  • Clomid and Fermara are commonly prescribed to women with PCOS and other diagnoses, as a first treatment to try and get them pregnant after an infertility diagnosis. This video covers the pros and cons to both as a fertility treatment for those with PCOS. I also cover the risks of each medication and the twin rate with each. Learning how to self-advocate is one of the hardest parts of infertility! In this "Skill Up" series I am hoping to equip you with the knowledge you need to have an educated discussion with your doctor. Obviously I am just a fellow infertility warrior, so please consult your doctor on all medical decisions.

    Femara is better. My wife had two miscarriages (12 week and 3 weeks) and later, going through many unsuccessful rounds of clomid, she underwent cleaning of uterus (HSG, I think) and next month she got pregnant. Maybe after your miscarriage your uterus is not clean and therefore pregnancy is not possible. Thanks to Allah, we have a baby now.

      Do anyone stop your period .I'm on my 2nd round of clomid 50mg and it stopped my period I took it days 3-7 my cycle are 28 day now I'm on cycle day 33 still no period and native pregnancy test.

    Great easy to understand video. Love the artistry to visually show the differences. I'm a man and my wife and I have been struggling with fertility issues for almost 4 years now. I recently got diagnosed with hypopituitary hypogonadism. I will begin Hcg & Clomid this week. I have a lot of knowledge on hormones both endogenous and exogenous so informative videos like this are highly appreciated. One thing I always wondered (which you explain in your video) is the mechanism of action Clomid has vs Letrozole. Clomid works on the brain and Letro works on the ovaries. My understanding is HCG works on the ovaries in women and testes in men similar to Letro. My question is I wonder if HCG is interchangeable with Letro since they both work on the hormone production front (opposed to the pituitary/brain front). I do not like the risks and side effects Clomid has so I have always tried to avoid Clomid. In the past I've taken Tamoxifen + Clomid together for TRT recovery purposes. Anyway, thanks for the video!

    Hey mam.can you pls clarify me 2doubts actual I have PCOS so doctor did hsg tube test and after hsg doctor gave metformin to use 3minths in the first month of metaformin u conceived but in 3month ending baby heart beat topped so doctor gave pills for abortion..so we are worried if I conceive next tym again this growth problem will come? And next .....so after that we gave 3months gap..so this month I got period on October 1st and took clomid for 5 days and follicular monitoring also so doctor gave hch injection on day 12 and my egg rupturd on day 14 so we are in contact from day 12,13 14 so when should I do pregnancy test on which day tell me

    I'm on clomid round 2...at round 1 on 13 cycle day follicles scan shows 2 mature follicles...at day 18 scan shows no eggs in ovary means both eggs released..but I was not pregnant in round 1.....what should be the cause ?

      That would be a great question for your doctor! My thought would be that you would fall into the Clomid range for twins though.

    Thank you for this video! I took my first round of femara this month 5 mg, I am waiting for my follicle to get a bit more mature ( ultrasound tomorrow) and then will take an ovidrel shot. I am so hopeful it hurts!

    I took only one shot of depo provera (September to December) . My period returned in February and March but very light. Can clomid help my cycle and consequently ovulate

      Depo can take up to two years to wear off completely. The majority of women need more than 3 months for their cycle to return to normal. I would use ovulation predictor kits(OPKs) to see if you are ovulating and chat with your doctor about how long you should wait to pursue fertility treatment. I think you should wait a bit longer before being too concerned. Best of luck!

    I have Pcos and Femara did not work at any dose for me then my doctor wanted me to loose weight and try femara again she refused to use clomid on me because she doesn't want multiple babies she said she doesn't prescribe it to hardly any of her patients. After that we just decided to give up.

    Hello mam I hope u see my message.I am 19 years old I have a regular period of 28-34days I was diagnosed with pcos on both ovaries my Gynecologist sent me blood test and all were okay 👍.until my Infection were 50 instead of 0-20 range she gave me some medication ❤And Prescribed clomid for me from day 2-5 100mg I hope it works For me the first round and Am 49kgs currently I actually don’t have Pcos symptoms like gaining weight it’s so hard for me gain weight.she diagnosed me with mild Pcos and said it’s not sever ❤️I hope clomid works and I conceive first month ❤️

    In PCOS women who struggle with high androgen symptoms, does Letrozole exacerbate that by raising androgens/testosterone? Also, how does it affect LH levels (which are commonly 3x higher in PCOS but flat-lined throughout the cycle)?

      For you first question I don't know the answer and would encourage talking with your doctor about. With medicated cycles you should still experience a LH surge. My doctor discouraged me from using at home tests since PCOS makes them virtually useless. Instead she confirmed ovulation with ultrasounds since it is more reliable.

    Hello I finished my treatment of femara (Letrozole) according to the doctor timing chart i am now ovulating i also test with the clearblue digital ovulation kit i got a {peak 😊} my question is ive been having lower pelvic pain area both side but most on my right side ! Is this what the medication supposed to be doing ? Is this normal sign that its working ?

