Few days back I treated a grade 3 dog bite , rabies monoclonal infiltration given and day 0 vaccine , thanks for choosing essential emergency topics needed in our country and explaining clearly , kudos 👍👏👏
Sir....there is a street dog that mostly stays at my home .so in January she scratched me and i went to a doctor who gave me 3 doses 0 3 7...after a few months in may ..he again strached me mildly and so i took 2 doses 0 and 3 ....after listening to this video i don't know what to do ...cant even do antibody titres ...the dog is fine and healthy...what should i do
1. sir what if you miss the ARV doses by few days ? do you have to repeat the entire schedule? 2. how long is the tetanus toxoid valid? when to repeat the vaccine?
Sir, @11:00 Rabies Antibody Titre Test is available. for Previously Vaccinated Persons.. Elisa ( lgG) Test. Where if the person has an antibody level above 0.5 Eu/mL (as per WHO Guidelines) . They are considered to have sufficient Antibodies against the virus... Recommended for people who are at high risk of exposure such lab workers, veterinarians, animal handlers etc.
What to be done if a person who has taken complete PEP and HRIG in the past, later got category III dog bite after 3years? Should we give the complete prophylaxis and Ig again or just two vaccine(0,3) doses are sufficient
What to be done if the pt is already took ARV due to dog bite in childhood, later after 20ys he had another episode of dog bite after age 25, vaccinated, and again after 6months
If re-exposed, persons who have previously received full pre-exposure or post-exposure treatment (either by IM or ID route) vaccine should be given for only two booster doses, intramuscularly/intradermally (0.1 ml at two sites) on days 0 and 3, but no rabies immunoglobulin. Only adequate wound washing would be required in case of re-exposure where animal bite victim has documented proof of complete PEP or PrEP within last three months. People who have previously received full post- exposure treatment with neural tissue vaccine (NTV) or vaccine of unproven potency or cannot document complete previous PEP or PrEP treatment should be treated as fresh case or given full PEP.

Vaccinated needs clarity??
Repeat single site id injection on D0 and D3.

Why the immunoglobulin is given distally nd not proximally
RIG should not be administered in the same syringe or at the same body site as vaccine since RIG may inactivate the vaccine
