You missed a few in Paper Mario TTYD and Super Paper Mario:
- In Paper Mario TTYD in the Dark Bones room Chapter 8, if you stand still instead of whacking away the Dry Bones hoarde, you'll be sucked into the floor.
- In Super Paper Mario, when you refuse to search for Luvbi for Queen Jaydes in Chapter 7-1.
- Also Super Paper Mario 8-3, when you agree to join Dimentio and get brainwashed into doing his bidding.
Also sorta maybe dunno counts, but the time where Paper Mario got blown up and sent to effectively hell.
Sinteleon - That doesn’t count because he was “sorta” alive while he was in the Underworld/Hell. The Queen of the Underworld told Mario that she can feel that he still has some life and is able to go back to the normal world/Flip Side.
Don't forget about Hooktail Castle, where you die if you don't get out of the room in time.
And what about the Sniffit or Whiffit game?
You also forgot at the very beginning if you refuse to save the worlds in SPM

Like the Walking Dead esque Paper Macho Chain Chomp that if you made the wrong decision and went the wrong path, you'd get a game over from being eaten alive, like a zombie.
I have died TWICE. Both times in the overworld, and from making dumb mistakes. I have "died" in two different battles (one was a boss, in which case, I forgot to heal), but I had a 1-Up Mushroom on me. ALWAYS BUY THE 1-UP MUSHROOM! IT IS LITERALLY A LIFE-SAVER!
You missed the snifit with the magic lamp in tok and the sudden death round in shy guys finish last.
Also the chain chomp KO.

Missed a few unique Game Overs in Origami King:
Agreeing to surrender your freedom after failing the first round of guess the sniffit's mood.
Choosing to follow Bowser Jr. or Olivia at the 3-way intersection in the jungle gets you nommed by the Macho chomp.
Answering the question wrong in the "Shy Guys Finish Last" Sudden Death round.
And I can't verify them myself, but I'd assume losing all hearts in the river boat and airship trips lead to game overs as well.

after what you get when dying in colour splash, the "origami-king" game over screen leaves something to be desired... perhaps after mario dies (if his corpse is left intact) some folded shy-guys come along and start to fold mario up into an origami form, before the screen folds itself into an origami square & falls off-screen... after that, the grey GAME OVER text, made out of origami, fades in above a background of greyscale ribbons...
They still need something to keep Mario from unfolding. But I agree with the faded part

I added a +1 on my turn and used my Legendary Iron Boots twice to jump on the scissors. Had a 1-up, but it didn’t matter. Iron boots allows you to jump on sharp and pointy objects so I thought... never mind I was an idiot I guess. Mario is paper and that is scissors