      Some women do feel themselves ovulate and can even tell what side they ovulated from, so that might be the pain you are feeling. If it last more than a few days, I would bring it up with your doc. I have my fingers crossed for you.

    I’ve been taking clomid for 6 cycles, I ovulated successfully with it but no pregnancy. Tested for pcos but not detected. My doctor is going to have me try cycles of letrozole now. With the fact that I ovulated with no pregnancies on clomid, will letrozole possibly have a different result with getting pregnant even though clomid worked for ovulation?

      Confirm with you doctor, but I am guessing that they switched you because there is usually a 6 cycle limit on using clomid because of long term side effects and increased risks of serious side effects. If you didn't ovulate every time on clomid they may be experimenting to see if you respond better to letrozole as well. Best of luck!

      The difference isn't as impactful for endometriosis patients but I have heard that Clomid can compound with preexisting symptoms to make them much worse.

      I am not aware of this having any other side effects but that may be something you want to discuss with your doctor. I have heard women who have been prescribed it earlier like this. I think it comes down to doctor's preference/experience, but I will add it to my list of things to research.

The last time I took Letrozole I got pregnant but had a miscarriage at exactly 5 weeks, hoping this time around I will take progesterone after I ovulate and get my bfp 🙏

    I have been bleeding for over 2 months, doctor put me on progesterone 2 weeks ago and I took them for 10 days but the bleeding did not stop so the nurse informed me that they felt that I should stop taking the pills and he wanted my cycle day 1 to start this past Friday and Tuesday I am supposed to take Clomid, I am now bleeding lighter! But I still think I should take the Clomid what are your thoughts?

      Hmmm what kind of monitoring are they going to do with this Clomid cycle? Ultrasounds or just blood work? Also prior to starting Metformin did you have a high blood sugar? My OB put me on that event though I didn't have overly high blood sugar because that use to be standard practice. My RE promptly took me off it because more modern studies say it isn't as helpful if you don't have high blood sugar to start with.

      JAKS Journey ultrasounds and blood work he wants me to do opk’s until I get a positive so they can do the test to see if I ovulated. I forgot to mention I have pcos and he tested me for high insulin and my results were border line.

      You might want to mention to him that you have PCOS after this cycle especially if you don't get a clear positive OPK. What he has you doing sounds similar to what my OB did, but after one cycle with her I moved on to an RE. OBs just don't have the training in fertility treatment.

      Usually they cap the number of times you can to Clomid to six because there are concerns with taking it too long. I would talk to your doc about moving on to something else. Best of luck!

    I am also trying to learn about self-advocating for infertility. I was on my first round of Clomid in September 2018. We were just trying the Timed Intercourse route hoping it would be cheaper and get the same results. The downside of Clomid was I did end up with so many ovarian cysts. I was in so much pain. I asked for another medication expecting femera, but they placed me on Follistim instead for this month of October 2018. However, thank you for the very detailed information on the difference. I can rarely understand this stuff.

      Always consult with your doctor for medication dosage. The doctor should also be monitoring you via ultrasound to see if you are ovulating. The closed may or may not be working and ultrasound is the best way to tell. If it is working, it may take several months to get pregnant. Best of luck!

      Live birth is the term used to indicate that a pregnancy ends with a healthy, happy, take-home baby. On the other hand about 25-50% of pregnancies end in miscarriages. So the pregnancy rate of a drug or treatment will always be higher than its actual live birth rate.

      It is different for every woman and depends a little on the protocol, but you should ovulate somewhere around day 13 to 15. I have heard of women ovulating at soon as CD 10. My doc did an LH blood test around CD 17 to see if I had ovulated.

      @@madrinekariuki3339 Some clinics have different protocols but all the studies I reference follow the standard day 3 to 7 protocol. Chat with your doctor about what protocol they want you to follow.

      It’s so weird to be so young and the only one of your friends who is going through this process.
      All of my other friends are basically always complaining about how easy it is to get pregnant.

      @@alibentz8692 I totally understand. It felt so weird for me to be sitting in a fertility clinic at 26 when I had been cautioned all my life to not get pregnant.

      IRL JAKS exactly! I was adopted and my entire life my adoptive mom always told me things like “you don’t want to get pregnant and ruin your life like your birth mom right?” So I stayed on the pill faithfully taking it every day, and now we are having a hard time!

      It doesn’t look like it worked, we didn’t do ultrasounds but I did take ovulation tests everyday from days 9-17 the line got darker, but never dark enough to be positive.

    Hello, how long should you usually take(femara) , just those 4 or 5 days or then you take it for other times? I read that it causes pain, weight gain and hair loss, and this is true? What about me? Hello and thank you

      Make sure you are monitored by a doctor while taking Femara. Usually they just prescribe it for 5 days but which days changes based on the doctor. All drugs have side effects; reference to the manufacturer's website for a full list. Usually side effects are mild because you are only taking it for such a short time.

      I also did not ovulate with just oral meds. I moved on to a reproductive endocrinologist to add injectable meds.
