I have one question.
in the fgets(str,5,ptr)
if we defined a string as str[4] size and asking for a value return of size 5 or more still it is not showing any warning or error message.
So, how does we explain this?
if you attempt to read more characters than the size of the array can hold, you will encounter undefined behavior. fgets does not independently verify buffer overflow its your job tto check it

IN w+ mode, how can we use fgets, fgetc to read input from file,infact w+ clears all the content in the file , then there is nothing left in the file so there would be no effect of fgetc or fgets functions. sir my doubt is , your table says opposite to this , how can we use w+ to do all this sir??
Agar hum writing ke liye koi function use karenge w+ mode toh sabkuch delete hoke likhna start lekin agar reading ke liye koi function use karenge toh delete hoga....
