"Taking the 5th" is invoking the right to remain silent (the 5th Amendment of the US Constitution), but commonly said even in casual conversations when you don't want to answer a question, or want to maintain your privacy.
It is funny that the therapist was saying it in the common conversational sense of the term, but to a mobster, people who are known for literally invoking the right legally.
That ending scene of Anthony looking at Tony during the funeral is one of my favorite scenes, it's so beautiful and I feel like it says so much with no dialogue

Especially with AJ’s blank stare. Says AJ isn’t very smart. Says he doesn’t really have a problem with his dad being a mobster. AJ is putting the pieces together on why Jeromy & others fear him & why his teachers car was sort of returned to him.
The shot of the mourners at the funeral and then pulling focus to Meadow looking at A.J. was brilliant.
2:57 Michael Andretti was a racecar driver. AJ stands for Anthony Jr. not Andretti Jr.
Tony's referencing his much more successful father, the legendary Mario Andretti. Not his son Michael who's only know by motorsports enthusiasts

When Tony calls AJ "Andretti" he's referencing Mario Andretti. AJ has the same name as his dad (hence "JR")
I hope you caught Tony using what he learned from therapy (about letting elders think they're in control) with the Junior situation.

ALL shrinks see other shrinks. It's part of the professional. They have to unload all the negative energy they get from the patients.
Most shrinks have serious mental issues which is why they went into the profession in the first place.
@@jesses5463 No. My dad was a psychiatrist. He is a normal, smart guy. I met several of his peers before he retired. They were no different than other people. Furthermore, many "serious" mental illnesses pop up when patients are college age. How would such a person get through medical school?

It's the fifth amendment to our constitution. It's a constitutional right to not answer a question.

2:57 ... slight misunderstanding, Andretti is not AJ's real name, it's the name of a very famous family of racers in the US. Mario Andretti being the most famous through the 70's, 80's and early 90's.

The entire show is ultimately based on the premise of Tony trying to keep his 2 families separated as much as possible so one doesn’t destroy the other. Tony’s kindred family vs his mob family in the main conflict in the show.
How much does Meadow know or AJ or Carm is something that’s always a problem for Tony. Even though they think they may know things... they really don’t know much. We the audience see it all pretty much. We see how both of Tony’s families really work.

Regarding the comments at 19:57, it has sadly become way too common (at least in America anyway) for the process to begin with prescribing meds and then following up with actual therapy.
