K and magnesium through my own research and not from the doctors. This makes me angry. Thank God for the internet.

It’s all well and good to be baseline tested before age 40, and I’m 63, having followed functional medicine for years, it saved my life, but the measurement unit has changed, and when my functional medicine doctor read my 23 year old DeXa scan, we really had no idea what the number s meant. Scientists, when new discoveries and ways of doing things (practicing medicine for example) need to leave roadmaps between an older way of measuring something to a new way of measuring something. It’s immaterial now, I’m here and dealing with it, but with no idea if I could have reversed anything back then.

Concern about lead is why I don't eat bone broth. Lead persists in the soil for years and gets taken up into the plants that grow in that soil. Even if one farms organically, if there's lead in the soil those organically-grown plants will pick it up, as will the animals that graze on those plants. There is no safe level for lead in one's food but the U.S. gov't says that less than one part per billion is ok. I contacted Kettle & Fire about possible lead in their bone broth and they replied that toxic metals in their broth are less than one part per million which is many times higher the government considers acceptable. So I don't eat bone broth even though I'd like to.
