He specifically looks at the most common fractures: Distal Radius (Colles) and Scaphoid (Navicular) fractures. He also looks at more rare fractures of the Hamate and Capitate.     Displaced bones of the wrist almost always require surgical intervention, usually in the form of pins if the displacement is small enough to be put back manually, but often requiring plates and screws to hold the bone in place if the displacement is very great. These plates and screws are generally left in the hand, and do not impede function, but in the case that adhesions develop on the hardware, or if they somehow cause irritation or damage to the nearby tendons, they may be removed at a later date. Surgeries of this type, involving a plate, usually take about 45 minutes, and are done under general anesthetic. After this, a smaller surgery may be required to remove the pins, which is done at a later date.
    Scaphoid fractures can be complicated, because the bone itself is not very well connected to the blood vessels of the hand, and so is delicate and susceptible to injury, and bone repair is difficult. Often, if there is enough vasculation in the bone, the scaphoid will heal itself without the need for surgery, but if it does not, then the bone can deteriorate, and must be either repaired removed. IF it is not removed, then a tension screw is used to hold the bone in place as it heals, and this can take five or six weeks.
    In a hamate hook fracture, the best course of action is to remove the affected hook entirely, which allows the wrist to then move about freely without irritation.
    Post surgery, splinting is the most important part of wrist recovery, and is necessary in all procedures to keep the wrist from moving and upsetting the delicate balance of bone placement for healing purposes.

    Thanks for this video. I'm a healthy 63 year old woman who slipped on a wet bathroom floor. Had wrist surgery with metal plate and screws one week ago. Recovery is going well. Luckily I live 2 blocks from a premier hospital with award winning Orthopedic department. Thank you Vancouver General Hospital

      I’m a 32 years old dude who just had same wrist surgery with metal plate and screws 3 weeks ago. Trust your recovery process going great.
      If you don’t mind, from your experience how many weeks post surgery does one begins physiotherapy session to regain motion/movement in the wrist region?
      Also, for how long should one carry the metal plate and screws?

      @@nadi8985 Everything is great 3 yr later. Metal plate and screws will stay in place. No need to remove. Hand is sometimes a little stiff in the mornings but that's all

    Is a Smith’s fracture (didn’t see it here) the reverse of a Collier’s, or is there also a “reverse Smith’s fracture?”
    An extra surgeons joined in just to see mine (years ago) and I just had an external fixator, 3 external pins at the radius and a summer I can barely remember. Highly sensitive nerve spots. I can still play guitar nearly as well, but I can’t hold up a coffee mug.

    This patient absolutely did not need surgery. All that metal adds extra stress to the bone after the healing phase and could cause a break in the foreseeable future. The medical industry really likes to push surgery unnecessarily.

      Not true. Metal plate and screws speeds up the recovery process and has more predictable outcomes. Closed reduction bones can easily shift

      Actually you’re wrong. I had wrist surgery after a bad fracture. I had absolutely no problems. My wrist was good as new within 90 days. Be sure to do physical therapy while still in the cast. I had minimal pain except for two days after surgery.

    Broke my radial bone in several pieces. A couple weeks in a cast seeing if the bones would align and then opted for surgery. I have a plate and 5 screws. Healing is coming along ok. Cast will be on till early June

    I’m 14 and had to get surgery with 2 pins to hold my wrist in place cause it wouldn’t stay and when I broke the radius the bone moved to the left and was hitting my growth plate so had to get surgery the very next day still have no function in my wrist but I have a 100$ string cast thing that I can take off I also didn’t get a plate my surgeon said if she put a plate in I’d lose all movement at age 24 so she put pins I broke my wrist dirt bike I hit a quad and jumped right before I hit so I probably landed on my arm I don’t know I was completely blacked out from this point on

    Hello Dr. John. I had a surgery on my wrist while l was overseas about 10 years ago where my orthopedic surgeon had to do bone graft to replace some of the bone lost on my radius, and l do have a plate with screws.
    My question; do l need to remove the plate, and the screws considering that l had my surgery almost 10 years ago? Thanks.

    Falling on street and breaking my left wrist I had urgent distal radius surgery with metal plate
    and 9 screws in August 2019. Now my wrist is still painful, swollen, my fingers numbed, it is irritating,
    very uncomfortable. I need urgent surgery to remove the metal plate + the screws and I can’t find
    understanding for that. Actually, this surgery is a deliberate attempt for torture?
    Please, intervene for me to have the removal surgery performed urgently in good conditions (Sydney July 2020).
    I have the removal surgery in November 2020 at RPA Hospital Camperdown, ending 16 months of torture.

    dr. knight i broke both my wrists/hands in a motorcycle wreck 3 yrs ago, they were never reset...no casr nothing...they are pretty deformed from this and hurt steady, sometimes more than others...i also have nerve and/or ligament/tendon damage...i waited approx. 30 hrs. before going to hospital after wreck and bc of this they did nothing...i never went back....can you help severe messes like mine?

    thank you so much for this video! I broke my radius playing tennis on Oct. 23. it apparently realined but it hurts so much, today very bad! I get my cast off Monday but it still feels like its burning and at times numb. I am 63 and have bad depression over all this. I broke my wrist, sprained my ankle and got a huge bump on my head and honestly don't feel like I will ever be the same!

