Frozen Shoulder - Causes, Symptoms & Treatment
The shoulder joint has the maximum range of movement in the body. When functioning properly, it allows us to reach out to objects effortlessly. However, if impaired, such as when wearing a tight shirt, lifting the shoulder becomes difficult. This condition, known as frozen shoulder, occurs when the joint capsule's soft tissue becomes tight.
Who is Prone to Frozen Shoulder?
Age: Commonly affects people aged 40-60 years and above. Aging reduces the elasticity of collagen fibers in the joint capsule, increasing the risk.
Gender: More often seen in women.
Medical Conditions: Higher prevalence in diabetics (especially with uncontrolled or long-standing diabetes), those with thyroid issues (both hyperthyroid and hypothyroid states), stroke or Parkinson’s disease patients, and individuals with heart ailments on long-term medications or post-angioplasty/coronary bypass surgery.
Other Factors: Bedridden individuals, those with repetitive stress injuries, tendinitis, fractures, or sprains/strains in the shoulder are more likely to develop frozen shoulder.
Symptoms of Frozen Shoulder
Global Restriction of Movement: Mobility in all directions is affected.
Internal Rotation Restriction: Difficulty taking the hand towards the back.
Night Pain: Pain is more pronounced at night.
Stiffness: Shoulder stiffness and difficulty in activities requiring overhead abduction.
Duration: Frozen shoulder persists for 8-9 months, sometimes up to a year, with severity increasing in the first 3 months, plateauing in the next 3 months, and gradually reducing in the last 3 months.
Treatment Options
Early detection and proper treatment are crucial for reducing severity and achieving recovery. Patience and regularity are key, as this condition does not subside quickly.
Hot Water Fomentation and Massage: Use oil or ointment to massage the affected shoulder twice daily to relax stiff tissues.
Medications: Anti-inflammatory drugs and medications to reduce nerve-related pain.
Exercises: Essential for recovery. Detailed later.
Physiotherapy: Modalities like heat therapy and ultrasonic massage to reduce tightness.
Intra-articular Injection: Rarely administered to reduce inflammation and stiffness.
Manipulation Under Anesthesia: Performed in severe cases to passively move the shoulder and reduce stiffness.
Essential Exercises
Stretching Exercises:
Cross Body Stretch: Stretch the stiff shoulder by pressing with the opposite hand.
Axillary Stretch: Stretch the armpit by holding a railing and lowering the body.
Wall Climb Exercise: Climb a wall with fingers to stretch the shoulder in all directions.
Strengthening Exercises:
Shoulder Squeeze: Pull shoulders back, hold, and relax.
Resistance Band Exercise: Use therabands or a cycle tire tube to strengthen shoulder muscles.
Rotator Cuff Strengthening: Use therabands to perform internal and external rotations with a rolled towel in the armpit.
Consistency and gradual intensity increase in exercises are vital. Perform exercises daily, in the morning and evening, to enhance movement, reduce stiffness, and build muscle strength. Initial pain might occur, but persistence and patience will lead to recovery.