    Wow this is exactly my break, I was snowboarding and had a distal radius fracture with deformity and had it reduced twice, after 3 and a half weeks I ended up needing surgery with a plate and screws, that was today and it’s definitely not a good time

      @@justincampos9206 it’s really not that bad, anxiety can be high before you go in but once you get sedated you’ll just wake up like you had a nap and it’ll be done, recovery process can be tedious and first week or so is painful but you should get meds for that. The big thing is with being in a cast so long you’ll have to work hard to get full mobility from the tendons surrounding the joint. All comes with time though

      @@harryobi5105 after you have the stitches removed and are done with the wrist brace the doctor should let you know that you’re ok to start moving it and trying to do new things, start with small stretches and activities and work your way up. You probably won’t need legitimate physio therapy from a facility unless you have issues with it in the future, I just kept active, worked my ligaments and tendons back to almost full range of motion over time and it just gets better on its own. Hope this helps!
      Also if you have issues with low range of motion from locked tendons I suggest filling a bathtub with as hot of water as you can handle and putting your wrist in there to try and stretch it a bit. That can also help loosen stuff up

      They can put in place and you will have plaster cast but there is slightly higher risk of arthritis

    When he shows how you break it that's what happend to me I have the same symptoms and when he shows the diagram of the actual break I have the same bump where my radius is but it's not as severe my family think I'm over reacting and won't take me to the doctor but its just getting worse

    Hi John,I had a severly comminuted distal radius fracture [7 pieces] and the doctors set it and left it in a cast for six weeks,with out any surgery.It,s been 18 months now and I still only have about 50% range of motion.I believe it should have been repaired surgically but the doctors lacked the expertise to tackle such a bad fracture. If you don,t mind me asking how do you normally treat severe fractures and do you generally get a decent result? Thanks for your time and very good video.

    Hi. I broke my radius and ulnar in my left wrist Christmas 2016. Had it reset 3 times with no operation. Plaster on for 7 weeks with my hand in a bent forward position. Since having the plaster removed I have had physio followed by intense physio. I still only have jest 10/20 percent of movement in my wrist. I went into hospital yesterday for wrist manipulation which is not normally known. I still have the same movement 10/20 percent along with a lot of pain right now.. I have now been told there is nothing else they can do as my x rays and MRI scans are not showing why there is not movement. I have also had a meeting with top 4 surgeons in Gwent and have been told its a mystery.. Any ideas?? I am left handed and this is a nightmare come true 😭😭😭😭

    For 11 years I fell and broke Radius and Ulna. I took time before it was okey since they didn't pull Ulna in place before cast. They had to do the cast several time because I was blue in the thumb etc. Ulna become 30 degrees from the original position. Now 11 years later I broke triquetrum, they didn't do a cast. After 5 weeks I can use a keyboard but not hold heavy things. I hope I will not get any more pain then before the last accident. I can almost do any movement, but with pain.

      I very much doubt you will notice I’ve just had mine fitted today on my wrist but had a broken jaw 13 years metal plate don’t notice it they generally don’t remove unless there’s infection or reaction to it.

    I had a bad break when I was 14. I'm now 36 and I refused a closed reduction due to fear. Now 22 years later I'm getting bad arthritis. Scared to have it checked out as after 22 years I'd require plates & screws. I wish I had a surgeon I could get to know. In the UK it'd be just who was assigned that day.

      Aches now whenever it's cold.

I had broken my wrist in a few choice spots so it was a relatively difficult procedure, I was told. They put a bar in my arm from the middle of my forearm to right up under my index finger. That has to be removed soon. Not looking forward to another surgery but it wasn't so bad. the first time.
      I'm in NJ and it's kinda the same deal here with doctors, when in the emergency room I was assigned a surgeon to go see the next day. It's the luck of the draw, I guess, who they send you to or who they refer initially if that surgeon is affiliated with hat hospital, etc. I got lucky , the surgeon I met was a top surgeon in the field and had some renown as a surgeon and a Lt Col in the Air Force, very nice, reassuring and smart guy. I can go see someone else, though, If I want a second opinion, whereas I think you guys can't do that?

      @@boristhespider4879 I could go to the doctors and say it's disrupting my daily life. They'd send me to hospital then and probably do it, but it's only every now and then that it hurts and it's not like I do pushups lol. I'm very scared about the idea of general anaesthesia too.

      @@nuggythebear Dude, I'm totally not braver than you or anyone , lol. I had no choice. It was either surgery or pretty much be one-handed forever, and I kinda suck but I love playing guitar. I was scared shitless of surgery and I'm no wilting flower. Turns out everything everyone said to me was true, you close your eyes for a second, breathe deep and next thing you know you're waking up and it's all over. Piece of cake. Good luck with yours, yo. I know it sucks.

    I remember breaking my wrist once in a bout of impulsiveness by trying to do a leap of faith out my bedroom window when I was about 14, it was not fun, never done anything like that since

    walking into a storage shed door hitting my head i went down abs turned to fall backward and ended up backwards in the ditch drive drainage broken wrist. she said it may not need surgery. we were in trouble with medicaid so we didnt go to the er and dr said whyd we go there to them the last time too and we didnt go to the dr but waited nearly 2 weeks to see the orthopedic dr. medicaid denied claims n threatened to cut off our insurance so we are scared to go to the er. it probably just made it worse by not going but cheaper or maybe not for medicaid

      Damn that sucks. Having a surgery at such a young age aint good. It must have been really serious since they usually just pop the bone back in and put your wrist in a cast for kids. Anyways hope you are doing better and have had a full recovery. Be sure to be careful from now! 😄
